Neurolinguistic analysis of the language abilities of a patient with a "double disconnection syndrome": a case of subangular alexia in the presence of mixed transcortical aphasia.

In contrast to the classic form of alexia without agraphia, subangular alexia results from a single lesion located deep in the white matter of the left parietal lobe. In the present report, a patient with subangular alexia and features of mixed transcortical aphasia is described. Neurolinguistic findings include: alexia without agraphia, paucity of spontaneous speech, moderate auditory comprehension difficulty, excellent repetition, echopraxia, colour agnosia, and naming disorder. Neurolinguistic tests revealed intact phonological organisation and grammatical filter." Our studies revealed a "double disconnection syndrome," the co-existence of two relatively rare neurobehavioural disorders. Furthermore, the studies reported here clearly show dissociations of language functions in both the visual and auditory modality, which demonstrates that the stages of language processing are separable.

Effect of cyclandelate on dementia.

Cyclandelate, a vasodilator, was administered to 24 patients with dementia. The dementia in these patients was presumed to be due to cerebral ischemia caused by atherosclerosis in cerebral vessels after other possible causes were ruled out. In a double-blind, cross-over study, patients received 200 mg of cyclandelate four times daily for six weeks and a placebo for six weeks. Six psychological tests, which reflect various aspects of higher cortical ability, were used to evaluate the effect of cyclandelate on the dementia. Cyclandelate was found to be no more effective than placebo in improving higher cortical function in these demented patients.

Metabolite composition in breast tumors examined by proton nuclear magnetic resonance spectroscopy.

BACKGROUND: In vivo characterisation of breast tumors using protein (1H) MR spectroscopy relies upon in vitro interpretation of tissue samples. The present study has investigated metabolite composition in extracts from breast tumors and non-involved breast tissue. Multivariate data analysis was used to determinate combinations of metabolites important for differentiation. MATERIALS AND METHODS: Tumor and non-involved breast tissue were obtained from 16 patients undergoing surgical treatment. 1H NMR spectra of perchloric acid tissue extracts were obtained at a BRUKER Avance DRX600 spectrometer. The data was analysed using principal component analysis and probabilistic neural networks. RESULTS: Low levels of glucose and high content of choline compounds were dominant findings in the tumor spectra. Principal component loadings demonstrated this strong association. The spectra were correctly classified using neural network analysis. CONCLUSIONS: Large differences in the metabolite composition of breast tumors and surrounding breast tissues have been documented.

The influence of progestins on receptor levels in breast cancer metastasis.

The influence of oral high-dose (HD) medroxyprogesterone acetate (MPA) and megestrol acetate (MA) treatment on steroid hormone receptor levels in metastatic breast tumor tissue was investigated. In ten postmenopausal women with advanced breast cancer, receptor biopsies were obtained from the same tumor before the start, and one and eight weeks after the start of oral HD progestin treatment. Endocrine treatment had been stopped in all patients at least five weeks before the start of progestin treatment. Estradiol receptor (ER) levels were reduced by 26.9% and 20.0% respectively after one and eight weeks of treatment. Progesterone receptors (PgR) were significantly reduced to undetectable levels, possibly due to MPA and MA binding to PgR. A stepwise decrease in androgen receptors (AR) was observed, indicating that the two progestins (MPA and MA) may also act through AR, and/or interfere with the replenishment of AR.

Serum concentration and drug effect after intravenous and rectal administration of diazepam.

In a randomized crossover study on sedation in outpatient oral surgery, the relation between the serum profile and the drug effect profile for intravenously (i.v.) and rectally administered diazepam was studied. Both sedation methods were found to be equally efficient at a mean dose of 0.25 mg/kg (range, 0.14-0.45) for i.v. administration, and 0.53 mg/kg (range, 0.50-0.58) for rectal administration. Both the serum concentration and the effect reached their mean peaks at the same time; however, this was 15 min later after rectal sedation than after i.v. sedation. Intravenous administration yielded a significantly higher serum concentration of diazepam at the clinical endpoint than did rectal administration, but the mean effect levels at the clinical endpoint were equal for both sedation methods. No linear correlation between log-serum concentration and the patient's estimation of effect was found.

Influence of tamoxifen on the tumor content of steroid hormone receptors (ER, PgR and AR) in patients with primary breast cancer.

A possible influence of tamoxifen (TAM) treatment on the measurable content of some steroid hormone receptors (i.e. ER, PgR, AR) from primary breast cancers has been evaluated. In eleven postmenopausal women, two biopsies were taken from the same tumor, the first biopsy before starting endocrine treatment, and a second biopsy after 8 weeks on tamoxifen treatment. A significant decrease in ER content was found both using a ligand binding (p < 0.005) and an enzyme immunoassay (p < 0.02). As for the PgR and AR content, no significant changes in the mean content were found.

Augmentation of the maxillary sinus floor with particulated mandible: a histologic and histomorphometric study.

This study presents the results of 10 consecutive patients treated with unilateral bone augmentation of the maxillary sinus floor using particulated mandible harvested from the symphysis. Endosseous titanium implants (Brånemark) were placed 6 months after the transplant procedure. Thirty self-tapping implants were placed with a mean follow-up period of 26 months (range 12 to 46 months). Bone biopsy specimens were obtained from the donor site and the residual alveolar process at the time of grafting. A second biopsy specimen was obtained from the grafted site after 6 months of healing (at time of implant placement), and a third biopsy specimen was obtained after 12 months of healing (at time of abutment placement). Histomorphometry was performed on microradiographs using a computerized image analysis system. The bone volume fraction at the donor site before particulation was 58% +/- 19%, and the volume fraction at the residual alveolar process was 45% +/- 15%. The mean volume fraction of bone in the grafted area increased from 40% +/- 12% after 6 months of healing to 48% +/- 10% after 12 months. Histology of the retrieved bone biopsy specimens revealed normal healing and bone maturation in all samples.
