[A complication of amniocentesis during second trimester of pregnancy. Case report].

From January 2002 up to June 2003 year we performed 19 amniocentesis during second trimester of pregnancy, upon medical indications. There was only one complication--flow out of amniotic fluid after the amniocentesis. We adopted the conservative treatement--bed rest and antibiotics: the flow stopped 18 days after the amniocentesis and te pregnancy reached uncomplicated termin with a healthy newborn.

Three novel point mutations causing haemophilia A.

Haemophilia A is an X-linked bleeding disorder caused by mutations in the factor VIII gene. In our efforts to elucidate molecular defects in the haemophilia A patients from the Republic of Macedonia, we employed nonradioactive single-strand conformation polymorphism analysis followed by direct sequencing, for identifying point mutations in the factor VIII gene. In the present study we report the detection of three novel missense mutations: Met19 --> Arg; Ala78 --> Pro and Cys2174 --> Gly, all causing haemophilia A.

[Analgesia in traumatism in childhood].

The current trends of pain treatment from anesthesiological viewpoint mark considerable progress. The ever increasing incidence of injuries in childhood (traffic accidents and home injuries alike), the different mechanism involved, and the multiple damage to organs and systems make mandatory prompt assessment of the condition, diagnosing and adequate therapeutic approach. Analgesia as an essential element of anesthesia is routinely applied to pediatric trauma cases.

[The intensive treatment of children with laparoscopies after ileus peritonitis].

Experience gained with the treatment of children undergoing laparostomies, necessitating numerous anesthesias and operative revisions in the early post-operative period, is shared. The open abdominal cavity is the cause of additional loss of liquids, requiring in turn adequate compensation with electrolyte solutions. Early administration of parenteral feeding contributes to prompt postoperative recovery. The active complex management, including antibacterial and immunostimulating therapy, as well as application of hyperbaric oxygenation in the event of severe infection, lead to a favourable outcome.

[Subphrenic abscess and suppurative pericarditis as complications in children with perforated appendicitis].

The combination of subphrenic abscess with purulent pericarditis is a rare postoperative complication of perforated appendicitis in children, with severe clinical course, difficult to diagnose and high case fatality rate. A 7-year-old child with this complication, successfully diagnosed and treated is reported. The importance of complex therapy--surgical and intensive and collaboration between pediatric surgeons, anesthesiologists, cardiac surgeons and cardiologists for the favourable outcome is emphasized.

[A case of chemical burns of the tracheobronchial tree in childhood].

A rare case of chemical burn of the tracheobronchial tree with powdered NaOH in a child is reported. In the course of the disease there were several periods of acute respiratory failure (ARF), requiring respiratory resuscitation with intubation, mechanical ventilation, aspiration and kinesitherapy. The chemical burn was followed by several hours of imaginary well-being, but subsequently pulmonary edema developed. On the fifth day obstruction of the air passages by secretions and mucosal hyperemia led again to acute respiratory failure. On the 15. day bleeding from the mucosa of the respiratory and gastrointestinal tracts developed, which incited again ARF. Such periodic deteriorations, leading to ARF after chemical burns, have been reported in the literature. They require recurrent respiratory resuscitation of different degree and form. Knowledge of these period in the pathologic process, leading to ARF and the adequate respiratory resuscitation are prerequisite for a favourable outcome of the morbid process.

[Determination of the minimal times for safe work following the Sayfos spraying of tobacco].

The semidisintegration period of Sayphos over tobacco leaves is determined--2.5 days at angular coefficient of the disintegration straight line K=0.12 1 ppm/ 1 mg/kg concentration of Sayphos over tobacco leaves is considered as harmless and accordingly safe for work. A table for minimal terms of safe work computation is also suggested. Because of the slow Sayphos decomposition in soil, the preparation should be employed but once--after the planting of tobacco.
