Pregnancy from cryopreserved embryos: an important new aspect of assisted reproduction.

Assisted Reproduction techniques have become well-established methods of treating infertility in the decade since their inception. The gradually improving efficacy of these procedures has made the risks of multiple pregnancy a serious concern. Cryopreservation of "extra" embryos has been developed to counter this problem, but has led to new difficulties. Assisted Reproduction in Kentucky has kept pace with progress worldwide; the first Kentucky live birth from transfer of cryopreserved embryos is reported.

Clinical comparison of immunoradiometric assays for intact versus beta-specific human chorionic gonadotropin.

Monoclonal immunoradiometric assays (IRMA) for human chorionic gonadotropin (hCG) are available for either intact hCG (IhCG) or beta subunit hCG (beta hCG). The authors evaluated the clinical applications of both methods. Serum samples (N = 180) were divided into the following five clinical groups: Group 1: elevated luteinizing hormone (LH), follicular stimulating hormone (FSH), and thyroid-stimulating hormone (TSH), which share alpha subunits with hCG; Group 2: pregnancy; Group 3: trophoblastic tumors; Group 4: malignancy; and Group 5: positive rheumatoid factor or anti-nuclear antibody (ANA). The values of beta hCG versus IhCG for Group 1 showed statistical significance but no clinical significance, indicating negligible cross-reactivity in the alpha subunit group. beta hCG values, although exceeding the IhCG values, correlated well (r = 0.97) in intrauterine pregnancies; however, these values were not believed to be clinically significant. There is negligible interference by alpha subunits, elevated rheumatoid factor, ANA, or malignancy in the determination of IhCG versus beta hCG. The authors conclude that either the IhCG or beta hCG assays may be used in clinical conditions in which a potential exists for interference of alpha subunits, autoantibodies, or heterophile antibodies or in malignancy.

Diet and the risk of breast cancer.

This article presents a review of the epidemiologic evidence on the relationship between breast cancer risk and nutritional factors. Particular attention is given to the relationship between breast cancer and dietary fats. Other factors discussed include intake of alcohol, micronutrients, and caffeine.

Vasoactive intestinal polypeptide immunoreactivity in the spinal cord of the guinea pig. A mapping study.

The distribution of vasoactive intestinal polypeptide-immunoreactive (VIP-IR) neurons in the lower medulla oblongata and the spinal cord has been analyzed in guinea pigs. This study includes results obtained by colchicine treatment and transection experiments. In the spinal cord, numerous VIP-IR varicosities were observed in the substantia gelatinosa of the columna dorsalis; some were also found in the substantia intermedia and the columna anterior. The spinal VIP-IR nerve fibers were mainly of intraspinal origin and oriented segmentally. VIP-IR nuclei in the spinal cord extended dorsally into corresponding regions of the caudal medulla oblongata, namely from the substantia intermedia medialis and lateralis into the vagus-solitarius complex and from the nucleus spinalis lateralis into the area of the nucleus reticularis lateralis. Additional VIP-IR perikarya were observed in the pars caudalis of the nucleus spinalis nervi trigemini. The VIP-IR nuclei within the caudal medulla oblongata probably form a continuous system with those localized within the spinal cord. They may be involved functionally in the modulation of cardiovascular and respiratory regulation in the guinea pig.

Factors influencing fertility after ectopic pregnancy.

Seventy-one cases of ectopic pregnancies that had been treated by salpingectomy or salpingo-oophorectomy were evaluated. All patients were attempting pregnancy after the surgical procedure. During a follow-up that ranged from 3 to 13 years the overall conception rate was 62%. Excluding the ectopic pregnancies, the uterine pregnancy rate was 52%. The repeat ectopic pregnancy rate was 12.6%. The uterine pregnancy rate was higher for patients treated by salpingectomy (p less than 0.02). The repeat ectopic pregnancy rate was similar after salpingectomy and salpingo-oophorectomy. The uterine pregnancy rate in women with a previous history of pelvic inflammatory disease was poor (19%), and those patients had a high incidence of recurrent ectopic pregnancy (27%). The pregnancy rate was higher in women who were under age 25 at the time of the ectopic pregnancy. Parity did not have an effect on future fertility.
