[Stress-associated hypertension in the work place: results of the STARLET project].

BACKGROUND AND OBJECTIVE: To assess the influence of work stress and initial blood pressure on the prognosis of hypertension. SUBJECTS AND METHODS: In a prospective, controlled, multicentre, observational study, ambulatory 24-hour blood pressure measurements (ABPM) of employees from different work places were recorded at the work place on working days. Recurrent ABPM were performed for up to 5 years on 3448 subjects (mean age 44.6 years) who gave consent for follow-up. Subjects with hypertension were told to consult their family doctor so that they could receive antihypertensive treatment (the angiotensin receptor blocker eprosartan, an ACE-inhibitor or a beta-blocker were recommended for initial treatment). Subjects were classified as being in mental strain (stress-positive [stress+]/ stress-negative [stress-]), using standardized questionnaires. RESULTS: Only 1242 (36.0%) of the 3448 employees (69.% males) were normotensives. Only 166 (7.5%) of the 2206 hypertensives had normal ABPMs (<135/85 mmHg) and received antihypertensive treatment at the time of inclusion into the trial. During follow-up 57.8% of patients were treated with eprosartan or ACE-inhibitors, 34.6% with beta-blockers. By the time of the final visit 80.5% of hypertensives had achieved improvement of systolic and/or diastolic blood pressures (29.1% normotensive). Patients with hypertensive ABPM at baseline had more cardiovascular events than normotensives (normotensives 3.0%; grade 1 7.8%, grade 2-3 9.8%). Hypertensive ABPMs at the last follow up or an increase in blood pressure grade were associated with higher event rates than normotensives (stable normotensives 1.8% events vs. stable hypertensives 7.9%, vs. worsening or grade 2-3: 9.1%) More hypertensives were classified as stress+ than normotensives. Persons classified as stress- (or changing to stress-) had fewer events (6.2%) than those regarded as stress+ or changing to stress+ (7.1%). Persons regarded as stable stress- had lower mean blood pressures than those who were stable stress+. Change to another stress group was associated with an increase or decrease of mean blood pressure. CONCLUSIONS: Many employed people are hypertensive at work and are not treated adequately. ABPM control and antihypertensive treatment based on eprosartan, ACE-inhibitors or beta-blockers resulted in a significant increase in the number of patients with lower blood-pressure levels and a reduction in cardiovascular events. Patients under mental strain were more likely to be hypertensive. Mental strain was associated with changes in blood pressure.

Diagnosis of impaired glucose tolerance in hypertensive patients in daily clinical practice.

Many patients with hypertension suffer from impaired glucose tolerance or type 2 diabetes mellitus. Although these diagnoses are generally simple and reliable, it is more difficult to diagnose impaired glucose tolerance. As the gold standard (oral glucose tolerance test (OGTT)) is complicated to perform, a simpler alternative would be useful. The aims of the Pre-Diabetes Score study are to correlate demographic and/or laboratory parameters that are clinically simple to determine with the results of the OGTT and to determine the diagnostic significance of the combinations of parameters with regard to impaired glucose tolerance. A total of 260 patients were included in the evaluation; 39% had impaired glucose tolerance and 12% had diabetes mellitus. A combination of HbA1c of > or =6%, a venous fasting glucose of > or =110 mg/dl, an age of > or =55 years, a systolic blood pressure of > or =140 mmHg and an enlarged waist size is highly predictive of impaired glucose tolerance.

[Recurrent hemispheric symptoms without proof of ischemia].

ANAMNESIS AND CLINICAL FINDINGS: Over the last 6 years, a 48-year-old patient recurrently experienced passing neurological left hemispheric symptoms of varying strength. Because of the patient's distinctive cardiovascular risk profile, these symptoms were diagnosed as transitory ischaemic episodes. On inquiry, the patient now reported frequent headaches in close temporal connection to the events. EXAMINATIONS: Repeated imaging of the neocranium using various techniques (CCT, cMRT, MRT-angiography) revealed no conspicuous features. Duplex sonography of the brain feeding arteries, echocardiography, EEG and 24h-ECG also gave findings within the normal range for the patient's age group. Clinical chemistry showed increased values for cholesterol and triglycerides. Incidentally, a moderate upper airway resistance syndrome was identified during polysomnography. DIAGNOSIS: Suspected migraine with aura in terms of a sporadic hemiplegic migraine. TREATMENT AND COURSE: On administration of ASS and rizatriptane, the neurological hemispheric symptoms disappeared completely over the course of a few hours. Under prophylactic treatment with metoprolol and acetazolamide over two months, no further events have been observed up to now. CONCLUSION: Recurrent, temporary neurological deficits, together with the presence of several cardiovascular risk factors, can easily be misinterpreted to be the result of an underlying cerebral ischemia. Even in cases with typical cardiovascular risk profile, a migraine with aura must be considered as one of the possible causes for hemispheric neurological symptoms.

[When in doubt, 24-hour monitoring. When 24-hour blood pressure monitoring is indicated].

Not only doctor's office hypertension, but also, conversely, doctor's office normotension can be detected by 24-hour blood pressure measurement. A diagnostic improvement is also achieved in patients with borderline or mild hypertension. A number of illnesses are associated with nocturnal blood pressure elevation. In particular patients with secondary hypertension experience this reversal of the usual decrease in blood pressure at night. In such cases, non-invasive 24-hour blood pressure measurement may provide the first evidence. In pregnant women in particular, untreated hypertension may have serious consequences. In such cases, close monitoring of blood pressure and treatment by long-term measurement is also of prognostic significance. Also, in patients with concomitant diseases, such as diabetes mellitus or renal disease, 24-hour blood pressure measurement for monitoring purposes should be done more often than in patients with a low risk profile.

Blood Pressure Monitoring, Ambulatory↗

Practice vs ambulatory blood pressure measurement under treatment with ramipril (PLUR Study): a randomised, prospective long-term study to evaluate the benefits of ABPM in patients on antihypertensive treatment.

The usefulness of ambulatory blood pressure monitoring (ABPM) vs casual blood pressure measurement in the physicians practice (PM) for the routine management of patients with hypertension concerning total mortality and morbidity has been compared in a prospective, randomised, open multicentre study with a 5-year follow-up. The study was performed in general practitioners offices in Germany from 1991 to 1997. A total of 1298 patients with essential hypertension were included. Cardio- and cerebrovascular events, total mortality/morbidity and drug-saving effects in hypertensives treated according to two different methods of blood pressure measurement were the primary and secondary endpoints of the study. A total of 239 patients from group 1 (ABPM, n = 651), and 208 from group 2 (PM, n = 647) prematurely discontinued the study. The reason for discontinuation in 55 of these patients (20 from group 1 and 35 from group 2) was that they reached the main endpoint (total mortality/morbidity and cardio- and cerebrovascular events) of the study. The difference was statistically significant (P = 0.037) in favour of group 1. Cardio- and cerebrovascular events also occurred in a lower number of patients (n = 14) in group 1, as compared to group 2 (n = 24). The difference however was not significant (P = 0. 097). A difference in a direct drug-saving effect could not be observed between the two groups but 22% of the initially screened patients were detected with the ABPM to have white coat hypertension and thus these patients did not receive antihypertensive treatment.

Hypertension and stroke--rationale behind the ACCESS trial. Acute Candesartan Cilexetil Evaluation in Stroke Survivors.

Antihypertensive treatment achieves its greatest benefit in the primary prevention of stroke. Primary prevention studies show 38% fewer strokes when systolic/diastolic values are reduced by 10-12/5-6 mmHg. Secondary stroke prevention has been less investigated, but restrokes seems to be reduced with antihypertensive treatment. Secondary prevention achieves 25-30% less strokes, if diastolic BP can be reduced by 3-4 mmHg. Today's guidelines for antihypertensive therapy in acute ischemic stroke suggest reducing BP values over 220 mmHg systolic (AHA) or 200/110 (German Hypertension Society). No data are available about antihypertensive treatment in acute stroke patients. No intervention trials have so far evaluated an immediate BP reduction on the clinical outcome of the patients neurological status (morbidity) or mortality rates in the acute stroke situation. However, some studies show an increase in mortality after a quick and rapid BP reduction in a short time interval. The ACCESS study was designed to evaluate the possible benefits of a careful and moderate, but immediate blood pressure reduction in patients with an acute stroke compared to a restrictive antihypertensive therapy. Candesartan cilexetil was selected as the antihypertensive drug for its slow onset of action and moderate BP reduction, as well as its very good tolerability. Experimental studies point at possible advantages in acute stroke. The study was designed as a prospective, randomized, double-blind, placebo-controlled, multicenter trial (500 patients). Inclusion criteria were an acute ischemic stroke with a motor paresis and severe hypertension. Primary endpoints were the patients morbidity (functional status measured with Rankin and Barthel index, degree of motor deficity by NIH scale) and mortality rates after three months. First results are presented.

Differential regulation of phospholipase A2 in human leukemia cells by the etherphospholipid analogue hexadecylphosphocholine.

Hexadecylphosphocholine (HePC) is the main representative of a new group of antineoplastic agents, the alkylphosphocholines, which were originally derived from cytotoxic etherlysophospholipids. HePC shows antiproliferative action against a whole variety of tumor cells and tumors in vitro and in vivo. Furthermore, it also induces differentiation in some hematologic cell lines and prevents invasive growth of neoplastic cells in vitro. To date, the precise molecular mechanisms mediating the biological effects of HePC have not been identified yet. As etherlysophospholipids seem to inhibit some pathways of lipid-dependent intracellular signalling, similar effects may be relevant for HePC. We therefore investigated the influence of HePC on phospholipase A2 (PLA2-EC 3.1.1) in the human leukemia cell line U 937. HePC seems to inhibit enzyme activity independently of protein kinase C (PKC) in differentiated U 937 cells stimulated by tumor necrosis factor alpha (TNFalpha). Inhibition of purified secretory PLA2 from snake venom (EC 3.1.1.4) in vitro shows characteristics of a non-competitive mode. In contrast, HePC leads to an enhancement of PLA2 activity in immature cells which cannot be explained by changes in membrane composition. Our data suggest that PLA, inhibition is most probably not the mechanism by which HePC mediates its antiproliferative effects.

Hexadecylphosphocholine inhibits phosphatidylinositol and phosphatidylcholine phospholipase C in human leukemia cells.

Hexadecylphosphocholine (HePC) is the main representative of a new group of antineoplastic agents, the alkylphosphocholines. Besides remarkable antiproliferative properties on tumor cells in vitro and in vivo, HePC also induces differentiation and inhibits invasive growth of neoplastic cells. Knowledge of the molecular mechanisms by which HePC mediates its biological effects is poor. The observation that analogous substances, the alkyllysophospholipids, may interfere with lipid dependent intracellular signaling suggested similar mechanisms for HePC. We therefore investigated the effects of HePC on phospholipase C (PLC) activation in intact human leukemia cell lines. HePC inhibited fMLP induced phosphatidylinositol-specific PLC activation in HL60 cells and TNF-alpha induced activation of phosphatidylcholine-specific PLC in U937 cells. HePC reduced the number of TNF-alpha receptors on the surface of U937 cells by about 60%. Receptors for fMLP were not affected. Inhibition of TNF-alpha induced PC-PLC activation, however, seemed to be regulated at a post-receptor level as PLC inhibition and receptor occupancy did not correlate.

Platelet and fibrin deposition on coronary stents in minipigs: effect of hirudin versus heparin.

OBJECTIVES: The present study was designed to test the hypothesis that the direct thrombin hirudin is more efficient than heparin in reducing thrombus formation after coronary stenting. BACKGROUND: Despite aggressive anticoagulation, subacute thrombosis of coronary stents is a major complication associated with these new devices. METHODS: In 19 minipigs indium-111-labeled thrombocytes and iodine-125-labeled fibrinogen were injected 14 to 19 h before coronary implantation of tantalum balloon-expandable stents. In group 1 (n = 6, seven stents), a bolus of heparin (100 U/kg body weight) was given before stenting. Group 2 (n = 6, 11 stents) received both dextran (500 ml) and heparin (a 100-U/kg bolus followed by a continuous infusion of 50 U/kg per h). In group 3 (n = 7, 13 stents), hirudin (recombinant desulphatohirudin HV 1 [CGP 39393] [1 mg/kg]) was given before stent implantation, followed by an infusion of 1 mg/kg per h. All animals were pretreated with aspirin (250 mg intravenously). RESULTS: Activated partial thromboplastin time was prolonged to > 1.8 times control values in groups 2 and 3. Histologic examination after perfusion fixation 12 h after stenting showed a variable extent of thrombus on all stents. Medial tear was observed in three stents in group 1, six stents in group 2 and six stents in group 3. The number of platelets on all stents averaged 116.2 (range 22 to 522) x 10(6) in group 1, 64.3 (range 11 to 169) x 10(6) in group 2 and 19.7 (range 9 to 38) x 10(6) in group 3 (p < 0.05 vs. group 1 and vs. group 2). The increase in platelet deposition, associated with medial tear in all groups, was lowest in the hirudin group. Similarly, fibrin deposition was lowest on stents in hirudin-treated animals. CONCLUSIONS: Recombinant hirudin significantly reduces platelet and fibrin deposition on coronary stents compared with the reduction achieved with combined heparin, dextran and aspirin.

[Long-term blood pressure measurement for evaluating the first dose response of captopril and ramipril in patients with a stimulated renin system].

First-dose-response of captopril 1 x 25 mg (no prodrug) and ramipril 1 x 2.5 mg (prodrug) were compared in two groups of 17 patients with moderate or severe hypertension and stimulated renin-angiotensin system (because of continuous diuretic therapy) by means of 24-h blood-pressure measurement at the 1st and 7th day of therapy. In the ramipril-group the antihypertensive effect started after 2 h, had its maximum (mean: -13/-8 mmHg) after 4 h and remained unchanged for 8 h. The antihypertensive effect was significant for 24 h. There was a slightly but not significant improved blood-pressure reduction at the 7th day compared to the 1st. The captopril-group showed a fast and marked decrease of blood pressure within the first hour, and reached its maximum (mean: -18/-10 mmHg) after 2 h. After 7 h there was no antihypertensive effect detectable. At the 7th day blood-pressure reduction was less pronounced compared to the 1st day. The results show that initial decrease of blood pressure in risk-patients is less severe with prodrug-ACE-inhibitors with slow onset of action so that counterregulation can be activated and prevent severe, fast, ACE-inhibitor-induced hypotension. 24-h-blood-pressure measurement is a sufficient method to evaluate first-dose-response of ACE-inhibitors.
