Predictors of risk for adolescent childbearing.

The purpose of this research was to develop a predictive model that identifies young women at risk for adolescent motherhood. A stratified sample of 357 adolescents was drawn from public health units and public schools in six randomly selected counties in North Central Florida. Data were collected regarding the environmental context (social and family), individual perceptions of self and trajectory, and adolescent motherhood status. A stepwise logistic regression was used to investigate the combined effects of these variables for identification of risk for adolescent motherhood. Using the combination of these variables, practitioners can identify adolescents at highest risk for early motherhood (97.66%) and those with the least risk of adolescent motherhood (approximately 1%).

Ethical models: facilitating clinical practice.

Modern health care presents significant dilemmas in nursing practice. Nurses are often entangled in the web of conflicts between the principles of autonomy, beneficence, and justice in managing the health care concerns of individuals, nurses, and the health care team. However, to achieve resolution the nurse needs an ethics model with which to determine appropriate care practices. According to the concept of autonomy, each individual has the fundamental right of self-determination in health care decisions. At the same time, the nurse strives toward beneficent care for individuals through the delivery of health care. Tension between these two principles generally is minimal. It is only when conflicts exist between the autonomous patient and the beneficent nurse that ethical problems surface. At these times, respect for individual freedom and the interest in doing good are juxtaposed. Nurses may have difficulty determining which actions best serve the purpose and interests of each party to the issue, or which issues, if any, must be regarded as fundamental obligations. For nurses to provide patient care within an ethical framework, clinical practitioners need (1) knowledge of basic rights and ethical principles; (2) conceptual models as ways of thinking about ethical dilemmas; and (3) opportunities to explore and resolve clinical dilemmas.

Models for moral response in care of seriously ill children.

This paper presents a discussion of three models for provision of care to seriously ill infants and their families. The statistical, the wait until near-certainty and the individual models are defined and compared. Comparisons were based on quality of life, legality, responsibility, withdrawing/withholding treatment and potential injury. The individual model offers the most humane and legally sound basis for care. Suggestions to redefine the model of care in the United States are offered with nursing leading the way.

Social support and adaptation to the parent role in first-time adolescent mothers.

The relationship between social support and adaptation to parenthood for first-time adolescent mothers was investigated in this descriptive correlational study. The outcomes of adaptation were defined as the synchrony of parent-infant interaction and the level of stress related to parenting. A nonprobability sample of 18 adolescent, first-time mothers who had an uncomplicated perinatal experiences and delivered healthy, term newborns were selected. Data were collected during a home interview at one month postpartum. The measure of social support was significantly related to parent-infant reciprocity. Total functional support scores were inversely, but not significantly, correlated with total scores of stress related to parenting (r = -.31). Finally, the measure of reciprocity and the measure of stress related to parenting were inversely but not significantly related.
