Evaluation of CA 15-3 serum levels in breast cancer patients.

The usefulness of post operative serum CA 15-3 determination was evaluated in a group of 57 breast cancer patients. CA 15-3 was assayed at baseline and every 3 months for a median follow-up of 34 months. Recurrence-free survival probability for patients with elevated serum CA 15-3 levels before surgery was 53.5% versus 88% for patients with normal serum CA 15-3 levels (p < 0.201). In 12 of the 15 patients with increased trend for CA 15-3 during follow-up, elevated CA 15-3 levels preceded (10 patients) coincided (2 patients) with the clinical detection of tumour recurrence, providing a predictive value of an increased trend of 80%. In the group of 10 patients who showed elevated serum CA 15-3 levels prior to the detection of recurrence, the median load-time was 5 months/(range 3-11 months). We concluded that CA 15-3 had prognostic value in breast cancer. Serial determinations of CA 15-3 was consistent with the treatment results.

Serum antithrombin III and alpha-2-antiplasmin concentrations in patients with Hodgkin's disease in the course of chemotherapy.

Disturbances in hemostasis in cancer disease frequently occur. This clinical situation may contribute to the spread and metastasis of cancer. The determination of inhibitors controlling hemostasis especially during treatment may be of value for the evaluation of effectiveness of treatment and balancing of hemostasis. Twenty four patients diagnosed with Hodgkin's disease were introduced to our studies. The level of antithrombin III (AT-III) and alpha-2-antiplasmin (alpha-2-AP) in sera were determined before, during and after treatment with MOPP. We found decrease of AT-III concentration before treatment, and chemotherapy increased the level of this inhibitor but still below the level of control. Alpha-2-AP concentration was higher in patients and MOPP application decreased the level below the values found in controls. The determination of concentration of both inhibitors may be important for the evaluation of the effect of treatment in understanding how the disturbances of hemostasis may be equilibrated.

[Granulocyte acid phosphatase activity in patients with Hodgkin's disease].

In 24 patients with Hodgkin's disease the activity of acid phosphatase was determined in granulocytes. Its level was only half that in the control group. The causes of this low activity of this enzyme may be increased number of cells without it, exocytosis, and possible influence of substances secreted by the neoplasm. It is suggested that the determination of this activity may be indirectly useful for verification of the efficacy of the antibacterial mechanisms, and also of antitumour resistance of the host.

The activity of neutrophil myeloperoxidase in patients with Hodgkin's disease.

Neutrophil myeloperoxidase activity has been studied in twenty one patients diagnosed with Hodgkin's disease. The presented data indicate no differences in total MPO activity, whereas we observed some differences in the percentage of granulocytes with different degree of scores. Changes in the intensity of reaction may indicate the possibility of exocytosis as a mechanism releasing MPO from the cell to the surrounding area. In peripheral, circulating neutrophils, such a phenomenon seems to be of no avail and may disorganise anti-cancer defence.

The activity of neutrophil myeloperoxidase in patients with lung carcinoma.

MPO activity in the neutrophils has been evaluated in 25 patients with lung carcinoma. Surprisingly for us we observed no differences between patients and controls in the total MPO activity. However, we noted the increase in the percentage granulocytes with the first and fourth degree of scores in cancer patients in comparison to normal adults. The highest activity was characterized by the distribution of this enzyme not only in granules but also outside them. Any changes in the total activity of MPO may reflect the balance between the factors which may stimulate it as tumor promoters enhancing the production of superoxide anion or the inhibitory effect of plasma. Diffuse enzymatic reaction may be due to some functional abnormalities of phagocytosis or membrane damage by the MPO system. The role of MPO in anticancer mechanisms is briefly discussed.

Serum concentration of alpha-2-macroglobulin, alpha-1-antitrypsin and alpha-2-antichymotrypsin in patients with lung carcinoma.

Serum concentration of alpha-2-macroglobulin, alpha-1-antitrypsin and alpha-2-antichymotrypsin was evaluated in 26 patients with lung carcinoma. We observed an evident decrease in alpha-2-M and alpha-1-antitrypsin level and no differences between tested and control groups in alpha-1-antichymotrypsin concentration. The deficiency of protease inhibitors may be due to the increased level of protease activity in malignant cells. Infiltration of granulocytes near tumor and released enzymes from them may exhaust proteolytic inhibitory capacity, too. Increased protease activity is associated with transformation and uncontrolled proliferation, therefore antiproteases may be accepted as anticancerogenic factors. Further investigations are needed to bring us closer to understanding this question.

Serum lysozyme activity in patients with Hodgkin's disease undergoing chemotherapy.

Lysozyme activity was determined in undiluted and diluted sera of 41 patients suffered from Hodgkin's disease and of 40 healthy individuals. Blood was collected before chemotherapy, midway of its course, just after completion of chemotherapy and three weeks and three months later. Lysozyme activity was evidently increased as well in undiluted as in diluted sera in all our tested patients. These findings may be interpreted by the presence of immune complexes in sera which may stimulate the release of lysozyme from viable neutrophils. High activity in diluted sera probably is related to the increased concentration of lysozyme inhibitors. The applied chemotherapy did not significantly change the activity of the lysozyme during its course of even after its completion as found by three-month observation. The enhanced lysozyme activity in sera of patients may be at least in part to compensate decreased antibacterial and anticancer mechanisms.

Nitroblue tetrazolium test in patients with Hodgkin's disease in the course of chemotherapy.

In 41 patients with Hodgkin's disease nitroblue tetrazolium (NBT) dye reduction with resting and stimulated granulocytes, before, in the course and after chemotherapy was examined. Twofold decrease of the capability of NBT reduction test with stimulated granulocytes was found, whereas with resting granulocytes only slight diminution was noted. Chemotherapy did not improve the tested parameters. No relationship was observed between the stage of disease and the presence of systemic symptoms and the NBT dye reduction. It can be assumed that granulocytes from patients with Hodgkin's disease are resistant to stimulation. The observations suggest that NBT test may be useful for evaluation of the function of granulocytes and to some extent for monitoring of the chemotherapy.

Nitroblue tetrazolium test in patients with breast cancer undergoing mastectomy and radiotherapy.

The reduction of nitrotetrazolium blue with stimulated and unstimulated granulocytes was performed in 40 patients with breast cancer. Blood was collected from peripheral vein before surgical intervention, during surgery from vein draining of tumor, two weeks after surgery and three months after surgery when radiotherapy was finished. In parallel experiments the NBT test was made in the peripheral blood of 20 healthy individuals. We have observed decreased reduction of nitrotetrazolium blue in stimulated granulocytes, which may indicate that granulocytes from these patients are unable for efficient stimulation. No evident effect concerning the advancement of disease and applied therapy could be found. The importance of determining the NBT test in cancer patients is briefly discussed.

Serum lysozyme activity in patients with lung carcinoma subjected to radiotherapy.

Lysozyme activity was determined in undiluted and diluted sera of 30 patients with lung carcinoma and of 40 healthy individuals. Blood was collected before radiotherapy, in half of its course, just after completion of radiotherapy, and three weeks and three months later. In the controls the same time spacing was kept. Lysozyme was found to be increased both in undiluted and diluted sera. Enhancement of the lysozyme activity may be due to the presence of immune complexes (antigen--antibody) which are probably responsible for the release of the enzyme from the viable neutrophils. High activity in diluted sera indicates an increase of lysozyme inhibitors in sera of cancer patients. Radiotherapy has not induced significant changes in the enzyme, either during its course of after its completion as confirmed by three-month observation. Taking into consideration the role of the lysozyme in anticancer mechanisms it appears that determination of its activity may be of value in the course of cancer disease.

Serum lysozyme activity in patients with breast cancer treated by mastectomy and radiation.

In 37 patients with breast cancer, lysozyme activity in undiluted and diluted sera was determined. Blood was collected from peripheral vein before surgical intervention, during surgery from vein draining when radiotherapy was finished. In parallel experiments the lysozyme activity was tested in peripheral blood of 40 healthy persons. Increased lysozyme activity both in undiluted and diluted sera was observed and radiotherapy did not change the activity of the tested enzyme.

Serum concentration of alpha-2-macroglobulin, alpha-1-antitrypsin and alpha-1-antichymotrypsin [correction of alpha-2-antichymotrypsin] in patients with Hodgkin's disease.

In twenty four patients diagnosed with Hodgkin's disease we have determined several antiproteases. Our findings have shown the decrease of concentration of alpha-2-macroglobulin and increase of alpha-1-proteinase inhibitor and alpha-1-antichymotrypsin. The observed changes were statistically significant p < 0.001. We did not find any correlation between the concentration of inhibitors and the clinical stage of carcinoma and histopathological data. The determination of the level of antiproteases in patients with cancer disease may be assumed to reflect partly at least the capability of anticancer mechanism in the host.
