[Osteochondroma of the mandibular condyle. Apropos of a case].

The authors report a case of mandibular condyle osteochondroma on a 33 years-old woman, similar to osteocartilaginous exostosis of the long bones. The lesion was revealed by a facial asymmetry and a change of the occlusion. The diagnosis was performed on the computed-tomography examination and histopathology. The removal of the lesion has conserved the condyle without recurrence after 20 months following-up. The authors remind the uncommonly location of osteochondroma in the facial skeleton and discuss the histogenesis of this lesion in its mandibular condyle site.

[Primary malignant non-Hodgkin's lymphoma of the salivary glands. An anatomo-clinical and immunohistochemical study of 2 cases and a review of the literature].

The authors reports two cases of primary non Hodgkin's lymphomas of the right submandibular gland in a 50 years old woman, and of a minor gland in another woman also 50 years old. The diagnosis was difficult for the second case, and was established on the immunochemistry study. The authors discuss the epidemiological, clinical, radiological, pathological features of this neoplasm and its management.

Burkitt Lymphoma↗

[Solitary plasmacytoma. Apropos of a case of craniofacial involvement].

The authors report the case of a 24 years-old man who had a first tumor in the right maxilla diagnosed as Ewing's sarcoma and treated by chemotherapy and radiotherapy. A second tumor appeared in the skull vault twenty seven months later. The histological diagnosis after removal of the lesion was IgG Lambda plasmacytoma. The revision of the histological cut of the maxillary tumor retained the same diagnosis. The patient is without evidence of local recurrence and systemic diffusion of the disease two years after treatment. The authors insist on the necessity of immunochemistry to establish the diagnosis.

[Malignant lymphoma of the mandible].

A 73 year-old man consulted for a swelling of the endentulous horizontal branch of the mandible. X-ray examination showed a lytic defect. Two successive, but superficial biopsies, directed the diagnosis towards osteomyelitis. In spite of antibiotic therapy, the destruction continued to progress and was finally complicated by a pathological fracture. A radial resection of the horizontal branch of the mandible was performed; on this surgical specimen the diagnosis of primitive lymphoma of the mandible (diffuse centroblastic type) was made. The authors discuss the diagnostic difficulties of mandibular lymphomas, which according to Ostrowski represent 4.7 per cent of the primitive bone lymphoma.

[Primary malignant non-Hodgkin lymphomas of the digestive tract: the role of surgery].

Forty-two patients with non-Hodgkin's malignant lymphoma (NHML) of the gastrointestinal tract were operated on between 1971 and 1989. NHML was located in the stomach in 23 (55%), in the small bowel in 16 (38%), and in the colon in 3 (7%). One-fourth of patients were hospitalized with an acute abdomen and had to undergo emergency surgery. Preoperative diagnosis was obtained in 40% of cases only. Since 1986, however, and thanks to immunohistochemistry, gastroscopy was able to diagnose gastric NHML in 10 of 11 cases. Twenty-six patients (62%) underwent curative surgery. Operative mortality was 11.9%, 42% in case of emergency surgery, but mortality was nil after elective procedures. Half the patients had no further treatment. Five-year survival was 67%. Except for the mucosa associated lymphoid tissue (MALT) tumors, histopathological characteristics had less influence on prognosis than expected. On the other hand, prognosis was significantly better after curative surgery (p less than 0.001).

[Pseudo-tumoral xanthogranulomatous pyelonephritis. Apropos of a case].

The authors report the case of a 20 year old patient in whom the clinical findings combined with the radiological signs, suggested the diagnosis of renal cancer. The histological findings after nephrectomy established the diagnosis of xanthogranulomatous pyelonephritis. This disease may rarely give the appearance of a renal tumor. This points out the importance of identifying this lesion in preoperative staging to avoid carcinologic surgery and all of its difficulties.

[Osteomyelitis of the jaw and osteopetrosis].

Osteopetrosis (Albers-Schönberg disease) is a rare sclerosing bone disorder in which osteomyelitis of the jaws is a frequent complication. Treatment of osteomyelitis is difficult and may lead to large resection. We report three new cases and we review the literature.

[Intramedullary D12-L1 epidermoid tumor. Report of a case and review of the literature].

The authors report, after a follow-up period of two years, a case of intramedullary congenital epidermoid cyst treated surgically. The patient, a 21 year-old man, presented with a seven year history of motor disturbances of his left lower limb and more recent urinary incontinence. The diagnosis of an intramedullary tumor was provided by a myelogram and a post-myelogram CT. In the absence of a dermal sinus or vertebral dysraphic anomalies, the epidermoid nature of the mass was an operative and histological finding. The main data from the literature are commented: frequency, topography, clinical diagnosis, and radiologic or M.R.I. imaging. Intracapsular removal of the mass seems to result, as in the presently reported case, in a satisfactory outcome.

[Chondrosarcoma of the mandible. Clinicopathological study of a case].

The authors report a new case of chondrosarcoma of the mandible in a 24 year-old female patient. They insist on the rareness of this entity and on the difficulties in establishing this diagnosis from the clinical, radiological and pathological points of view. Elements of prognosis are discussed through a review of the literature.

[Melanoma of the vagina. Apropos of a case and review of the literature].

We describe the unusual case of a 60-year-old woman with malignant melanoma of the vagina and positive right inguinal lymph nodes. Etiopathogenesis of this lesion, prognostic factors, and therapeutic attitude, are discussed in the light of data from the literature and the case reported herein.

[Exogastric cystic schwannoma. Observation of a case].

The authors report a case of giant gastric schwannoma with dominating extra-gastric development and a cystic component simulating a pancreatic pseudo-cyst on ultrasound and computed tomography examination, the diagnosis was made only at surgery that consisted in total gastrectomy. The rarity of these tumors, their diagnostic difficulties and the particularities of their surgical treatment are outlined.

Pulmonary blastoma. Immunohistochemical and ultrastructural studies of a case.

The immunohistochemical and ultrastructural features of a case of pulmonary blastoma in a 39-year-old woman are documented. Three types of cells were observed: epithelial, mesenchymal and 'blastomatous'. The latter stained with intermediate filament antibodies similar to those of mesenchymal and epithelial elements, thus supporting the hypothesis that this tumour is derived from a pluripotent undifferentiated element.
