External fixator in management of fractures and leg lengthening.

The authors review the fundamental scientific factors that govern the healing of bone and conclude that they form a physiologic basis for the preferred use of external fixation in the management of fractures of the long bones as well as in the management of leg lengthening.

A simple and safe technique for tibial lengthening.

This is a report of long-term results and complications in 62 patients. Closed tibial osteotomy, direct tibial distraction, and simultaneous indirect proximal fibular epiphyseal distraction are the essential features of this new technique. Fibular osteotomy and complications from tibiofibular synostosis are eliminated. Bone graft and plating procedures have been proven to be unnecessary. The patient is ambulated immediately. Hospitalization has been reduced to a few days.

The incidence of scoliosis due to neurofibromatosis.

Cobb in 1950 stated that a high proportion of scolioses, classified as idiopathic, may be due to neurofibromatosis. This paper describes a retrospective study of a series of 3209 scoliosis cases to determine the incidence of neurofibromatosis. It was found to be just over 3 per cent. Two different clinical pictures of scoliosis were seen among the neurofibromatosis cases.

Tibial lengthening using a new extension device. Report of thirty-two cases.

Using a new type of external fixation device for lengthening the tibia, lengthenings of from 3.5 to 6.5 centimeters were accomplished in thirty-two children, eight to seventeen years old, the six complications, none of which jeopardized the result. The procedure was painless and the patients were ambulatory on crutches two to three days after operation, while lengthening was continued as an out-patient procedure.

Leg lengthening with a new type of external fixator (a preliminary report on 24 cases).

A new type of external fixator bas been designed for absolute fixation and prevention of lateral migration of fragments of bone. The apparatus has been used for leg lengthening. The apparatus and the technique are described and results of the first 24 cases are presented. Complications were minimal. The technique proved to be simple to apply, and painless to the patient with his ambulation from the start. And less expense are involved because of a very short period of hospitalization.
