Quality of residential settings for elderly adults.

Based on the Multiphasic Environmental Assessment Procedure (MEAP), an instrument for evaluating residential settings for elderly adults, eight indices of quality were formulated to cover both the structure and process of care. MEAP data were available for 244 facilities from throughout the United States, including 127 nursing homes (NHs), 55 residential care facilities (RCs), and 62 congregate apartments (APTs). The three types of facilities differed predictably on the quality indices. Nursing homes were more supportive and restrictive than were APTs, and RCs generally scored between NHs and APTs on the quality of care measures. These indices of quality tended to be independent of one another, suggesting that quality of care is multidimensional. Larger facilities and nonprofit facilities tended to score higher on the quality indices, although the effects of size varied somewhat with facility type and ownership. These indices may be useful for describing and comparing settings and for analyzing the correlates of quality.

[Visually controlled space orientation in endogenous psychoses].

The visual controlled behaviour in space is measured in 30 psychotic patients. In a room with a row of 8 equal chairs we measured the right-left-choice and the choice of distance. The neglect of the left chairs in patient with "distrust" and "delusion" let us suppose a right hemispherical dysfunction. There is no correlation between psychosis and choice of distance, but a good correlation between disturbed parental home and near distance.

[Biomagnetism from the neuropsychiatric viewpoint--principles of recent developments in diagnostic procedure].

The following survey is intended to demonstrate and discuss some principle problems of biomagnetism and the application in clinical fields. The development of special technical parameters especially by the SQUID method is initiating new possibilities of research and completing the neurobiologically determinated basis and conditions of neuropsychiatric disorders. The Magnetoencephalography (MEG) will be represent an important role of a new functional diagnostic method.

[Finger spreading as an indicator of psychomotor activity and laterality].

In 23 healthy right- and left-handers and in 8 psychotic patients the angels between fingers were measured after maximal spreading. It can be demonstrated, that there are differences in spreading out between right- and lefthanders. The dominance of the right hand in right-handers is fluctuating. In depressiv psychotics and patients after schizophrenia we found decrease of spreading out. The laterality imbalance in psychoses we interprete as a sign of hemispherial dysfunktion.

[Drinking behavior in the chronic phase of alcoholism].

40 patients in the chronic phase of alcohol addition were questioned in order to identify drinking stereotypes with regard to consumption in the course of the day and year. Alcohol intake reaches a maximum between 8 and 9 PM. No clear differences were found in this respect between days of abstinence for social reasons and days without such social pressures. In the course of the year, alcohol consumption reaches a maximum in autumn and a minimum in spring. The uniform, repetitive drinking rituals are suspected to have become habitual in the course of several years until they are finally unaffected by environment. This leads to a convergence of addiction forms, but the duration of the daily abstinence period still permits differentiation between gamma and delta addicts even when addiction has reached an advanced stage.

[Retinal hemodynamics in cerebrovascular accidents].

In 32 patients the times of retinal passage were determined in 112 examinations immediately after cerebrovascular insult and following in daily intervals. In this case after the insult a significant change of the retinal haemodynamics could be observed: the bolus-tension time and the time V2 showed a continuous deterioration of the blood supply of the retina possibly parallel to the cerebral haemodynamics. Furthermore it was shown that the initial prolongation of the arm-retina-times over 17 seconds refers to serve cerebral lesions with poor prognosis. It seems that the method described is not suitable for the preliminary diagnosis of extracranial vascular processes: In acute extracranial vascular occlusion we found equality of the sides of the inflow of dye as well as differences of the sides of more than 30 seconds in free extracranial vascular lumina.

The suprapersonal environments of sheltered care settings.

The Resident and Staff Information Form (RESIF) measures the aggregate characteristics of residents and staff of sheltered care settings (the suprapersonal environment) in terms of nine conceptually unified dimensions. The dimensions include measures of residents' social backgrounds, current functioning, and use of facility services. The development of the RESIF is outlined, nd psychometric characteristics, based on data from 93 representative facilities, are presented. Relationships among RESIF dimensions and between these dimensions and facility characteristics are examined. The data show that the aggregate characteristics of residents vary markedly among facilities. The suprapersonal environment in turn is related to other facility characteristics. Residents who are high on social resources or whose functional abilities are high are likely to be more active and to live in facilities that provide more physical resources, more control, and higher cohesion, organization, and physical comfort. The results indicate the existence of mechanisms of selection and social allocation that determine where a person is likely to live.

[Non-invasive cardio-circulatory diagnosis with the help of retinal dye clearance].

Using the fluorescence microscopy the times of retinal passage A 1, V 1, A 2 and V 2 were determined in 36 patients with heart diseases. They were compared with different haemodynamic parameters, in 27 patients with intracardially got circulatory indicators. The time of the renal circulation (V 1-A 1) in certain respect correlastes with the time volume of the heart, the bolus-extension time (A 2-A 1) with the degree of insufficiency of the heart. The value V 2 appears as a very sensitive, unspecific measuring value, which is also influenced by the peripheral haemodynamic situation. The method seems to be suitable as non-invasive method. Its advantages consist in the small temporary, personal and material expenditure.

Assessing the institutional policies of sheltered care settings.

The Policy and Program Information Form (POLIF) measures the policies and services of sheltered care settings in terms of ten conceptually unified dimensions. The development of the POLIF is described, and psychometric characteristics, based on data from 93 representative facilities, are presented. These data show that facilities providing a higher level of care tend to provide residents with less choice and control. Larger facilities tend to be more selective, to offer more health services and social activities, and to have formal means for transmitting expectations and involving residents in decision making. Sample profiles illustrate the usefulness of this instrument for describing and comparing facilities; additional uses are discussed.

Assessing the physical and architectural features of sheltered care settings.

The Physical and Architectural Features Checklist (PaF) measures the physical resources of sheltered care settings in terms of nine conceptually unified and empirically derived dimensions. The development of the PAF is described, and psychometric characteristics, based on data from 93 representative facilities, are presented. These data show that facilities which have more physical resources are seen as attractive by outside observers and pleasant by residents. Larger facilities are more likely to provide barrier-free environments, more flexible physical settings, and more staff facilities. Cost is not related to any PAF dimension; however, nonprofit facilities score higher on several dimensions than do proprietary facilities. Richer physical settings tend to offer residents more policy resources. They are more selective, provide more privacy, allow residents a higher degree of influence and choice, and have clearer policies. Sample profiles illustrate the usefulness of this instrument for describing and comparing facilities; additional uses are discussed.

[The passage of dyes through the retinal vessels].

Examinations of the retinal haemodynamics after injection of fluorescein sodium were performed on three groups of test persons, in normal persons, patients with diseases of the retina as well as patients with organic heart defects. In the group of heart diseases statistically significant disturbances of the retinal haemodynamics could be proved, which the retina tolerates without any lesion. In comparison to this retinopathies with severe changes of the fundus show only slight disturbances of the haemodynamics. These do not give the possibility of the explanation of the changes of the fundus.
