Standard photopatch testing with Musk Ambrette.

495 patients were standard photopatch tested with Musk Ambrette 5% in petrolatum and in dimethyl phthalate. 4 cases gave positive photopatches; 3 were men and 1 a woman. 2 cases, both men, had current or past relevant reactions to Musk Ambrette presented with typical perfume dermatitis. The other 2 cases had positive reactions to Musk Ambrette of uncertain relevance. Photoallergic reaction to Musk Ambrette is a rare event. Only a few relevant reactions will be missed by excluding it from standard phototesting.

Standard photopatch testing with Waxtar, para-aminobenzoic acid, potassium dichromate and balsam of Peru.

194 patients were standard photopatch tested with Waxtar as is (coal tar 5%) and 161 patients were photopatch tested with para-aminobenzoic acid (PABA) 5% in alcohol, potassium dichromate 0.5% in petrolatum, and a mixture of balsams of Peru as is. The photopatches were irradiated with UVA. 40 patients (25%) had phototoxic reactions to Waxtar and 4 of them showed pigmentation after 7 days. Only a few patients had photocontact urticaria. 1 patient had a late-reaction to PABA and showed a cross-reaction to glyceryl PABA but a negative reaction to paraphenylenediamine (PPD) and benzocaine 5% in the standard test. No patients had positive photopatch reactions to potassium dichromate when irradiated with UVA. 2 patients had phototoxic reactions to balsam of Peru. None had photoallergic reactions. Standard photopatch testing is a time consuming procedure which creates problems both for the staff and for the patients. The yield of unexpected, relevant positive reactions is insignificant. From a cost-benefit view, photopatch testing is only warranted in cases giving rise to a clinical suspicion of photodermatitis.

Allergy to human seminal plasma: a presentation of six cases.

Six women had symptoms of immediate type allergic reactions such as localized oedema, urticaria and pruritus elicited at, or soon after intercourse. All had positive skin prick tests with dilutions of human seminal plasma. The symptoms failed to appear when the women's partners used a condom an intercourse. None of the women had IgE antibodies in serum against human seminal plasma when tested with the radioallergosorbent test. This type of allergic reaction is easily overlooked.

A comparison of 99Tc and 123I scintigraphy in nodular thyroid disorders.

Out of 450 patients referred for thyroid scintigraphy 83 consecutive patients with nodular goitre were selected for dual scintigraphy with 99mTc and 123I. Seventy-four patients had one or more palpable nodules and the other nine patients had palpably diffuse goitres with one or more nodules on the routine 99mTc scintigram. For the comparison of scintigrams an eight level colour display was used, and regional differences between the 99mTc and 123I scintigrams were defined as a minimum of 25% (2 colour levels) variation of the relative activity in well defined areas. Thirteen patients, i.e. 16% of those with nodular goitre, had discordant scintigrams. All 13 patients had non-malignant thyroid disorders, (simple goitre, radio-iodine treated hyperthyroidism, autonomous adenoma, subacute thyroiditis, Riedel's and lymphocytic thyroiditis). The diagnosis was verified histologically in eight of these patients. Eight of the 13 patients had a high 99mTc activity relative to 123I, five patients showed the opposite discrepancy. Another 5 patients with a follicular or mixed follicular thyroid carcinoma had identical scintigrams. These findings indicate that 123I does not offer such diagnostic advantages over the cheap and readily available 99mTc, that its use for routine scintigraphy of the thyroid gland seems to be justified at present.

Hepatitis C virus antibodies in homosexual men and intravenous drug users in Denmark.

In order to evaluate the role of sexual transmission and parenteral transmission of hepatitis C virus (HCV) in homosexual men and intravenous drug users (IVDU) serum samples from 147 homosexual men and 126 IVDU were tested for anti-HCV. Anti-HCV was found in two (1.4%) of the homosexual men and in 123 (98%) of IVDU. The presence of anti-HCV could not be correlated to the presence of HBV markers or HIV-antibodies. HCV is widespread among Danish IVDU. Risk of sexual transmission seems low even though sexual contact is a much more prevalent risk factor than needle sharing.
