Villous lipomatous proliferation of the synovial membrane (lipoma arborescens).

The cases of five patients (7 knees) who had villous lipomatous proliferation of the synovial tissue (lipoma arborescens) are reported. The longest duration of symptoms was thirty years. Six of the seven knees were treated by synovectomy. The length of follow-up ranged from two to eleven years, and six knees were followed for five years or more. The patients who were treated by synovectomy had complete healing of the synovial lesion, but the osteoarthritis tended to progress, especially in patients who had long-standing symptoms. Because of the clinical, macroscopic, and microscopic features of this rare disease, we suggest discarding the name lipoma arborescens, which implies that the lesion is a neoplasm, and using the more descriptive term of villous lipomatous proliferation of the synovial membrane.

Huge mesenteric mucinous cystadenoma in normal pregnancy.

A 36-year-old woman required laparotomy early in the puerperium after a normal delivery because of the presence of a large abdominal cyst. At laparotomy, a large mucinous cystadenoma was found attached to the root of the ileal mesentery; the ovaries were normal. This is the third case of mesenteric cyst published in association with pregnancy, but the first with a mucinous pattern. Pathologic aspects, possible complications, and treatment are discussed.

Frozen-section diagnosis in surgical pathology. A prospective analysis of 526 frozen sections.

Five hundred eighty-six consecutive frozen-section consultations performed during a 1-year period were studied prospectively in order to assess the accuracy of the method and develop a quality control mechanism. The overall accuracy was 97.1%. The accuracy of the method with breast lesions was 97.9%. Specimens from the gastrointestinal tract and thyroid were incorrectly interpreted in 5% of the cases. The accuracy for lymph node specimens was 96.2%, with more than 50% consulted out of curiosity. The authors conclude that frozen section of lymph node is not recommended. Most of the errors were sampling errors made by the pathologist. The authors therefore conclude that in clinically suspected malignancy, more than one sample must be examined in order to decrease the false-negative diagnosis in frozen section.

Metachronous leiomyosarcoma in colostomy after abdominoperineal resection for rectal carcinoma.

Leiomyosarcoma of the colon is a rare neoplasm. It is widely known that primary malignant lesions may present either simultaneously (synchronous tumor) or at different times (metachronous tumor). The incidence of metachronous colonic mucosal carcinoma is generally considered to range from 3 to 5%. Metachronous leiomyosarcoma in colostomy 4 years after abdominoperineal resection for rectal carcinoma is unusual and we have found no reports on this in the literature. The clinical symptoms, diagnostic features, and mode of treatment are discussed.

Double mitral valve orifice.

A two-day-old baby with a type-I truncus arteriosus and double mitral valve orifice is reported. The double mitral valve orifice and additional lesions of each subdivision of the mitral valve were clearly shown by two-dimensional echocardiography. The medial orifice had a cleft leaflet and the lateral one a parachute-like disposition of the tension apparatus. This is the first report of double mitral valve orifice associated with truncus arteriosus, and the first in which anomalies of the subdivisions of the mitral valve were detected by two-dimensional echocardiography.

Renal hemodynamic changes in humans. Response to protein loading in normal and diseased kidneys.

This study was undertaken to define the renal hemodynamic changes that mediate the acute response to an oral protein load. Three groups of subjects were studied: (1) disease-free subjects; (2) patients with chronic renal disease of various causes, except for diabetes mellitus, documented by history and/or renal biopsy; and (3) patients with diabetes mellitus, that is, a history of hyperglycemia requiring antihyperglycemic therapy. All subjects were studied before (baseline) and after (test) ingestion of a protein load. Glomerular filtration rate and effective renal plasma flow were evaluated by inulin and para-amino-hippurate, respectively. In the disease-free subjects, the mean baseline glomerular filtration rate and renal plasma flow were 122 +/- 10 ml/minute/1.73 m2 and 644 +/- 64 ml/minute/1.73 m2, whereas test glomerular filtration rate and renal plasma flow were 151 +/- 15 ml/minute/1.73 m2 and 791 +/- 111 ml/minute/1.73 m2, respectively. In patients with chronic renal disease, the test glomerular filtration rate and renal plasma flow were related to the severity of the disease. The more severe the disease, the lower the absolute test values and the smaller the increment from baseline to test values. Patients with diabetes mellitus had a paradoxic response to ingestion of a protein load. Glomerular filtration rate fell while renal plasma flow remained unchanged. This response was observed in all diabetic patients regardless of the type of diabetes or whether clinical evidence of diabetic nephropathy was absent, minimal, or severe.

Efficacy of vincristine and cyclophosphamide in the therapy of recurrent medulloblastoma.

We conducted a Phase II study of combination therapy with vincristine and cyclophosphamide in the treatment of patients with recurrent or metastatic medulloblastoma. Fourteen patients were treated with vincristine 2 mg/m2 (2.0-mg maximal dose) by intravenous bolus on Day 1 and cyclophosphamide 1 g/m2 by intravenous infusion on Days 1 and 2, with cycles repeated every 4 weeks. All 4 patients with extraneural disease (biopsy-proven bony metastases) responded (duration of responses 2+, 6+, 8, and 16+ months) and 4 of 8 evaluable patients with neuraxis disease responded (duration of response 2, 2+, 2+, and 21+ months). Toxicity was limited to neutropenia without any episodes of infection. These therapeutic results compare favorably with other reports of therapy for recurrent medulloblastoma and support the inclusion of vincristine and cyclophosphamide in randomized adjuvant therapy trials of patients with medulloblastoma.

Inflammatory fibroid polyp of sigmoid colon.

A case of inflammatory fibroid polyp of the sigmoid colon is presented. This is the eight case of this type of polyp in the colon and, to the best of our knowledge, the first one involving the sigmoid and producing intussusception. Symptomatology of the inflamed fibroid polyp in this part of the gut closely simulates gastrointestinal malignancy. The treatment is surgical excision of the polyp, or colonoscopic resection when it is possible. Intraoperative colonoscopy helps the surgeon to localize the lesion and to role out the existence of other lesions.
