Synthesis and anticonvulsant activity of some potential thiazolidinonyl 2-oxo/thiobarbituric acids.

A series of 5-[(N-substituted benzylidenylimino)amino]-2-oxo/thiobarbituric acids (3a-3h) have been synthesized by the condensation of 5-hydrazino-2-oxo/thiobarbituric acids (2a-2b) with various aromatic aldehydes. Cycloaddition of thioglycolic acid to 3a-3h, yielded 5-[(2'-substituted phenyl-4'-oxothiazolidin-3'-yl)amino]-2-oxo/thiobarbituric acids (4a-4h). All these compounds were screened, in vivo, for their anticonvulsant activity and acute toxicity studies. Compounds 4f and 4g were found to be most potent compounds of this series and were compared with the reference drugs, phenytoin sodium, lamotrigine and sodium valproate. The structures of these compounds have been established by IR, 1H NMR and mass spectroscopic data.

Second trimester diagnosis of Neu Laxova syndrome.

This is the first report of a prenatally diagnosed case of Neu Laxova syndrome (NLS) from India. This also includes a case of NLS in monochorionic diamniotic twins and two more cases in which we were able to detect most of the features of NLS as early as 19 to 20 weeks by routine antenatal ultrasonography. Severe intrauterine growth retardation (IUGR), microcephaly, central nervous system (CNS) abnormality, joint contractures, and abnormal facies are the major diagnostic features observed in prenatal ultrasonography. Risk factors such as consanguinity and history of intrauterine death or stillbirth in siblings have been noted in all the cases, but none of the three families that were reported had previously had an affected fetus. The spectrum of skin manifestations and frequency of occurrence of major clinical features of the syndrome have been discussed. Review of the literature on NLS and possibility of detecting the syndrome in the second trimester is discussed.

Urinary fluoride levels and prevalence of dental fluorosis in children of Jhajjar District, Haryana.

A study was conducted in the Jhajjar district of Haryana to assess the fluoride excretion in the population exposed to environmental fluoride and efforts were being made to correlate the results with dental fluorosis. The mean fluoride concentration in drinking water samples of Bahadurgarh, Beri, Jhajjar, Matanhail and Sahalawas blocks of Jhajjar district were 2.05 mg/L, 2.14 mg/L, 2.05 mg/L, 2.14 mg/L and 1.93 mg/L respectively. The mean urinary fluoride concentration was 1.58 mg/L in Bahadurgarh, 1.48 in Beri, 1.50 in Jhajjar, 1.51 in Matanhail and 1.56 in Sahalawas. The mean prevalence of fluorosis on the basis of stage of dental fluorosis was highest of yellowish brown type and on the basis of TSIF score, score 4 type fluorosis was highest in most of the blocks of Jhajjar district. The study revealed that more than 50% of the individuals were found to be affected with flurosis in this district.

Tranquilizing, antihistaminic and purgative activity of Nyctanthes arbor tristis leaf extract.

The decoction of the leaves of Nyctanthes arbor tristis Linn. (Harsingar) is widely used in Ayurvedic system of medicine for the treatment of sciatica, arthritis, fevers, various painful conditions and as laxative. In the present investigation, the water soluble portion of the alcoholic extract of the leaves was screened for some CNS activities (viz. hypnotic, tranquilizing, local anaesthetic, hypothermic, anticonvulsant), antihistaminic and purgative activities. The extract produced general depression of spontaneous motor activity, significantly increased pentobarbitone sleeping time though it had no effect on righting reflex. Furthermore higher doses of the extract abolished CAR without affecting motor coordination. Moreover the extract exhibited hypothermic effect and protected guinea pigs from histamine aerosol. These activities are common to major tranquilizers and support the usage of the plant by Ayurvedic physicians in aforementioned conditions. In addition significant purgative activity was also exhibited by the extract.

Evaluation of analgesic, antipyretic and anti-inflammatory activity of spirobarbitunylphenothiazines in rodents.

Analgesic, antipyretic and anti-inflammatory activities of newly synthesized spirobarbitunylphenothiazines viz 10-[7, 11-Di(4-4' dimethoxphenyl)-3-oxo-9-methylaminoimino-2, 4-diazaspiro [5.5] undecane 1, 5 dione] acetylphenothiazine (test drug A) and 10-[7, 11-Di (N.N-dimethylaminophenyl)-3-oxo-9-methylaminoimino-2, 4-diazaspiro [5, 5] undecane-1, 5 dione] acetylphenothiazine (test drug B) have been screened in Swiss mice and Wistar rats. The peripheral analgesic activity of test drugs A and B was investigated by acetic acid induced writhing test in Swiss mice while the central analgesic action was assessed by hot-wire (tail flick test) of the analgesiometer and tail-clip test in Wistar rats. Antipyretic activity was assessed on Brewer's yeast induced pyrexic model while antiinflammatory activity was seen on carrageenan induced hind paw oedema. Analgesic activity was found to be only of peripheral type as there was reduction of 66% in writhing responses by test drugs A and B in dose of 80 mg/kg in mice. No change in the tail flick responses was observed on analgesiometer or by tail clip by both the test drugs. Reduction of 1.5 to 2.0 degrees C in rectal temperature was observed in pyretic rats by test drugs A and B in dose of 80 mg/kg. 80% reduction in paw volume was noted in 80 mg/kg dose of both the test drugs which was comparable to the anti-inflammatory activity of 300 mg/kg, p.o. of phenylbutazone.

Glucose-6-phosphate dehydrogenase deficiency in blood donors: screening by micromethaemoglobin reduction test.

500 blood donors were screened for G6PD deeiciency using micromethaemoglobin reduction (microMRT) test. Most of the blood donors were young adult males (95.8%). The overall incidence of G6PD deficiency was found to be 0.8%. There, was apparently decreased frequency of G6PD deficient blood donors with increasing age, and no relation could be ascertained between G6PD) deficiency and blood groups.

[Acute suicidal poisoning with Dapsone].

Course of disease and treatment of 27-years old female patient suffering from celiac disease, who took Dapsone on suicidal purpose in a dose 23 times over maximal daily dose. Methemoglobinemia and hemolysis with anemia typical for this poisoning have been observed. In the first period after intoxication hypokalemia of unknown reason and shedding of hair 7 weeks after intoxication have been noticed. It is remarkable that methemoglobinemia maintained until 8 day after intoxication, in spite of intensive treatment with reducers and blood transfusions. That indicates tissular accumulation of Dapsone. Patient treated with Dapsone shouldn't possess the quantity of this drug that-taken as a single dose-can put him at risk.

Accidental oral poisoning with carbamate in a 26-year-old patient.

Acute, accidental, oral poisoning with carbofuran in a chronic alcoholic was presented. In clinical picture muscarinic and nicotinic symptoms dominated. Toxicological examination revealed non acetylcholinesterase activity. A role of an interview and physical symptoms in diagnosing the poisoning with cholinesterase inhibitors, a specific treatment conduct and its side effects were discussed. So far six cases of poisoning with cholinesterase inhibitors have been treated at II Department of Internal Diseases and Acute Poisoning in Tarnów. The case presented below was characteristic for a considerable intensification of side effects during the treatment conduct. It was the reason for presenting this case.

Modification of glybenclamide hypoglycaemia by verapamil in rabbits.

The study was designed to demonstrate the interaction between verapamil and glybenclamide on blood glucose in rabbits. Glybenclamide (0.05 mg/kg, po) induced hypoglycaemia was observed 1 h after treatment and persisted till 3 h. Verapamil (8 mg/kg, sc) per se produced hyperglycaemia which lasted for 2 1/2 hr. Concurrent administration of verapamil was found to impair significantly the hypoglycaemic response of glybenclamide.
