Atelosteogenesis type 3: the first patient in Japan and a survivor for more than 1 year.

We report the first patient of atelosteogenesis type 3 (AO3) in Japan. The patient had multiple craniofacial abnormalities at birth, including ocular hypertelorism, a flat nasal bridge, micrognathia and a cleft palate. There was rhizomelic shortness of the limbs and a club-foot. The infant had short broad thumbs in the hands similar to those observed in the feet. There were no chromosomal abnormalities. Radiological examination demonstrated striking hypoplasia of the humerus with proximal rounding and distal tapering giving a 'drumstick' appearance, 'S'-shape configuration of the cervical spine, scoliosis and coronal cleft in the thoracolumbar vertebral bodies. The infant experienced recurrent apnea and persistent severe tracheomalacia, which necessitated tracheostomy at 5 months of age. Despite his multiple skeletal deformities and respiratory problems, this patient survived more than 1 year with motoneuronal developmental delay.

Statistical investigation of lower distal free-end denture design used by general practitioners in Japan.

Tooth loss eventually occurs in most adults, and masticatory function is restored with a prosthesis such as a fixed crown & bridge, removable partial denture (RPD), or complete denture according to the extent of loss. For the RPD there are different structural-element combinations and different designs that can be applied to the same loss pattern, so that the effect and prognosis vary. Results from a statistical investigation of removable distal free-end dentures in Japan suggested that in many cases, insufficient consideration was given to mechanical stability and protection of remaining peripheral tissue in the design of removable partial dentures.

[Selection error of upper anterior artificial teeth].

Seventy two cases of selection error of upper anterior artificial teeth due to the esthetic factors were investigated out of the 1,507 cases of removable dentures, which were made at the Prosthodontics department of Nihon University school of dentistry at Matsudo for 17 years from November, 1971 to November, 1988. The following were the results of the investigation. 1. Frequency of the selection errors amounted to 4.8% of the total selected cases as broken down in 1.6% by Shade and 4.0% by Mould, the rate of selection errors of the Mould being significantly high. 2. By Mould, selection errors in Ovoid type occupied a significantly high rate. 3. Regarding frequency of Mould after re-selection ST and TO occupied a high rate of 45.9%. 4. Mould of re-selection were almost uniformly small, showing a trend of intumescence becoming emphatically.

Denture, Partial, Removable

[Cross sectional study of removable partial denture design in general practitioners].

Many reports have been made on statistical observation on prostheses, among which removable partial denture frequently becomes the subject of investigation from the two sides of cross- and longitudinal- sectional surveys, because its constituent factors are divergent. However, many of such reports used, as subject, the removable partial dentures fitted at clinical practices at the hospitals affiliated to dentistry universities (dentistry departments), and it is hard to say that these reports have grasped the actual circumstances in which removable partial denture is involved since each therapeutic principle as educational hospital constitutes its own specific environment, if the fact that difference occurs in the selecting criteria for each constituent factor according to the operators' designing conception and medical environment is taken into consideration. Then, we have made investigation of edentulous pattern and kinds of denture base material and retainers in 141 cases with removable partial denture fitted at general practitioners, and the following conclusions have been reached. 1. Among the direct retainers of free-end denture, those of clasp with distal rest (Aker's clasp) were the largest in number. 2. By base material, among the direct retainers of free-end denture, many of those of Bar type which has mesial rest (RPI clasp) were observed, in case of metal base, compared with resin base, but difference was observed in the frequency of use of the other retainers. 3. Free-end denture held a majority both in the upper and lower jaws. 4. As base material, resin base was overwhelmingly large in number, compared with metal base.

Cross-Sectional Studies

[Horizontal stress of abutment. Particularly regarding difference in removable partial denture design].

Prosthesis with removable partial denture is a daily clinical procedure for compensating the morphological loss and hypofunction of teeth and various gnathostomatic system. The removable partial denture not only comprises of many elements such as denture base, abutment and clasp but accordingly has to undergo morphologically complicated operative process, thus posing many factors concerning the functional stability coping with diverse masticatory motions. However, from the fact that retention and stability of the denture at it's functioning are obtained basically from it's connection with abutment, assessments in multiple aspects have been made regarding the influence exerted on abutment by difference in the structural elements, i. e. clasps, which transmit the movement of denture directly. There were the assessments by means of the displacement, measuring system by the use of microdial gage, stereoscopic camera, eddy currents transfer, and magnetic resistant element, and deformation measuring systems such as photoelasticity method, holography, moiré method and strain gage method. However, these systems are the methods for assessing abutment as subject but not the assessing system including all the basic structural elements of denture. Therefore, the author has made observation of the influence exerted on the dynamics of abutment by the difference between clasps in 2 kinds and by cross arch stabilization by designing aker's clasp and RPI clasp for the abutment by the use of cast frame standardized.(ABSTRACT TRUNCATED AT 250 WORDS)

MR findings of surgically retained sponges and towels: report of two cases.

MR findings of surgically retained sponges (gossypiboma) in two patients are reported. The sponges were well-encapsulated round masses with a thick wall of low signal intensity on both T1-weighted images (T1WI) and T2-weighted images (T2WI). The signal intensity of the center of the masses varied in both patients on T1WI and T2WI. In one patient, the mass was high in signal intensity on both T1- and T2-weighted images and had a low signal intensity whirl that represented a surgical sponge. The sponge was rich in serosanguinous fluid with a high protein concentration (8.2 g/dl). In the other patient, the mass was low in signal intensity on both T1- and T2-weighted images, and was mainly composed of organized cotton matrix with little fluid. The protein concentration of the fluid was lower than in the former case (6.4 g/dl).

Aged
