Therapeutic embolization with detachable silicone balloons. Early clinical experience.

A recently developed percutaneous transcatheter technique for selectively occluding arteries with detachable silicone balloons was applied in four patients. Uncontrollable hemorrhage from carcinoma of the cervix was controlled by selective uterine artery occlusion in one patient. Life-threatening hemorrhage from cystic fibrosis in another patient was controlled by balloon occlusion of a bronchial artery. A patient with hypernephroma and another with right ischial fibrous histiocytoma each had preoperative occlusion of the arteries supplying the neoplasms to reduce operative blood loss and to obviate the need for surgical ligation of the internal iliac artery in the latter patient. Detachable balloon embolization techniques represent a nonoperative method for permanently occluding arteries without the risks of inadvertent embolization.

Percutaneously placed biliary stents in the management of malignant biliary obstruction.

The management of malignant obstruction of the biliary tree is difficult and often unsuccessful. Advances in the technique of percutaneous transhepatic catheterization have made possible the nonoperative internal drainage of obstructed lesions. We report successful decompression of ten patients with unresectable biliary neoplasms using percutaneously placed internal biliary stents.

Detection of virus-specific RNA in simian sarcoma-leukemia virus-infected cells in in situ hybridization to viral complementary DNA.

An in situ molecular hybridization system which will detect retrovirus RNA in the cytoplasm of individual virus-infected cells has been developed. The technique was applied to cells infected with simian sarcoma-leukemia virus, where the virus-specific RNA was detected by hybridization to simian sarcoma-leukemia virus 3H-labeled complementary DNA. The system is useful for detecting viral RNA-containing cells in the presence of an excess of virus-negative cells and for determining which type of cell in a heterogenous population is expressing viral RNA.

Embolotherapy with detachable silicone balloons: technique and clinical results.

Catheter techniques and early clinical results of therapeutic embolization with detachable silicone rubber balloons are described in 13 patients. Effective control of post-traumatic or spontaneously occurring hemorrhage or preoperative devascularization of neoplasms was achieved in 10 patients. Three additional patients, one each with pelvic arteriovenous malformation. pulmonary arteriovenous malformation, and hemangioendothelioma of the liver, were partially treated by balloon occulsion. Therapeutic embolization with detachable silicone balloons provides a method of long-term, extremely selective arterial occlusion with reduced risks from inadvertent embolization of neighboring circulations.

Hemodynamic significance of iliac artery stenosis: pressure measurements during angiography.

Peak systolic pressure gradients were obtained before and after vasodilatation in 42 patients (50 limbs) with arteriographic iliac artery stenosis of questionable significance. Patients were divided into three groups according to per cent narrowing of the iliac artery. Pressure gradients across the site of stenosis tended to be significant in patients with greater than 75% stenosis (greater than or equal to 20 mm Hg) but not in patients with less than 50% stenosis; patients in the middle group (50-75% stenosis) demonstrated a wide variance. Thus the arteriogram is not an accurate indicator of hemodynamically significant lesions, particularly in patients with 50-75% stenosis where pressure measurements are of greatest value. Variations in the aortic and femoral artery systolic peak pressure occurred following vasodilatation, indicating the importance of simultaneous pressure recording.

Chronic vascular reactions to steel coil occlusion devices.

Original Gianturco steel coil vascular embolization devices and two modifications were compared for their occlusive effects and for the degree of adverse reaction in the surrounding vessel wall. Modifications consisted of silk or Ivalon replacing the wool strands of the original device. Single coils were placed into the left renal artery of nine mixed breed swine and restudied 3 months later. The bare coils did not occlude the vessels nor lead to significant foreign body reaction. Wool, silk, and Ivalon coils produced prompt and sustained renal artery occlusion. Upon histologic examination of the vessel wall, foreign body and chronic inflammatory reaction was found to be strongest against wool. This reaction was less with silk and markedly diminished with Ivalon. The results indicate that silk and Ivalon coils, while having the same occlusive effect, are preferable to wool coils if less local reaction is desired.

Protean manifestations of mycotic aneurysms.

Nine patients examined by arteriography were shown to have mycotic aneurysms involving the thoracic aorta, subclavian artery, renal artery, middle cerebral artery, hepatic artery, and splenic artery. Patients presented with sepsis, chest pain, mediastinal mass, headache, hypertension, and intraperitoneal bleeding. Etiologic factors included endocarditis, septicemia, drug abuse, and poorly controlled soft-tissue infection. Most mycotic aneurysms were virulent processes with rapid progression and only three of the nine patients (33%) survived. Since mycotic aneurysms may be associated with rapid progression and poor prognosis, early recognition is mandatory.

In vitro computed tomography of the human heart.

In vitro computed tomography (CT) was performed on nine human hearts. Mean attenuation coefficients from a 2.5 X 2.5-cm area of the left ventricular wall were determined from a 160 X 160 computer printout before and after formalin fixation in five hearts. These values averaged 27.1 and 24.7 units, respectively (p less than 0.05). The absorption characteristics of normal human myocardium do not differ from those observed in animals and the potential of CT application to ischemic heart disease can be assessed by this mehtodology applied to postmortem hearts of patients with myocardial infarction of varying duration.

Superior mesenteric artery embolization: an angiographic emergency.

Eleven cases of superior mesenteric artery embolization are presented. The extent of bowel necrosis was found to be related to two major factors: the site of occlusion and the elapsed time between the onset of symptoms and definitive therapy. Angiography, available on an emergency basis, provides a definitive diagnosis which can only be suspected by other means. Therapy instituted within 24 hours of the onset of symptoms in all cases resulted in a mortality of only 9% attributable to intestinal infarction.

Control of variceal bleeding by superior mesenteric artery vasopressin infusion.

Liver function and clotting tests were evaluated in 39 patients with variceal bleeding prior to superior mesenteric artery vasopressin infusion. In six patients with mild hepatic dysfunction (Child's class A), permanent control of hemorrhage was achieved in all six and all survived the hospitalization. In 21 patients with moderate dysfunction (Child's class B), permanent control of hemorrhage was achieved in 13 (62%) and temporary control for 24 hr or longer in the remaining eight (38%). Survival in class B was 67% (14 of 21). In only four of 12 patients with severe hepatic dysfunction (Child's class C) was control of hemorrhage achieved (33 percent). None of these patients survived. Therapeutic failure also was associated with clotting derangements and the initial bleeding rate. It is concluded that the effectiveness of vasopressin in variceal hemorrhage is a function of the underlying liver disease and derangements in clotting function.

Transitional cell tumors of the kidney: how diagnostic is the angiogram?

Angiograms of 51 tests cases including 34 transitional cell carcinomas, eight inflammatory lesions, four hypernephromas, and five miscellaneous disorders were shown to six radiologists for analysis of the angiographic features and specific diagnosis. Overall accuracy was 60% with a false positive rate of 44%. The most common false positive diagnosis occurred with inflammatory lesions (seven of eight). False negative cases were variously called normal, hypernephromas, inflammatory, or other. Vessel encasement (26%), neovascularity (56%), and tumor blush (15%) were recognized far less frequently in the transitional cell tumors of this series than in previous reports in the literature. Further, when hydronephorsis was present, the angiogram was not helpful in determining the etiology. It is concluded that the "classic" angiographic findings associated with this tumor are only moderately reliable since they can be mimicked by other conditions.

Hepatic venography and wedge hepatic vein pressure measurements in diffuse liver disease.

Ninety patients with chronic diffuse liver disease were evaluated with free hepatic venography, wedge hepatic venography, hepatic vein pressure measurements, and liver biopsy. Free hepatic venograms were normal and minimally pruned in patients with hepatic sarcoidosis and fatty liver due to alcohol, and their biopsies showed little or no fibrosis. Pruning of hepatic vein branches on free hepatic venography correlated well with the corrected wedged hepatic vein pressure and with the degree of fibrosis in patients with alcoholic hepatitis, alcoholic cirrhosis, and postnecrotic cirrhosis. Free hepatic venography correlated better with hemodynamic measurements and fibrosis than did wedge hepatic venography. Free hepatic venography is a reliable predictor of the presence and degree of hepatic fibrosis and may be a useful alternative to liver biopsy in patients with clotting disorders.
