Ultrastructural study of Helicobacter pylori-associated gastritis.

Endoscopic biopsies of antral mucosa from 26 patients with Helicobacter pylori-associated gastritis were studied by electron microscopy (EM). Scanning electron microscopy (SEM) showed clustering of H. pylori in the intercellular areas, being entrapped by the microvilli which were decreased at the sites where the bacilli were seen. The observations of SEM were confirmed by transmission electron microscopy (TEM), which showed adherence of the bacilli to the cell surface, producing cup-shaped depressions in the epithelial cells, and occasionally intracellular infiltration by H. pylori. There were also depletion of mucus granules, degenerative changes, and disruption of intercellular junction complexes of the epithelial cells. Post-treatment biopsies showed complete disappearance of the bacilli, and ultrastructural changes associated with H. pylori infection were resolved.

Flow cytometric analysis of cell surface carbohydrates in metastatic human breast cancer.

Helix pomatia agglutinin (HPA)- and Concanavalin A (Con A)-binding carbohydrate expression were studied on 32 tumour samples from primary adenocarcinoma of the breast and 12 samples from lymph node metastases. Live cells were spilled from each of the fresh samples and the extent of fluorescent-labelled HPA and Con A-binding was assessed by flow cytometry. The extent of brightness was expressed in a defined quantitative fashion and the percentage of positive cells was accurately determined from a sample of 10,000 cells per tumour. Correlation of binding with clinicopathological features showed that HPA but not Con A related to lymph node involvement (P = 0.001) in tumours of higher grade (II and III). Spilled tumour cells (non-lymphocytes) were selected from the lymph nodes and the presence of HPA binding cells in the involved lymph nodes was found to relate to positive HPA binding in autologous primary tumours (P = 0.002). Dual-label analysis of HPA and Con A binding showed characteristic features for each tumour. The study demonstrates the use of flow cytometry as a simple and effective technique in detecting differences in lectin binding in live spilled cells from fresh breast cancer tissues. This method may prove to be particularly useful if performed preoperatively on cells in fine-needle aspirates.

Use of LeuM1 monoclonal antibody for the diagnosis of Hodgkin's disease.

Monoclonal antibody to LeuM1, a granulocyte-related differentiation antigen, represents a highly effective reagent for detection of diagnostic "Reed-Sternberg" cells and variants in paraffin-embedded tissues of Hodgkin's disease. The "Reed Sternberg" cell in all the cases of Hodgkin's disease except lymphocyte predominance variety revealed positive intracytoplasmic/paranuclear granular staining with LeuM1 marker. The R-S cells in lymphocyte predominance variety contain probably sialylated LeuM1 antigen. All the cases of non-Hodgkin's lymphoma and reactive lymphadenitis showed no staining with LeuM1 monoclonal antibody. Therefore this antibody represents a potentially helpful diagnostic discriminant in the assessment of Hodgkin's disease and its distinction from non-Hodgkin's lymphomas and morphologically similar reactive lymphoid lesions.

Association of Helicobacter pylori with acid peptic disease in Karachi.

The prevalence of H. pylori infection in 200 dyspeptic patients undergoing upper G.I. endoscopy was investigated by histology and rapid urease test. H. pylori was associated with 86% cases of chronic gastritis, 84.6% cases of duodenal ulcers and 78.5% of gastric ulcers. None of the 15 histologically normal gastric biopsies showed H. pylori.

A placebo controlled trial of bismuth salicylate in Helicobacter pylori associated gastritis.

In a placebo controlled prospective clinical trial of bismuth salicylate in helicobacter pylori associated gastritis, 52 adult patients were randomly allocated to treatment with bismuth salicylate or placebo. Helicobacter pylori were totally cleared in 77% patients in bismuth group but none in placebo group (P less than 0.001). Resolution of gastritis (P less than 0.001) and improvement of symptoms (P less than 0.01) were significantly better in patients where H. pylori infection cleared as compared to patients where the infection persisted.

Correlation of endoscopic and histological diagnosis in upper gastrointestinal lesions.

A correlation between endoscopic and histological diagnosis was studied in 240 patients undergoing upper gastrointestinal endoscopy for dyspeptic symptoms. The biopsies from one or all the three sites were taken from oesophagus (158), stomach (193) and duodenum (146) according to the presenting symptoms of the patients. There was a better correlation between endoscopic and histological diagnosis when a positive finding was seen by the endoscopists as compared to when the endoscopy revealed no abnormality. One out of six benign looking ulcers in oesophagus and 1 out of 9 benign looking gastric ulcers turned out on endoscopy, to be malignant on subsequent histology. Inflammation of oesophagus, stomach and duodenum were the most commonly missed lesions on endoscopy.

Cellular and humoral immune responses in Campylobacter pylori-associated chronic gastritis.

Gastric cellular and humoral immune response investigated by immunoperoxidase staining of 53 antral biopsies showed significant differences in Campylobacter pylori-associated gastritis as compared with non-bacterial chronic gastritis and normal controls. IgA, secretory component, and complement C3 coated bacilli were seen in all cases of active chronic gastritis. C3 was always associated with coating by IgA, IgM, or both, which were rarely seen in gastritis without polymorphonuclear neutrophil infiltration. Intraepithelial mononuclear cellular infiltration was seen in 18 of 26 cases of C. pylori-associated chronic gastritis. The intraepithelial mononuclear cells stained positively for T cells and histiocytes.
