Detection and genotyping of Borrelia burgdorferi sensu lato by polymerase chain reaction.

AIM: To isolate and genotype Borrelia burgdorferi genospecies in serum samples of Croatian patients with erythema migrans. METHODS: DNA isolates from sera of patients with erythema migrans were analyzed by nested polymerase chain reaction (PCR), amplifying a segment of flagellin gene with primers encompassing the conserved region of the gene. To screen PCR products for heterogeneity, we performed single-stranded conformation polymorphism (SSCP) analysis. The samples showing differences in SSCP patterns were sequenced, and the sequence compared in the GeneBank for sequence homology with known Borrelia burgdorferi genospecies. We also constructed phylogenetic tree of all known borrelial sequences. RESULTS: The nested PCR method using specially designed flagellin gene primers, achieved the sensitivity of 10 genome copies (0.01 pg of purified Borrelia burgdorferi DNA from culture) by dilution analysis. The assay specificity was confirmed by amplification of a part of the flagellin gene from different bacterial species. The primer pairs successfully amplified only Borrelia burgdorferi flagellin gene. The genome of Borrelia burgdorferi sensu lato was detected in the sera of all 10 tested patients with erythema migrans. Sequence data and phylogenetic analysis confirmed that all amplified samples belonged to Borrelia afzelii genospecies. CONCLUSION: Phylogenetic tree analysis placed the borrelial isolates together with Borrelia afzelii sequences into a single group. This finding was additionally supported by sequence homology analysis, which produced a homology score of 99%. In patients with erythema migrans who come from the northwest Croatia, an endemic area for Lyme borreliosis, Borrelia afzelii was the cause of skin manifestations of Lyme borreliosis.

Characterization of visually similar diffuse diseases from B-scan liver images using nonseparable wavelet transform.

This paper describes a new approach for texture characterization, based on nonseparable wavelet decomposition, and its application for the discrimination of visually similar diffuse diseases of liver. The proposed feature-extraction algorithm applies nonseparable quincunx wavelet transform and uses energies of the transformed regions to characterize textures. Classification experiments on a set of three different tissue types show that the scale/frequency approach, particularly one based on the nonseparable wavelet transform, could be a reliable method for a texture characterization and analysis of B-scan liver images. Comparison between the quincunx and the traditional wavelet decomposition suggests that the quincunx transform is more appropriate for characterization of noisy data, and practical applications, requiring description with lower rotational sensitivity.

Alpha- and beta-globins of the anemic Belgrade laboratory rat. II. The effect of hemin and iron-dextran treatment.

We have studied the changes in bone marrow and spleen globin chain levels after in vivo hemin and iron-dextran treatment of hereditarily anemic Belgrade laboratory (b/b) rats. The increase of globin chains was detected in the bone marrow and in the spleen when b/b animals were treated with either iron or hemin. The analysis of changes in alpha- and beta-globin chain ratios revealed the distinctive role of these molecules in regulating globin chain status. Iron-dextran, as expected, ameliorated the imbalance of alpha- and beta-globin chains in the b/b rat spleen. On the other hand, hemin, as we have hypothesized in the accompanying paper, leads to a surplus of beta-globin chains in the bone marrow, similar to the one detected in the b/b rat spleen. Therefore, an iron-rich microenvironment has a stimulatory effect, while a hemin-rich microenvironment has an inhibitory effect on erythropoiesis.

A new approach to persistent traumatic peroneal nerve palsy.

Currently used tendon transfers for persistent traumatic paralysis of the common peroneal nerve are based on the transfer of the posterior tibial muscle, an antagonist muscle to the paralytic group of muscles. In order to achieve voluntary active dorsiflexion of the foot and automatic walking we have transposed the lateral head of the gastrocnemius to the anterior side of the lower leg, at the same time suturing the undamaged proximal end of the deep branch of the peroneal nerve to the motor branch of the tibial nerve innervating the lateral head of gastrocnemius muscle. After nerve regeneration and neurotisation the transposed lateral head of gastrocnemius was innervated by the deep branch of the peroneal nerve and thus it took over the function of the paralytic muscles. The indications for the operation, the surgical technique, and the results obtained in 6 cases are presented.

Evaluation of left ventricular diastolic function by radionuclide ventriculography in patients with coronary artery disease.

Radionuclide ventriculography (RNV) was used to assess the peak filling rate (PFR) in 20 patients with coronary artery disease (CAD). Dipyridamole RNV was used in 5 normal subjects and 10 patients with CAD with the view to evaluating PFR increase. Significant difference in PFR values was established between normal subjects and CAD patients. Decreased value of PFR was found in 78% CAD patients, and 72% of them had normal ejection fraction (EF). During dipyridamole RNV increase of PFR was not significantly different between normals and CAD patients. The peak filling rate is a very sensitive indicator of LV dysfunction in CAD patients, and is useful in early detection of abnormalities in diastolic function in CAD patients.

Arteriovenous fistulae after free flap surgery in a replanted hand.

AV fistulae are extremely rare complications after hand replantation. In the case presented, the formation of an AV fistula did not occur immediately after the replantation, but after the insertion of the free lateral arm flap to the extensor surface of the replanted hand. This paper discusses the mechanisms responsible for the formation of AV fistulae.

Evaluation of diagnostic reliability of radionuclide phlebography using 99mTc-MAA to detect deep venous thrombosis; its role in establishing indications for inferior vena cava filter implantation.

Radionuclide phlebography (RNP) of the lower extremities and pelvis was performed using 99mTc-MAA in 40 patients with pulmonary thromboembolism. Deep venous thrombosis (DVT) was found in all patients, more frequently in the right calf and in the left iliac veins in 20 patients (55%). RNP and contrast phlebography (CP) were performed in 18 patients and confirmed the presence of DVT in all cases. The greatest specificity of RNP was obtained in the left (92.4%) and in the right pelvis (80%). In 12 of 18 patients in whom a cava filter was implanted, specificity of RNP was 100% for the left thigh and 91.7% for other localizations. In establishing indications for cava filter implantation, RNP should be performed prior to CP and bilaterally, but in case of non-indicated CP, RNP findings should be sufficient.

Genetic markers in the blood of multiple sclerosis patients.

Poligenetically determined predisposition to multiple sclerosis (MS) defines the way of immunological reaction to environmental factors and leads to clinically manifest disease. Although the connection between MS and some loci of the HLA system has been established, the hereditary predisposition to MS remains to be elucidated. We determined the phenotypes of monogenic hereditary characteristics linked to the surface of red blood cells that were obtained from 45 MS patients and 458 healthy subjects. The antigens on the erythrocytic surface of the ABO, Rh, MN, Ss, Kell, Kidd, Duffy, P and Lewis system were analyzed. Our results demonstrate that the MS patients differ from the normal subjects with regard to the Rh, ABO and Lewis erythrocytic antigens. The Rh positive factor was present in 95.55% of the MS patients compared to 84.29% of the controls, whereas the Rh negative factor was found in only 4.45% of the MS patients and 15.71% of the healthy subjects. The blood group O was demonstrated in 22.22% of the MS patients compared to 40.42% of the healthy persons. The MS patients had the blood group A in 15.11% of the cases as opposed to 43.99% of the subjects in the control groups. The blood group B was found in 22.22% of the MS patients compared to 11.10% of the controls. The distribution of the Lewis system in the MS patients was also demonstrated to be different from that in the general population. The MS patients were found to have less frequently the Le a+b- phenotype (11.12%) and more frequently Le a- b- (13.33%) compared to the healthy subjects who had the Le a+b- phenotype in 12.00% of the cases and Le a- b- in 5.00%.

[Biopsy of the chorionic villi with the aid of ultrasound in the diagnosis of chromosome disorders in the 1st trimester].

Chorion frondosum biopsy is a method of antenatal diagnosis based on the chorion villi cell analysis, with the aim of detecting hereditary disorders of chromosomal and genetic origin. The method was applied at the Department of Gynaecology and Obstetrics in women with undesired pregnancy from 8 to 12 weeks. This was the first phase of our investigation. Chorion frondosum samples were taken by Jackson's forceps and "Portex" cannula for transcervical aspiration. The role of ultrasound, as a component part of this method, is pointed out in the paper. Ultrasound was applied in a way not to damage the embryo and its membranes, and to locate chorion frondosum--the site of taking the appropriate chorionic villi. The success of detection of foetal chromosomal constitution in our study was 84%. Chromosomopathy was registered in on case. The foetus had tetraploidy--92, XXYY.

Correlative tissue pathology of intracranial neurinomas with computed tomography scans.

On computed tomography (CT) layers through the tumor the quality of picture depends upon the tumoral tissue structure, i.e., upon its histological appearance. In the formation of this histological appearance to a greater or lesser extent a series of various elements are involved, which are responsible for the density on CT scan, e.g., blood vessels, fatty tissue, calcium, connective tissue, collagen, etc. For some tumors the most important characteristic is specific organization of tissues in the volume, which can be clearly seen on high-quality CT scan. Possible limitations are due to technical capabilities of a CT apparatus. In our study we have obtained the most valuable results by analyzing the quality of picture of neurinomas on the so-called native CT scans, and by evaluating the way in which the picture of the tumoral tissue changed by opacification after an intravenous injection of contrast medium. Our study included 23 intracranial neurinomas. Twenty of them were neurinomas of the statoacoustic nerve, 1 of the orbit, 1 of the Gasser's gaglion and 1 was in the parasagittal region. For each particular tumor on histological specimens Antoni A and Antoni B types of tissue were semiquantitatively identified. In our study we have chosen only those cases for which we had a plenty of tumoral tissue suitable for semiserial analysis. The results of the histological analysis were compared with the results of the CT assessment of the tumoral tissue. Larger necrotic areas and cystic formations within tumoral tissue, which we could not analyze on our histological specimens, were excluded from the evaluation of CT scans of these tumors. Our results indicate a positive correlation between the histological appearance of neurinomas and their CT scans. This correlation between the histological properties of the tumoral tissue and the CT scan was almost 100% in those cases in which one histological type of neurinomas significantly prevailed. The prevalence of one type of neurinoma tissue was found in 74% of cases. In almost 61% of neurinomas in the histological appearance Antoni A type of tissue prevailed, what was clearly seen on CT in the form of hyperdense areas of the tumor. Antoni B type of tissue was histologically found in about 13% of neurinomas. These tumors had a marked hypodense picture of the tumoral tissue. The rest of neurinomas had mixed types of tissue. In this group of tumors we could not use the CT findings as an indicator in preoperative analysis of histopathological tissue characteristics.

[Polytrauma with retroperitoneal injuries].

From 1971-1987, 598 polytraumatized patients had concomitant injuries of the retroperitoneal space. The mortality rate came up to 33.4%. Diagnostically abdominal lavage, laparoscopy, selective angiography and CT were carried out. Often life saving laparotomy had to be practised without further delay.

[Diaphragmatic rupture in present-day injuries].

In this report the authors took all material found on KBC Zemun for period from 1968 to 1988 on 43 causes traumatic rupture of diaphragms treated on this clinic. On their own experience and taking the experience of the others the authors found that: Traumatic rupture of diaphragm is a consequence of thoracoabdominal compressive trauma, which it accompanies in 3% of cases. Any traumatic rupture of diaphragm is an acute surgical state which requires an urgent surgical treatment. No other treatment is justified. Radiological method is applied in diagnosing the traumatic rupture of diaphragm. The surgical operation ought to secure a good detection of accompanying injuries. They are surgically taken care of by breaking the communication between the two cavities and, by providing their proper drainage. The major factor causing this type of traumatism presents the traffic accidents. The death rate with these traumas is very high and largely depending on accompanying injuries, reaching 20.9% in the material that we studied.

Genetic factors and fetal growth sex constitution and birthweight in twins.

Fetal growth is considered a multifactorially-influenced characteristic. Genetic factors were found to play the main role in fetal growth determination, and it is the polygenic inheritance. The paper reviews the birthweights of 360 unlike-sexed two-egg twins, born at the Clinic of Gynecology and Obstetrics, Faculty of Medicine in Beograd, from January 1, 1970 through June 30, 1984. The mean birthweight differences between the male (2954.94 +/- 704.28 g) and female (2313.19 +/- 651.32 g) fetuses, amounted to 141.75 g. The difference is highly significant (P less than 0.01); variation coefficient shows that the examined groups were homogenous (VC was below 30%). The results were discussed from the genetic point of view, and particularly from the point of view of sex constitution, with special respect to the effect of Y chromosome on fetal growth. It has been known that human somatic development, body weight, bone maturation and teeth development are influenced by variety of factors. In addition to nutritional and other factors of the external environment, genetic and humoral factors are leading. The fact that fetuses with XY male constitution are heavier at birth than female fetuses with XX constitution, made us consider the correlation between genotype-phenotype, and the mentioned characteristics. This study was aimed at finding out the role of genetic factors in fetal growth, with respect to sex constitution, particularly to chromosome effect on fetal growth in unlike-sexed two-egg twins.

Maternal translocation t(13:18)(q34:q11) and Edward's syndrome in a fetus: 47,xy,t(13:18)(q34:q11) + 18.

A 32-year-old woman, who presented with four spontaneous abortions, was found to have a balanced translocation: 46,XX,t(13:18)(q34:q11). In the last pregnancy an amniocentesis was done. Abnormal constitution of the fetus had been detected: 47,XY,t(13:18)(q34:q11) + 18, and an abortion was induced. Examination of the fetal tissue confirmed the finding. The fetus showed the characteristics of Edward's syndrome. Through the patient's pedigree it was discovered that balanced translocation appeared in three generations.
