Friend murine leukaemia virus is integrated at a common site in most primary spleen tumours of erythroleukaemic animals.

A common proviral integration site was identified and characterized in erythroleukaemias induced by Friend murine leukaemia virus (F-MuLV). Using inverse polymerase chain reaction, we found a proviral integration site common to at least 90% of 20 primary tumours tested. This site was identical to Fli-1, a locus recently reported by others to be rearranged in 75% (9/12) of cell lines established from spleens of erythroleukaemic mice and to code for a member of the ets gene family. Our data suggest that about half of the F-MuLV-induced erythroleukaemias contained more than one cell clone with a proviral integration in Fli-1, with different individual integration sites within Fli-1 in each cell clone. All proviruses were found to be integrated in the same transcriptional orientation with respect to flanking cellular DNA. We discuss these findings in relation to multistage models of neoplasm.

Animals

Long-term results after incisional hernia repair.

After repair of incisional hernia, 154 patients were followed up for periods ranging from 4 to 10 years. An actuarial analysis showed a cumulated recurrence rate of 31%, with most recurrences appearing in the first three years. The recurrence rate after repair of recurrent incisional hernia was 44%. A cautious attitude to surgical treatment of incisional hernia is suggested if the patient has only mild discomfort.

Adolescent

[Cryogenic surgery].

Local tumor destruction by cryosurgery was observed clinically and morphologically in 184 patients. If indication is observed strictly this method seems to provide real palliative effect. Complete tumor destruction was observed in 14.1%. Local tumor destruction was followed by tissue inflammation and nonspecific phagocytosis of tumor cells. Immunological reaction and epithelialization was noted only after radical destruction of tumors. Most cases showed immediate relief from stenosing cancer; colostomy could be avoided.

Cryosurgery

[Cancer of the colon: clinical and morphological aspects of local cryosurgery (author's transl)].

Using strict indications, local cryosurgery was performed in 86 patients with cancer of the colon. Survival time was longer both for patients with advanced tumour growth and distant metastases and those with potentially curable tumour but in too poor a general condition for operation. Other advantages of cryosurgery are the low complication and mortality rates, the possibility of out-patient treatment without anaesthesia, and thus a general reduction in cost. Indications for colostomy as final palliation can therefore be more narrowly defined than previously customary.

Aged

[Colon anastomosis: single-layer (row) suture (author's transl)].

Comparison of two groups of patients with different anastomosis techniques (622 large-bowel resection) demonstrates a lower rate of complications with the single-row anastomosis. These observations were proved in dogs and rats. The double-row suture prolonged the healing process, delayed revascularization, and led to the formation of intramural abscess and stenotic intraluminal roll building. These problems were not observed in single-row anastomosis, which is designed to promote faster healing.

Abscess

[Cryosurgical treatment of rectal carcinoma (author's transl)].

After comprehensive experimental especially morphological studies in animals, we started cryo-therapy of rectal carcinoma in 1972. We now present the first critical 5-years review concerning indication, clinical results and cryo-immunological features. In strongly indicated cases local tumour destruction by extremely low temperatures is superior to other palliative methods. Of 67 patients with rectal carcinoma treated with cryosurgery, 37 survived, 12 more than 36 months and 16 more than 24 months. Though there are only few hints indicating an increased immunological response following cryo-surgery, our investigations suggest a general influence on host's immunity by cryo-therapy.

Cryosurgery

[Modification of immunoglobulins during combined peroral and parenteral nutrition in colitis, ileitis and rectal carcinoma].

Reaction of immunoglobulins (IgA, IgG, IgM) during a four week period under condition with a special parenteral and peroral alimentation was observed in three groups of patients (rectum neoplasm, ulcerative colitis, Crohn's disease) during a period of two weeks all patients received 150 g aminoacids and 510 g carbohydrates by central venous catheter. In the following 14 days parenteral nutrition was limited and oral alimentation began. All patients showed a remarkable approvment of recreation. Globulins rose in between normal spectrum. The most marked changes with a significant uprising of IgG showed patients with rectum carcinoma treated cryosurgically.

Colitis, Ulcerative

[Healing process of colonic anastomosis after contamination with Staphylococcus aureus (author's transl)].

In 30 rats the healing process of colonic anastomosis contaminated with Staphylococcus aureus was studied. At the same time on each animals a single- and double-layer anastomosis was performed. All sutures were infected during the surgical procedure with highly infectious S. aureus. The rats were sacrificed after 1, 2, 3, 4 days, and 2 1/2 weeks. Criteria for estimation were mainly visible abscess formations, leakage spots, bacteriologic and histologic examinations. After apparent inflammatory reactions were seen on the first postoperative days with a maximum on the 4th day, all sutures were completely healed and free of S. aureus after 2 1/2 weeks. As for the extent of infectious inflammation, the single-layer suture technique proved to be more resistant than the double layer.

Animals

[Dietetic and antibiotic preparation in surgery of the large intestine].

Besides patient's cardiac, pulmonary, renal and hepatic status, a correct operative technique and mechanical bowel cleansing the protein status is of central importance to prevent anastomotic leakage and wound infection in colon surgery. Using an oral elemental diet prior to surgery both sufficient caloric and protein intake and reduction of intestinal flora can be achieved. Wound infection and anastomotic leakage are reduced significantly without the side effects of antibiotic bowel preparation. If oral antibiotics are used preoperatively only a short term low dose regimen is effective for the control of infection after colon surgery which reduces the concentration of aerobes and anaerobes.

Anti-Bacterial Agents

[Local cryosurgical management of colonic and rectal cancer].

Complex experimental investigations created the basis for cryosurgical treatment of colon and rectum cancer. 27 patients were chosen for cryotherapy according to strict indication. Especially developed instruments and technique are presented. A three year-follow up-study shows 19 patients alive, most of them without recurrence or troubles. Immediately after starting with cryosurgical treatment patients recover remarkably. In collaboration with the Dept. of Microbiology (Rhein.-Westfäl. Techn. Hochschule Aachen) the possibility of a special cryoimmunological response is investigated.
