Altered WAF1 genes do not play a role in abnormal cell cycle regulation in breast cancers lacking p53 mutations.

Seventy-five to 80% of breast cancers are negative for p53 gene mutations. We have investigated the possibility that altered WAF1 genes provide an alternative mode of cell cycle disruption in these tumors. DNA from a total of 85 primary breast tumors and cell lines from both the United States and Australia were examined for WAF1 and p53 mutations. With the exception of one primary tumor containing the polymorphic codon 31 (AGC-->AGA), no missense mutations in the WAF1 gene were found in 33 primary tumors or in the 19 cell lines from the United States. By contrast, 2 of 33 tumors from Australia contained tumor-specific missense mutations in the WAF1 gene, while an additional six cases contained the AGC-->AGA polymorphic 31st codon in the WAF1 gene. The p53 mutation frequency in the Australian cohort (18%) was found to be similar to that reported by us (Glebov et al., Cancer Res., 54: 3703-3709, 1994; Runnebaum et al., Proc. Natl. Acad. Sci. USA, 88: 10657-10661, 1991) in the tumors of United States patients (13%) with sporadic breast cancer. Thus, mutations in the WAF1 gene are rare in tumors with or without p53 mutations, suggesting that except in a minor population of breast cancer patients of Caucasian origin, cell cycle dysregulation by mutated p53 or WAF1 genes may not contribute to breast tumor initiation or progression.

The Stroop effect in schizophrenic patients.

Schizophrenic patients (n = 44) and normal controls (n = 50) performed a computerized version of the Stroop color-word interference task. Schizophrenic patients generally showed more Stroop interference than normal subjects. The effect was neither related to demographic variables, nor to actual psychopathology. However, the course of the disorder was related to the Stroop effect, in that acute, chronic, and schizoaffective patients displayed a larger interference effect than patients with a recurrent episode. From a methodological perspective, the computerized version of the Stroop task proved to be more sensitive to interference effects in the patient group. The finding of enhanced reverse Stroop interference in patients strongly preoccupied with colors is discussed within the framework of MacLeod and Dunbar's (MacLeod and Dunbar, 1988) theory of Stroop interference involving differential practice effects.

The analysis of limiting dilution assays conducted under non-saturating conditions.

A form of mathematical analysis is presented which permits an extended analysis of limiting dilution assays under conditions in which either accessory factors, or cells or both are limiting. This is in contrast to the traditional form of analysis which permits only a single limiting parameter. Both make use of in vitro experiments in order to determine the number of antibody-producing cell precursors: the traditional form of analysis requires that all accessory cells or their factors be present in saturating concentrations; whereas in the analytic method employed here, not only B cells, but also accessory cells (macrophages) or their factors may be limiting. Analysis under non-saturating conditions permits greater elucidation of the mechanisms of intercellular interaction.

Dopaminergic modulation of semantic network activation.

In order to examine the effect of dopamine on semantic processing, we performed a double-blind, placebo-controlled study. Healthy volunteers (n = 31) were tested in a lexical decision paradigm after ingestion of either L-dopa 100 mg with benserazide 25 mg or placebo. While direct semantic priming was influenced only marginally by L-dopa, the indirect priming effects was reduced significantly. These data support the hypothesis that dopamine increases the signal-to-noise ratio in semantic networks by reducing the spread of semantic processing, thereby leading to a focussing of activation.

Functional MR imaging of semantic information processing and learning-related effects using psychometrically controlled stimulation paradigms.

Functional magnetic resonance imaging (fMRI), in conjunction with carefully designed, psychometrically optimized stimulation procedures, was used to investigate the relation between brain activation and the processing of word associations. A semantic discrimination task of word-pair similarity was performed by normal subjects (n = 17) within a clinical 1.5-Tesla whole-body MRI system. A color similarity task of psychometrically equivalent difficulty, as indicated by behavioral data acquired online during fMRI, served as active control condition. Comparisons between tasks dramatically improved results compared to comparisons between task and resting condition. The language paradigm selectively activated left frontal and left fronto-temporal areas. Cortical activation during the semantic task decreased significantly over three runs of the same word list and was paralleled by decreased reaction times. No such changes were observed in the active control condition indicating selective learning of the language task only. When combined with psychological activation schemes and the acquisition of behavioral data, fMRI represents a powerful tool for the study of brain-behavior interaction.

[Minimally invasive treatment of stress urinary incontinence by collagen administration. Comparison between endosonography controlled and transurethral submucous collagen injection].

For many years, injectable substances have been in use for the treatment of incontinence. We examined different methods of injecting glutaraldehyde cross-linked bovine collagen injection for their effectiveness in treatment of genuine stress incontinence. In the first group of patients collagen injection was done under endo-ultrasound guidance. In the second group an endoscopic system was used. There was no major difference in patient selection for the two groups. Incontinence symptoms became less severe in only 5 of 15 (33%) patients in the endo-ultrasound group, who then had moderate stress incontinence. Endoscopic collagen treatment resulted in social continence in 15 of the 22 (68.2%) patients. We conclude that endoscopic collagen is superior to endo-ultrasound-guided collagen injection in the treatment of incontinence.

[Is radical prostatectomy a suitable model for determination of PSA half-life?].

A review of the literature relating to PSA half-life reveals great variability in absolute values and pharmacokinetic models. A critical view is needed, however, since some authors suggest that the PSA half-life has implications for diagnosis and prognosis after radical prostatectomy. The aim of our study, therefore, was to characterize the value of PSA half-life determination after radical prostatectomy. Serial serum PSA detections were performed in 16 patients with localized prostatic cancer who had undergone radical prostatectomy. Serum PSA was detected on days 0, 1, 2, 3, 6, 9, 12, 15, 18, after radical prostatectomy. In all patients elimination of PSA from serum followed a biphasic logarithmic decay pattern indicating a two-compartment model of first order elimination kinetics (t1 = 1.01 +/- 0.06 days, t2 = 3.42 +/- 0.23 days; P < 0.00001). In this two-compartment model 56.3 +/- 4.8% of the preoperative PSA serum concentration was cleared by the first compartment. To find a biological correlative for the first compartment a mathematical model was developed to approximate the effect of operative blood and plasma loss on PSA serum concentration. In this model changes of hematocrit were used to estimate blood and plasma loss. These calculations showed that 50.12 +/- 3.04% of the preoperative PSA serum concentration was excreted by operative blood loss. This value was not significantly different from the clearance rate calculated for the first compartment. It is, therefore, concluded that the determination of PSA half-life after radical prostatectomy without correction of the operation-related PSA loss is only of limited value.

Continuous in situ cold perfusion with histidine tryptophan ketoglutarate solution in nephron sparing surgery for renal tumors.

PURPOSE: In transplantation medicine the superiority of Bretschneider's histidine tryptophan ketoglutarate solution has been demonstrated in various animal investigations and in donor organs under clinical conditions. Because of the wide use of ultrasound scans and computerized tomography there has been a sharp increase in the number of small renal tumors detected. Therefore, efficient protection methods designed to conserve kidney tissues are attracting growing attention. MATERIALS AND METHODS: We report on 11 patients with space-occupying lesions involving a solitary kidney. All tumor resections were performed under continuous in situ perfusion with Bretschneider's solution. Even when dealing with extensive tumors involving the hilum, complete resection and reconstruction of the kidney in situ proved feasible. RESULTS: The mean cold ischemia time was 62.1 minutes (range 18 to 88) and mean Bretschneider's perfusion volume was 2,875 cc. The kidneys resumed excretion within an average of 16.8 minutes (range 0 to 60) and no patient required dialysis. Apart from temporary elevation of serum creatinine levels, postoperative renal function was unimpaired. There were no changes in serum electrolyte levels and no disorders of cardiac conduction. One kidney was lost postoperatively due to renal artery embolism. CONCLUSIONS: Continuous in situ perfusion with Bretschneider's solution is a new and interesting method for patients undergoing partial nephrectomy. Particularly for locally extended tumors, this procedure is highly efficient for intrarenal surgery.

[Continuous in situ cold perfusion with HTK solution in conservative surgery of kidney tumor].

Ex situ protection of donor organs for transplantation with initial cold perfusion is routinely used. The superiority of histidine-tryptophan-ketoglutarate solution (HTK) has been demonstrated in animal models and clinical use, however. On the other hand, nephron-sparing surgery for surgical or functional solitary kidneys has been an established procedure for many years. Owing to wide use of ultrasound and computerized tomography, the detection of small renal tumors has increased. New concepts in conservative renal surgery are therefore gaining in importance. In this study we report on 11 patients with renal masses in a surgical or functional solitary kidney. For the first time, all enucleations were performed with continuous in situ perfusion with HTK solution. Despite extensive tumors with central extension, complete in situ tumor resection and kidney reconstruction were possible. There were no intraoperative complications. Postoperatively one kidney was lost secondary to renal artery embolism. Urine production started within 1 h postoperatively in all other cases. No further patients needed hemodialysis. Apart from temporary elevation of serum creatinine, postoperative renal function was unimpaired. There were no changes in serum electrolytes and no disorders of cardiac conduction. The indications, the surgical procedure and the first clinical results of continuous in situ perfusion with HTK solution for conservative renal surgery are presented.

Semantic and phonological priming in schizophrenia.

This study of 70 schizophrenic patients used a lexical decision task involving the recognition of words that were preceded (primed) either by meaningfully or phonologically associated or by nonassociated words to study the intrusion of contextually inappropriate associations in thought disorder (TD). The patients were split into subgroups of TD and non-TD patients, and the data from these two groups were compared with data from 44 normal control participants. TD schizophrenic patients exhibited more semantic priming than non-TD patients and controls, and differences in phonological priming dependent on stimulus onset asynchrony (SOA) were obtained. Results support the hypotheses of an increase in activation or a decrease in inhibition in the spreading of semantic and phonological associations in TD schizophrenic patients.

[Venous tumor invasion by renal cell carcinoma. Surgical technique, complications and survival rate].

Renal cell carcinoma invades the vena cava in 4-10% of cases. Another 10% of patients present with invasion of the renal vein. The surgical approach, complications and long-term outcome of 95 patients were investigated. Intraoperative complications occurred in 1 of 73 patients with involvement of the renal vein and 5 of 22 patients with vena cava thrombus. One patient in each group died due to pulmonary emboli in the perioperative period. Minor renal insufficiency occurred in 39 (54.2%) and 11 (50%) of the patients respectively. The rates of minor complications such as wound infections, haematoma and pneumonia were similar in the two groups. The mean intra-operative blood loss of 780 ml in patients undergoing tumour nephrectomy was significantly lower than the blood loss of 1485 ml in patients with tumour thrombus extension into the vena cava. The 5-year survival rate of patients with V1 tumours (71%) is comparable to that of patients without venous involvement. Tumour extension into the vena cava (V2) influences the 5-year survival rate significantly, decreasing it to 56.7%. In conclusion, long-term survival can be achieved for patients with renal cell carcinoma and venous involvement, though for patients with lymph node metastases or distant metastases only short-term palliation can be achieved. However, the potential benefits should be carefully weighed against the possible complications, the surgical morbidity and the resources expended in vena cava resection.

[Comprehension of metaphoric speech by healthy probands and schizophrenic patients. An experimental psychopathologic contribution to concretism].

The comprehension of auditorily presented sentences with a metaphoric content was investigated in three experimental studies. Experiments 1 and 2 established the time course in control subjects: At 400 ms after prime offset only the concrete meaning was activated, whereas 1200 ms after prime offset both concrete and metaphoric meaning were active. Experiment 3 demonstrated a heightened activation of the concrete meaning and no activation of the metaphoric meaning in schizophrenic patients. These results resolve the dispute over the use of concepts by schizophrenic patients (overinclusive versus underinclusive), and can be related to the neurobiology of working memory and of associative semantic memory. The three experiments may serve as paradigms of how the method of mental chronometry can be applied in the field of psychopathology.

Associative semantic network dysfunction in thought-disordered schizophrenic patients: direct evidence from indirect semantic priming.

The characteristics of the spread of semantic activation in associative networks in normal subjects, thought-disordered (TD) and nonthought-disordered (NTD) schizophrenic patients with respect to time and semantic distance are examined. Direct and indirect semantic priming effects at two stimulus onset asynchronies (SOA) in a lexical decision task reveal that semantic associations spread further and faster in TD schizophrenic patients than in normal controls and in NTD schizophrenic patients. From a methodological point of view, indirect semantic priming at short prime-target intervals appears to be the best indicator of associative network dysfunction. The findings are discussed within the framework of current research on the effect of dopamine on the signal-to-noise ratio in cortical neural networks. Data suggest that semantic associative memory operates at a comparatively lower signal-to-noise ratio in thought-disordered schizophrenic patients.

Indirect semantic priming in schizophrenic patients.

Schizophrenic patients and normal controls performed a lexical decision task involving the recognition of words which were preceded (primed) by either associated, indirectly associated, or non-associated words. In contrast to normal control subjects who showed no indirect semantic priming effect at an inter-stimulus interval (ISI) of 0 ms, a trend towards such an indirect semantic priming effect was found in schizophrenic patients. With a longer ISI of 500 ms, an indirect semantic priming effect was obtained in both groups. In the framework of network models of semantic memory, the results are interpreted as further evidence for an increase in activation or decrease in inhibition in the spreading of associational activation in schizophrenic patients.

[Semantic activation phenomena in healthy probands and in schizophrenic patients. Analysis on the level of word pairs].

Semantic priming in normal controls and schizophrenic patients can be demonstrated in lexical decision tasks. Data from such a lexical decision task are analyzed with special respect to the question as to whether associative processes in schizophrenic patients are characterized by the occurrence of entirely new associations or by the intrusion of normal associations, i.e., associations which can be found in normal subjects. The analysis of the semantic priming effects of single word pairs demonstrates little qualitative differences between the associations of normals and of schizophrenic patients. This finding provides further support for Maher's hypothesis of activated normal associations in schizophrenic patients. From a methodological perspective, the item-by-item analysis of lexical decision task data can be used to optimize this method.

The scalp topography of P300 in the visual and auditory modalities: a comparison of three normalization methods and the control of statistical type II error.

This study was designed to replicate recent findings suggesting that the P3 component of the event-related potential is dependent on the modality of the eliciting stimulus. When assessing this research hypothesis two methodological problems are of special interest: first, the amplitudes have to be normalized, due to problems with the model of the analysis of variance; second, special care has to be taken regarding the beta error, which is the probability of falsely accepting the null hypothesis of a statistical test. A possible modality independence is associated with the acceptance of a null hypothesis. The first problem was assessed by using different normalization procedures and comparing their results. The second was solved by controlling the beta error. Results for P3 amplitudes from two sessions in which 61 subjects performed in each session an auditory and a visual oddball task (EEG measured at 11 locations) showed no influence of modality on the P3 elicited by the rare, task relevant, stimulus. Influences of modality were observed for the P3 elicited by the frequent stimulus. As it is quite unlikely that P3 generating sources are strongly active during the processing of the frequent stimulus, this effect is possibly due to a component overlap from the vertex potential.
