Schwannoma with secondary erosion of mandible: case report with a review of the literature.

Schwannoma (neurilemmoma) is a common, histologically distinctive, benign, usually encapsulated, peripheral nerve tumour of Schwann cell origin. We report a case of schwannoma arising from soft tissue near the mandible. A 53-year-old female presented at our department with painless swelling of lingual mucosa of the mandible. The first molar was extracted 20 months before. Panoramic radiograph showed a suspected residual cyst. It was impossible to determine prior to surgery whether this was a peripheral nerve sheath tumour. The lesion was completely removed; the tumour appears to have originated in soft tissue and caused secondary erosion of the mandible.

PTCH gene altered in dentigerous cysts.

Motivated by the evidence that odontogenic keratocysts are associated with genetic alterations, we examined the possibility that development of other odontogenic cysts can be attributed to gene malfunctioning, in particular to the PTCH gene. Cyst epithelium was examined for polymorphism on chromosome 9q22.3, the region that contains the PTCH gene. Loss of heterozygosity (LOH) for the D9S287 marker and/or D9S180 marker was observed in about 50% of dentigerous cysts, whereas radicular cysts gave no indication of lesions in the PTCH region. As a more direct argument for PTCH involvement in cystic growth, we report evidence of PTCH expression in dentigerous cyst lining, which indicates malfunctioning of the relevant signaling pathway. While we found no reason to believe that PTCH should be associated with radicular cysts, other genes may be implicated in their development. We performed immunohistochemical comparisons of keratocysts, dentigerous and radicular cysts for the nonmetastatic marker Nm23. A graded response placed radicular cysts in between the other two types, suggesting a similar neoplastic character for their epithelial proliferation.

Inflammatory pseudotumor of the trachea.

Primary tracheal tumors are extremely rare, and the majority of them are malignant. Inflammatory pseudotumor is a benign, tumorlike lesion, most likely of a reactive nature. Its basic morphologic characteristic is spindle cell (myoblasts and fibroblasts) proliferation with a variable number and type of inflammatory cells. A case of intratracheal inflammatory pseudotumor in a 14-year-old boy is presented together with a review of all similar lesions in the available literature. The discussion includes the presentation of tracheal tumors, their basic morphologic and immunohistochemical characteristics, and treatment modalities that are available. The surgeon must exercise caution not to perform radical surgery based on the initial pathologic diagnosis from the intraoperative frozen section, because the prognosis of these benign lesions generally is excellent. This is the second reported case of intratracheal inflammatory pseudotumor successfully endoscopically vaporized using a CO(2) laser, which is an excellent alternative in cases in which surgical treatment is feasible. Involvement of PTCH gene in various noninflammatory cysts.

Constitutional hemizygous inactivation of PTCH, the Shh signaling pathway gene that moderates the signal, manifests itself as nevoid basal cell carcinoma syndrome or Gorlin syndrome, a condition variably characterized by a number of developmental disorders and malformations, and by predisposition to some malignancies, basal cell carcinoma in particular. Loss of heterozygosity for the PTCH region was found several years ago in the epithelial lining of odontogenic keratocysts, the cyst type with highly increased incidence in nevoid basal cell carcinoma syndrome. This finding confirmed the expectations that the gene responsible for the syndrome would have a decisive role in the genesis of these cysts even when they are not syndrome related. Suggestive temporal distribution of Shh signaling, recently observed during tooth development, lead us to investigate PTCH association with dentigerous cysts, the other major noninflammatory cyst of odontogenic origin. We report here that PTCH appears to be inactivated in dentigerous cysts, suggesting that it is responsible for their genesis as well. More generally, if our similar observations of incomplete heterozygosity in this region for dermoid cysts can be interpreted as loss of heterozygosity, PTCH alterations may prove to be a necessary, and perhaps the initiating event, in formation and growth of various noninflammatory cysts. This would be consistent with our view that local PTCH inactivation can, under favorable circumstances, lead to persistent though not by itself truly aggressive cell proliferation.

Calcified leiomyoma of the lateral pterygoid muscle in an 8-year-old boy.

Deep soft tissue leiomyomas are extremely rare benign tumors in childhood. An unusual case of benign calcified leiomyoma of the pterygoid muscle in an 8-year old boy is presented. Clinical manifestations and radiographic and histologicpathologic findings, as well as the differential diagnosis and possible histogenesis of this rare tumor, are discussed.

Glandular odontogenic cyst: a case report and clinicopathologic analysis of the relationship to central mucoepidermoid carcinoma.

Glandular odontogenic cyst (GOC), an unusual multilocular cystic lesion of jaws exhibiting the features of both botryoid odontogenic cyst and mucoepidermoid tumor has been reported recently as a new entity with uncertain histogenesis. The World Health Organization named GOC as an independent pathologic entity and classified it as a developmental odontogenic epithelial cyst. We report an additional case, describe its clinicopathologic features, and discuss the similarity to the central mucoepidermoid carcinoma.

Correlation between instability of fundamental voice frequency and malignant infiltration of vocal fold nerve endings.

Performing systematically acoustical objective voice analysis in patients with chronic chorditis and glottic cancer we detected a certain number of cases with exceptional acoustical pattern. In 7 of 50 patients with chronic chorditis this acoustical pattern was highly specific demonstrating frequent changes of fundamental frequency in short time interval. In 31 of 50 patients with glottic cancer the same phenomenon was recorded. Histology revealed in 6 of those 7 patients with clinical diagnosis of chronic chorditis invasive cancer of the vocal fold. Detailed and more sophisticated techniques (PAF Halmi for elastic and collagen fibres and neurohistochemical technique S-100 and NSE for nerve elements) demonstrated mainly preserved layer structure of vocal fold lamina propria and infiltrated nerve endings. Such histological findings strongly suggest that cancer infiltration is responsible for rapid changes of fundamental frequency (via mucosal reflexes) although vibratory segments are regular because of preserved layer structure. Such findings could contribute to better understanding of laryngeal control mechanisms and laryngeal mucosal reflexes in human. Also this acoustical sign may be a useful diagnostical parameter for very early diagnosis of glottic cancer. This is possible because of the great sensitivity and complexity of phonatory function.

Histology of nasal polyps in various conditions.

At the Salata ENT Department in Zagreb, 28 patients with nasal polyps were examined in the 1992-1993 period. Results of histologic analysis of these nasal polyps are presented. In 10 cases, nasal polyps were histologically examined at three horizontal levels, and the rest by vertical section. The method of hemalaun eoisin and histochemical methods (PAS, Alcian, Mallory, Giemsa and orcein) were used to analyze the following parameters: epithelium, stroma, basement membrane, cell infiltrate and glands. Patients were grouped according to the presence or absence of allergy, relapse of polyps and age group ( < 40 and > 40 years of age). The lateral nasal wall mucosa served as control. Histologic picture of the polyps showed some differences related to allergy and other groups of polyposis. In conclusion, histologic study of polyps should not be taken as the only indicator of the etiology of polyposis but, supplemented with other examinations, as an evidence supporting the diagnosis.

Qualitative analysis of drug prescription in a geriatric population sample.

The rationality of specialist drug prescription in 154 patients older than 64 in 3 Zagreb homes for the aged was analyzed over an 11-month period. Categories analyzed were: number of prescribed drugs, justification of prescription, adequacy of the dose, route, form and therapy duration of the prescribed drugs, adequacy of the type of the prescribed drugs in the advanced age and the presence of contraindications of drugs used. The criterion of the rational drug use was the WHO's modified definition of rational drug therapy: "Application of an appropriate drug by a correct route in an adequate dose over a sufficiently long period of time". This basic criterion was further elaborated in relation to the analyzed categories. There were 2.66 (+/- 1.65) prescribed drugs per visit in which drugs were prescribed. In all analyzed categories, distinctive aberrations from the principles of rational drug prescription were found. One hundred and sixty-four (41.4%) of all drugs were unjustifiably prescribed and 103 (26.4%) were not dosed correctly. The duration of the therapy was inadequate for 60 (15.4%), route for 41 (10.5%), form for 32 (8.2%) of drugs prescribed. Seventy-five (19.3%) were not adequate because of patients' age and 15 (3.9%) were prescribed in spite of existing contraindications.

[Central odontogenic fibroma of the mandible].

Central odontogenic fibroma of the jaw is not a clearly defined lesion. It is rare, asymptomatic, usually found in the mandible and equally involving both sexes. The characteristic feature of the lesion is a high histomorphologic variability, while clinical, x-ray and histologic behavior of the tumor is not specific. All this contributes to the difficulty in diagnosis. Conservative enucleation of the benign tumor is the treatment of choice. The recurrence is rare, but, those with more epithelial elements are more prone to recur.

[Comparison of clinical and histopathological diagnosis in oral surgery].

Only a successful cooperation of an oral surgeon and the pathologist can ensure a reliable diagnosis. The authors have analyzed operations performed during 1990 at outpatient clinic of the Department of Oral and Maxillofacial Surgery, University of Zagreb, School of Dentistry. Clinical appearance of the disease and clinical diagnosis sometimes enable us to recognize the real nature of the disease. The clinical diagnosis in correlation with the histological diagnosis can be either denied or improved. The aim of this investigation was to correlate the clinical and the histological diagnosis and to find out the number of improvement. Material most often sent for histological examination was an operated cyst. Although the removed tissues were analyzed under different clinical diagnoses, approximately 70% of the clinical diagnoses corresponded to the histopathological findings. In one case the clinical diagnosis of benign lesion--hyperkeratosis-- was after histopathological examination find as carcinoma planocellulare. The authors recommend every part of the excised tissue to be sent for a histopathological examination.

Metastatic carcinomas involving the heart. Review of postmortem examination.

This article deals with metastatic involvement of the heart and is based on autopsy material of the Institute of Pathology, Medical Faculty of the University of Zagreb, from the beginning of 1983 through June 1988. The incidence of heart metastases of carcinomas originating from various sites as well as positions of the tumor within the heart were considered. Heart metastases were recorded from 39 of 477 malignant neoplasms of epithelial origin, which made an incidence of 8.2%. Pericardium and myocardium were equally involved. Pericardial involvement occurred mostly as diffuse carcinosis, whereas the myocardium was involved most frequently on the left side. Metastases from primary lung carcinoma affected the heart in 7.4% of all cases and from carcinoma of the female genital tract in 7%. Great attention is deserved by primary carcinomas of the tongue and sublingual region which involved the heart in 23.7% of all cases. No similar data were found in the available reference.

[Ameloblastic fibroma--case report and discussion about relationship of mixed odontogenic tumors].

The authors have presented the case of ameloblastic fibroma of the upper jaw in 21 years old adult. Dimensions of the tumor, which were the result of long standing growth, and the occurring at the age when odontogenesis was practically completed, have supported the concept of distinct pathologic entity, but have not been agree with the theory of interrelationship of ameloblastic fibroma, ameloblastic fibro-odontoma and odontoma. In the context of those two studies the authors have discussed relationship of the mixed odontogenic tumors.

Immunohistochemical analysis and prognostic value of cathepsin D determination in laryngeal squamous cell carcinoma.

Cathepsin D, a protease with the capability of degrading matrix proteins, is implicated in the process of breast and colorectal cancer invasion and metastasis. Biochemical studies in laryngeal cancer have shown a potential prognostic significance of cathepsin D content determination. We studied immunohistochemical positivity of cathepsin D in tumor epithelium and stroma of 61 surgical specimens of squamous cell laryngeal cancer. Immunohistochemical reaction was quantitatively assessed using a PC-based image analysis system SFORM-VAMS. The results were correlated to clinical and morphological parameters and survival. Immunohistochemical positivity was noted in neoplastic cells and tumor stroma. Significant prognostic value for cathepsin D was established separately for epithelial tumor component and tumor stroma using log-rank test, the Cox proportional hazards regression model, and C4.5 machine learning system. In all groups, patients above the median cathepsin D staining showed significantly shorter survival time. C4.5 machine learning system extracted cutoff values for the decision tree that defines the probabilities of patients survival and death with high sensitivity (92.8% alive, 73.6% dead), 100% specificity, and 86.9% accuracy. This makes immunohistochemical cathepsin D estimation an independent prognostic parameter in laryngeal carcinomas within a 5-year period from the time of tumor surgery.

Oncoproteins and tumor suppressor proteins in congenital sacrococcygeal teratomas.

Congenital sacrococcygeal teratoma (SCT) is the most common germ cell tumor of infancy and childhood with a female preponderance. Most SCTs are diagnosed at birth, are benign, and consist of fully differentiated, mature tissues. Tumorigenesis of SCTs remains poorly understood. Almost nothing is known about possible oncogene activation or tumor suppressor inactivation in these rare tumors. We describe the presence of various oncoproteins and tumor suppressor proteins in eight cases of congenital SCT. The following oncogenes were examined: ras family (c-H-, c-N-, and c-K-ras), early genes (fos, jun), and tumor suppressor genes (p53 and nm23-H-I). There was no relationship between the intensity of expression of these oncoproteins and tumor suppressor genes and the following parameters: tumor size, age, and survival of the patients. We did not observe any difference, however, between the expression of the examined oncogenes and tumor suppressor genes nm23 and p53 in immature and mature teratomas. Our findings suggest that the ras family of oncogenes, fos and jun oncogenes, and nm23 and p53 tumor suppressor genes are present in congenital SCT, indicating a possible role in genesis and development of these tumors.
