[Hemorrheological changes in microcirculation caused by fats of different dosage, composition and mode administration].

Descriptions of the nature and pathogenesis of disturbances of the microcirculation induced by intravenous infusion of a triglyceride emulsion or by daily oral administration of a cholesterol-in-oil suspension are different and partially contradictory in the medical literature. Hyperlipemic disturbances of the microcirculation are characterized by a general decrease in the rate of flow reflected in all parameters of flow in the terminal blood vessels including a rise of the number of capillaries temporarily functionally isolated from the circulation and by an increase in capillary plasma skimming. The degree is dependent upon the amount of administered fat and on lipid levels. The disturbed capillary perfusion can be attributed to various rheological mechanisms. The hemodynamical relevance of the short-time hyperlipemia produced by intravenous infusion of a fat emulsion and the enduring hyperlipemia produced by the daily oral administration of a cholesterol-in-oil suspension differ. Some of the capillary-rheological bases of these differences are discussed.

[Correlative pathologic studies on the role of vitamin D in vascular calcinosis in childhood].

The large arteries (aorta, common carotid arteries with the adjacent segments of the internal and external carotid arteries, common, internal and external iliac arteries) of 64 children and young adults were sectioned longitudinally and removed with adjacent tissue. The macroscopic demonstration of calcium deposits was performed by using the Kossa reaction modified by Meyer and Stelzig (16). The macroscopically positive cases were selected for histology, fixed in formalin and embedded in paraffin. The total material comprised 4 premature infants, 6 mature neonates, 8 babies aged 1 to 12 months, 13 children aged 1 to 5 years, 12 school children between 6 and 13 years, and 21 young adults between 15 and 30 years. Calcium incrustations are visible in various arterial segments (in 59 of the 64 cases examined) as black dots, stripes, patches or polygonal figures. Extent and density of calcifications display differences not only from case to case but also from segment to segment. Contrary to the literature, disseminated calcinoses were even detected in some of our prematurely born babies. The severity of calcifications increases with increasing age. Calcium incrustations in mature neonates do not appear exclusively in the common and internal iliac arteries, but also in the other arteries examined. The least pathological changes consist in a focal disintegration, calcium impregnation and circumscribed fragmentation of the lamina elastica interna. In advanced cases, the inner layers of the wall show additional compact calcification foci of different size. We were afraid that the high single doses of vitamin D, within the frame-work of rickets prevention, may cause calcium deposits in the arteries of children. A clarification of these problems might be accomplished by comparative studies of the population in countries where rickets prevention is not practised by application of such high doses.

[Pathogenesis and hemodynamic significance of veno-arterial reaction in diabetic microcirculation disorders].

In human diabetes mellitus, functional microangiopathy starts with venule dilatation, the cause of which is unknown. The results of comparative measurements of animals with alloxan diabetes verify also an increase in the arterio-venous calibre relation. As studied vital-microscopically, these alterations are due to venule congestion and passive dilatation as a consequence or red cell aggregation and an increase in the viscosity of blood. The increase of the flow resistance on the postcapillary side depends only on rheological but not on nervous-humoral factors, which leads by means of a veno-arterial reaction to increased constrictions of the arterioles.

[Capillary hypoperfusion in diabetes mellitus: a comparative microscopic study on its pathogenesis and temporal manifestation in in vivo animal experiments].

In animal experimental alloxan diabetes changes the distribution of microcirculation with reduction of the nutritive capillary perfusion and simultaneous formation of hyperperfused functional microshunts. The nutritive capillary hypoperfusion is due to an increased precapillary vasoconstriction and a changed rheological behaviour of the blood with increase of the cohesive forces between the erythrocytes (aggregation of erythrocytes) and the intravascular cellular swellings.
