[Hyperthyroidism in TSH-producing hypophyseal adenoma].

HISTORY AND CLINICAL FINDINGS: For 5 years a 59-year-old man had been treated with thyrostatic drugs for hyperthyroidism of unknown aetiology. As he had been losing weight, computed tomography had been performed in the search for a malignancy. It revealed a hypophyseal space-occupying lesion. INVESTIGATIONS: Peripheral thyroid hormone activities were raised, while thyroid-stimulating hormone (TSH) was also raised to 12.4 mU/l and there was evidence of partial hypophyseal insufficiency. Octreotide scintigraphy demonstrated increased activity in the area of the hypophysis. TREATMENT AND COURSE: The transsphenoidal surgery and immunohistochemical tests confirmed the diagnosis of TSH-producing hypophyseal adenoma. The patient became euthyroid after the operation. CONCLUSION: TSH-producing hypophyseal adenoma should be considered as a rare cause of hyperthyroidism when it is combined with non-suppressed TSH.

[Bilateral laparoscopic transperitoneal adrenalectomy in pheochromocytoma].

Laparoscopic transperitoneal and endoscopic extraperitoneal adrenalectomy are two safe options in minimally invasive surgery associated with a very low morbidity. Comparative studies with the conventional access to the adrenal gland demonstrated the advantages of the endoscopic technique. The anterior transperitoneal approach yields a better exposure of the anatomic structures and allows the surgeon to orient himself more easily, while at the same time he may perform additional laparoscopic maneuvers. In two cases of bilateral pheochromocytoma a bilateral laparoscopic adrenalectomy was performed simultaneously by employing the transperitoneal approach. The duration of surgery was approximately 210 and 270 min, respectively, with an intraoperative blood loss of about 350 and 400 ml. There were no complications following this procedure. Already on the 1st postoperative day, the patients could be fully mobilized. Furthermore, immunological data obtained perioperatively support the minimal invasiveness of this technique.

Realistic modeling of clinical laboratory operation by computer simulation.

An important objective of laboratory management is to adjust the laboratory's capability to the needs of patients' care as well as economy. The consequences of management may be changes in laboratory organization, equipment, or personnel planning. At present only one's individual experience can be used for making such decisions. We have investigated whether the techniques of operations research could be transferred to a clinical laboratory and whether an adequate simulation model of the laboratory could be realized. First we listed and documented the system design and the process flow for each single laboratory request. These input data were linked by the simulation model (programming language SIMSCRIPT II.5). The output data (turnaround times, utilization rates, and analysis of queue length) were validated by comparison with the current performance data obtained by tracking specimen flow. Congruence of the data was excellent (within +/- 4%). In planning experiments we could study the consequences of changes in order entry, staffing, and equipment on turnaround times, utilization, and queue lengths. We conclude that simulation can be a valuable tool for better management decisions.

Determination of uric acid on continuous-flow (AutoAnalyzer II and SMA) systems with a uricase/phenol/4-aminophenazone color test.

This report describes a new specific colorimetric procedure for uric acid assay with AutoAnalyzer II and SMA (Technicon) systems, made specific by the application of uricase. Hydrogen preroxide, formed in this reaction, effects the oxidative coupling of 4-aminophenazone and 2,4-dichlorophenol under the catalytic influence of peroxidase. The red dye formed is measured at 505 or 520 nm. A sample blank measurement is not necessary, and the reagents show very good stability. The test shows linearity up to 714 mumol of uric acid per liter. Results of thie method correlate very well with those by the uricase-ultraviolet and uricase--catalase methods. There is no interference by hemoglobin, bilirubin, lipemia, and various drugs, except a minor interference by alpha-methyldopa. Interference from ascorbate is eliminated by ascorbate oxidase. This method can be regarded as a considerably improved routine test for uric acid on continuous-flow systems in clinical laboratories as compared with the commonly used phosphotungstate method.

[Enzymatic determination of total cholesterol with the Greiner Selective Analyzer (GSA-II) (author's transl)].

A fully enzymatic method to determine total cholesterol in serum is described. The method appears especially suitable for adaptation to discrete mechanical analyzers either of the single channel or the multi-channel type. The method uses the enzymes cholesterol esterase (EC 3.1.1.13), cholesterol oxidase (EC 1.1.3.6) and peroxidase (EC 1.11.1.7) with 4-aminophenazone and phenol as substrates in the indicator reaction. The method was adapted to the Greiner Selective Analyzer GSA-II. For this purpose the critical parameters of the reaction were intensively examined. The complete reagent is stable within the GSA II dispenser at 4 degrees C for at least 1 week. By omitting cholesterol oxidase in the blank reagent a sample bland and a partial reagent blank are obtained. Within a range up to 10.4 mmol/1 (4.0 g/l) the maximum colour is developed within 6 minutes. The calibration factor was stable for 4 months. The method allows absolute measurements. At concentrations between 2 and 4 mmol/1 within-batch precision ranged from 0.5 to 1.4%. Precision from day to day for the same control sera amounted to 2.8; 2.0; 2.7 and 2.0% for a period of 3 months. Examination of accuracy yielded satisfying results. Ascorbic acid in the physiological range did not alter results to a significant extent. Catalase or novaminesulfone added in vitro did not interfere. Optical interferences by bilirubin, hemoglobin or turbidity are compensated by a sample blank. A comparison of results with the enzymatic method of Roeschlau et al. (Z. Klin. Chem. Klin. Biochem. 12, 226 (1974)) yielded satisfactory agreement. The limits of detection of the present method can be lowered by a factor of 2.2 by replacing phenol by dihalogen phenols.

A study of inflatable cuffs on endotracheal tubes. Pressures exerted on the trachea.

A simple method of measuring the pressure exerted by the cuff of an endotracheal tube on the trachea is described and has been used to measure the pressures exerted by 16 commercially available cuffs on the wall of a model trachea. The Shiley, Portex soft-seal, Kamen-Wilkinson (Bivona Fome) tubes had the lowest tracheal wall pressures. Using this method in vivo the changes in tracheal wall pressure exerted by a low-pressure cuff during percussion and vibration physiotherapy, and when the patient "fights the ventilator", were recorded.
