Acute suppurative renal inflammation.

Diagnostic and therapeutic aspects of acute suppurative renal inflammation are discussed in connection with 374 cases. Despite negative X-ray and laboratory findings, patients were submitted to operative exploration and 27% of them revealed subcapsular abscesses. Anamnestic facts (fever, shiver, etc.) and closely observed progression in the patient's morbid condition are factors to indicate surgery. Better results from the second part of the observation period (1981-1989) are due partly to better timing of the surgical intervention, partly to modern, highly effective chemotherapy. The mortality rate of acute suppurative renal inflammation dropped from 11 to 3.5%.

Fournier's gangrene. Experience in the treatment of gangrenous inflammation of the male genitals.

Four cases of acute gangrenous inflammation of the male genitals are discussed, after a brief review of the literature, with reference to the authors' own experience. In addition to dermal and fascial necrosis the disease causes severe sepsis and shock. Characteristics include rapid progression, occasionally in a matter of hours, and high rate of mortality. Attention is called to the importance of early surgical intervention and to the difficulties in combatting the septic shock.

Primary retroperitoneal tumours, in the material of our department.

After giving a brief survey of the literature, the authors discuss the symptomatology, diagnostics and management of retroperitoneal tumours. Four of their own cases treated between 1979 and 1985 summarized in tables are presented. All of them were operated and three of them received irradiation therapy. The fate of the first patient is unknown. The second patient survived for 24 months following the first operation, with additional three operations and irradiation on two occasions for recurrence of tumour. The third patient survived for 5 months. In the fourth, it could be revealed by ultrasound and angiography that the retroperitoneal tumour was independent of the kidney. Thus, the intact left kidney was preserved during operation. The patient died three months postoperatively at the department of pulmonology. The histological finding of the tumours of all patients was a malignant tumour of mesenchymal origin, a type of sarcoma. Based on their experiences, the authors stress the importance of the early recognition and removal of the tumour, since the patients have a poor chance of surviving these extremely malignant sarcomas in spite of the surgical and irradiation therapies.

Successful pregnancy after surgical repair of vesico-uterine fistula.

After a survey of the pertinent literature, a brief account of the signs and symptoms, diagnosis and therapy of vesico-uterine fistula is given and two cases are reported. In the first case hysterectomy had to be done because tissue destruction was too extensive for organ saving surgery. In the second case the extent of the lesion was compatible with preservation of the uterus. Healing was smooth, and free from complications in both cases. In Case 2, five years after operation the patient gave birth to a healthy female child of 3750 g weight. It is emphasized that in surgical repair of vesico-uterine fistulae preservation of the uterus should be sought.

Echinococcosis of the kidney.

A case of renal echinococcosis, most uncommon in Hungary, is reported. The problems of diagnosis and therapy are discussed. It is emphasized that difficulties of diagnosis, particularly differential diagnosis, arise mainly in countries where the incidence of echinococcosis is low. It is with the aid of the new diagnostic methods that an echinococcus cyst has been identified preoperatively in an occluded kidney which had been out of function for years.

[Urologic complications of pelvic fractures].

In 37 of the 258 patients with pelvic fractures rather severe urological complications were diagnosed. It seems that the majority of polytraumatized patients can be considered as potential urologic cases. The characteristic urological complications associated with the various types of fractures are described. The importance of urgent urological examination is stressed in order to detect urological injuries requiring acute care and to determine the need for operation or for conservative treatment. Co-operation between the traumatologist, the urologist and the radiologist will ensure the definitive care of the polytraumatized patient and the best chances for recovery.

Treatment of acute suppurative nephritis.

The diagnostic problems and treatment of acute suppurative nephritis are discussed on the basis of 146 patients. The biological properties of the pathogens isolated from urine and their manifestation with clinical symptoms are discussed. The role of incorrectly prescribed antibiotics in changing the characteristic symptoms and thus making diagnosis difficult is stressed. It appears from the material that in 20% of the operated patients all x-ray and laboratory tests gave negative results, despite the fact that in the course of operation abscesses were detected under the renal capsule. In 60% of the cases x-rays supplied reliable information for the indication of surgical intervention. The history of fever and shaking fits and the progression revealed by careful observation of the patients are considered the most important information needed for deciding for surgery.

Acute Disease

Symptomless renal tumours diagnosed from their metastases.

After a brief review of renal tumours diagnosed on the basis of metastases, two cases are described in which subcutaneous formations believed to be lipomas were metastases of symptomless hypernephromas. One case was diagnosed early on basis of the histological finding. The primary tumour could be localized only by angiography. After nephrectomy and resection of the contralateral rib and pleura, the patient was free of symptoms for one year. In the subsequent year a pathologic subtrochanteric fracture called attention to a new metastasis. After laminar osteosynthesis function of the hip and knee joints was satisfactory and so has been the general condition ever since the first operation now for three years. In the second case, the knot assumed to be a lipoma was removed but no histological examination was carried out. These experiences have induced the authors to call attention to the importance of histological examination of apparently banal lesions and to the considerable survival which can be achieved in these patients.

Adenocarcinoma

[The modified Denis prostatectomy].

The favourable experience gained with the so-called modified Denis-type method is described. It is stressed that the method has all the advantages which are ensured by the Hencz-type haemostatic thread. In addition, the patient is spared many unpleasantness and post-operative complaints. Post-operative treatment is easier since a separate flow from the loge and from the bladder is provided for. The results of different methods are compared on the basis of 150 prostatectomies performed in the Department of the authors.

Aged

[Gluteal fistula that developed spontaneously from pyonephrosis].

A case is described in which pyonephrosis of 21 months standing has caused gluteal fistula by spontaneously breaking through the deep dorsal musculature. Though the patient has been hospitalized in the surgical ward and treated as out-patient in the specialized surgery for gluteal abscess, i.e. fistula, pyonephrosis maintaining of the fistula was detected and treated only after 18 months. After nephrectomy the fistula closed spontaneously and the patient was cured when leaving the hospital.

Abscess
