[Menstrual cycle, opioids and hypothalamic amenorrhea: from physiology to pathology].

The central part of menstrual cyclicity is the arcuate nucleus within the mediobasal hypothalamus, which is shown to be the origin of GnRH. This neuronal oscillator is influenced by many endogenous substances and its periodically activity is modulated by them. The endogenous opioids are such substances. They are widely distributed within the organism and are endogenous ligands of opiate receptors. The endogenous opioids participate in many somatic functions such as regulation of ovarian cyclicity. Furthermore they are causal involved in hypothalamic amenorrhea.

[The influence of endogenous opioids in hypothalamic amenorrhea. Diagnostic and therapeutic aspects].

We investigated the possibility of induction of ovulation by means of chronic opioid receptor blockade with naloxone. Daily 4 mg were given as an intravenous bolus injection in 4 women suffering from hypothalamic amenorrhea till the 30th day of therapy. A possible maturation of ovarian follicles and a subsequent ovulation should be proved by means of daily determination of LH, FSH, estradiol and progesterone as well as of a sonographic folliculometry. The day prior to and the first day of naloxone treatment we took blood samples every 10 minutes during 4 hours for determination of LH pulse frequency and amplitude. Neither we found any alteration of the basal values of LH, FSH and estradiol, nor we observed a follicular growth. These results lead us to the conclusion to introduce a naloxone stimulation test as a further diagnostic step. In this way opioid mediated hypothalamic ovarian insufficiencies could be registered and a sufficient therapy could be reached.

[Echocardiography in the preoperative diagnosis of acquired heart valve diseases].

With the aim of minimizing the risk and stress of an invasive diagnostics, 102 patients with acquired heart valve diseases were examined by two-dimensional echocardiography combined with pulsed Doppler technique. The results were compared with those obtained at invasive diagnostics in 91 and/or during surgery in 52 of the patients studied. In the group of operated patients the sensitivity of non-invasive diagnostics was 89.5%, specificity 97.2%; analogous data for patients not subjected to surgery were 91.8% and 93.8%, respectively. The results of the study show that in the majority of patients with acquired valvular disease a complete preoperative invasive investigation is not necessary. Only in patients over 50 years of age the intended valve replacement should be preceded by additional coronarographic examination, as in 6.8% of cases--even without kinetic disturbances at rest--a haemodynamically important stenosis was found on at least one coronary artery.

Morphological changes upon subculturing of freshly isolated strains of Leptospira interrogans serovar hardjo.

The morphology of freshly isolated strains of Leptospira interrogans serovar hardjo was studied by electron microscopy and was compared to that of established laboratory strains of the same serovar and to some other serovars within the Hebdomadis serogroup. Changes in the morphology of freshly isolated strains of serovar hardjo were observed when the isolates were subcultured in the laboratory. Cells of strains subcultured more than 20 times were longer and had longer wavelengths, and electron lucent inclusions were observed less frequently than in cells of strains subcultured less than 10 times. Upon hamster passage, cells subcultured 40 and 50 times became more similar to cells of low passage number than to those of the inoculate. The results are discussed and the importance of environmental factors for the growth of leptospires is emphasized.
