[Accessory spleen: echographic images].

In a three-year period, 25 cases of accessory spleen were diagnosed by means of ultrasound. Accessory spleens were demonstrated to be eterotopic nodes of normal splenic tissue, situated usually near the main spleen, to which they may be bound by a pedicle. Their dimensions are usually less than 2 cm. Accessory spleen is usually present in a 10% of normal subjects and represents a TC or US occasional finding. When large, the accessory spleen causes problems about a differential diagnosis with neoplasms of the left upper quadrant. It must not be confused with polysplenia or with splenosis; TC, angiography and scintigraphy may be useful in these cases.

A combination of cefuroxime and N-acetyl-cysteine for the treatment of lower respiratory tract infections in children.

One hundred and three children with lower respiratory tract infections were treated with a combination of cefuroxime and N-acetyl-cysteine. Positive clinical results were obtained in 100 patients, symptoms being completely relieved in 58 and distinct improvement produced in 42. Chest x-rays provided objective evidence of recovery or improvement in these 100 patients. Of the 72 pathogenic strains isolated before treatment in the trial population as a whole, none were still detectable at the end of treatment. No side effects or adverse reactions of any kind were observed.

Gore-Tex prosthetic ligament in anterior cruciate deficient knees.

Reconstruction for symptomatic anterior cruciate deficient knees has yielded varying success rates. Prosthetic cruciate replacement has recently become a potentially attractive alternative. The results of the Gore-Tex polytetrafluoroethylene ligament, which is intended as a permanent replacement, are reported. Eighty-two patients were followed prospectively, mean age was 28 years (range, 16 to 51 years) and mean followup was 18 months (range, 12 to 30 months). Subjective scores improved in all categories, including pain, swelling, giving way, locking, and stair climbing. All patients without complications had no episodes of actual giving way, considered themselves improved, and returned to activities of daily living at 3 weeks and athletics at 8 months. Range of motion lacking at 3 months was 2 degrees of extension and 10 degrees of flexion, and at 12 months was 0 degrees of extension and 4 degrees of flexion. All mean objective data, including the anterior drawer, Lachman, and pivot shift, demonstrated improvement at final followup. Cybex testing revealed improvement in relative quadriceps strength from 88% to 99%. The KT-1000 Arthrometer showed improvement in the injured-normal knee difference score throughout the follow-up period. Of importance is that while final objective data was improved over initial data, an early nonprogressive shift toward loosening was indicated by worsening of the drawer, Lachman, pivot shift, and KT-1000 scores. This shift may be attributed to resorption of interposed soft tissue, creep, or loosening of the graft. Subjective scores remained stable after improving. Complications included four ruptures, four chronic sterile effusions with partial attenuation, one infection, and one symptomatic loose body.(ABSTRACT TRUNCATED AT 250 WORDS)

Influence of posts and cores on light transmission through different all-ceramic crowns: spectrophotometric and clinical evaluation.

PURPOSE: The influence of different types of posts and cores on light transmission through all-ceramic crowns was assessed by spectrophotometric analysis and clinical evaluation. MATERIALS AND METHODS: Three extracted natural teeth were replicated in acrylic resin, with roots prepared to receive standardized posts and cores. Using a silicone impression material as a template, various prosthodontic reconstructions were obtained by combining four types of posts and cores (polished and matte-finished gold alloy, all-ceramic, and ceramized metal alloy) and three types of all-ceramic crowns (IPS-Empress 2 surface-colored, IPS-Empress 2 stratified, and In-Ceram). The spectrophotometric analysis was performed in the dark at 25 degrees C. The teeth were backlit with an incandescent lamp at the color temperature of A illuminant and shielded to avoid spurious light entering the spectrophotometer. The transmitted light was analyzed in terms of luminance at four points of the sample surface (cervical, middle, incisal, and proximal). Twelve measurements were performed for the natural teeth, and 144 were performed for the artificial teeth. RESULTS: Natural teeth had the highest luminance. Among all-ceramic crowns, surface-colored IPS-Empress 2 had the highest luminance, and stratified IPS-Empress 2 had the lowest. Regarding posts and cores, the luminance was highest with the all ceramic, lower with the ceramized and the polished gold alloy (which had very similar luminance), and lowest with the matte-finished gold alloy. No significant difference among prosthodontic combinations was detected under clinical observation. CONCLUSION: The surface-colored glass ceramic (IPS-Empress 2) was the most translucent crown. At the standard crown thickness used, there were small, significant spectrophotometric, but not clinical, differences among the combinations tested. These findings show no esthetic contraindications for the use of polished gold alloy posts and cores with all-ceramic crowns.

[Echotomography in the study of the pathology of Achilles tendon].

Achilles tendon was evaluated by B scan ultrasound in 54 cases (20 normal and 34 symptomatic subjects). The ultrasonographic features of tendon's diseases were correlated with the anatomical patterns. On the basis on their experience, the authors concluded that ultrasonography is a simple and highly accurate method for the diagnosis of Achilles tendon diseases and that it should be the initial imaging procedure.
