De novo Kaposi's sarcoma in renal transplantation. Case report and brief review.

This report describes a de novo development of Kaposi's sarcoma in a Puerto-Rician man 9 months after a cadaveric renal transplant. Progression of the disease was observed despite local irradiation, while the patient remained immunosuppressed with prednisone and azathioprine. This was accompanied by depressed immunologic tests. Discontinuation of azathioprine and addition of chemotherapy (bleomycin and vincristine), while continuing prednisone to maintain functional survival of renal allograft, has led in this patient to regression of extensive cutaneous and suspected pulmonary Kaposi's sarcoma lesions. The possible importance of a depressed immunosurveillance mechanism and activation of latent oncogenic virus by the presence of an allograft in the de novo appearance of Kaposi's sarcoma in transplant recipients is briefly discussed.

Arteriovenous fistulas for hemodialysis in 100 consecutive patients.

Arteriovenous fistulas were performed in 100 patients over a three year period with a 90 per cent overall success rate. Various technics for construction of the fistula using autologous veins are discussed, with emphasis on pre- and intraoperative assessment of vessel condition and its relation to the prevention of immediate and delayed failures.

The physiotherapist in the developmental management of the visually impaired child.

The role of the developmental physiotherapist, rather than the general physiotherapist, in a developmental vision team is described. Physiotherapy can contribute to both the assessment and intervention programmes. Motor development and associated postural reactions will be trained, abnormal postures prevented or decreased, and body image and use of residual vision developed. Selection and supervision of aids and play equipment, as well as decreasing of physical handicap, are also included in the physiotherapist's role in helping the child and his family.

Identification of constraints acting on motor development in young visually disabled children and principles of remediation.

Babies and preschool children with severe visual disabilities have been studied in a developmentally orientated clinical research setting. Severe congenital visual disability delays and alters development in all areas; the impact on motor development is complex with much secondary to delays in other areas. The constraints on motor development are identified and discussed together with ideas for remediation and developmental prevention.

Fistula-in-ano: a prospective study of 107 patients.

BACKGROUND: While the majority of fistulas-in-ano are anatomically simple and easy to treat, a significant number are high or anatomically complex and have the potential to become a major management problem. METHODS: One hundred and seven consecutive patients undergoing surgery for fistula-in-ano were studied prospectively with standardized anatomic diagrams. RESULTS: Fistulas were classified as superficial (15%), intersphincteric (43%), trans-sphincteric (35%) or 'high' (7%). Within each group fistulas were considered either simple or complex (high tracks, extra tracks or other complications). Trans-sphincteric fistulas were more often complex than intersphincteric fistulas (32 vs 6%). A prior history of perianal sepsis and surgery was more frequent among the trans-sphincteric and 'high' groups. An external fistula opening within a narrow are 30 degrees either side of the posterior midline was almost always associated with a simple superficial or intersphincteric fistula (97%). Anterior and especially posterolaterally located external openings were frequently associated with complex fistulas (16 and 47%, respectively) and often had trans-sphincteric or 'high' tracks (58 and 56%). Goodsall's Law was more accurate for posterior (91%) and intersphincteric (93%) fistulas than for anterior (69%) and trans-sphincteric (68%) fistulas. Histopathology of fistula material showed unremarkable fistula-in-ano in 87% of requests. Six patients had unexpected abnormal results, including three new diagnoses of Crohn's disease. CONCLUSIONS: The presence of additional anatomic complexity should always be anticipated in trans-sphincteric fistulas. Trans-sphincteric and 'high' fistulas are more likely to occur in females, and in patients with previous perianal sepsis or surgery for fistula. External openings close to the posterior midline almost always underlie simple fistulas, whereas posterolateral external openings are predictive of complex fistulas.

Fine needle aspiration diagnosis of malignant lymphoma with confirmation by immunoperoxidase staining.

Cytospin preparations of fine needle aspirates in 14 cases of suspected lymphoma were studied by immunoperoxidase techniques. The combination of cytologic smears and immunoperoxidase studies resulted in a working diagnosis in 13 of the 14 cases. The immunologic markers in conjunction with the cytologic appearance of the aspirates were reliable and consistent in differentiating between malignant and benign lymphoproliferative lesions and in determining the B-cell or T-cell nature of the process.
