[Results of abdominal x-ray computed tomography in 25 patients with the acquired immunodeficiency syndrome or the lymphadenopathy syndrome].

An abdominal computed tomographic examination was performed to 20 patients with Acquired Immunodeficiency Syndrome (AIDS) and to 5 patients with Lymphadenopathy Syndrome (LAS). Intraabdominal lymph nodes were seen in 18 out of 20 cases of AIDS and in 5 cases of LAS. Lymph nodes have a normal size or are slightly enlarged but they are too numerous. Splenomegaly was found in 17 patients. Rectal modifications secondary to a proctitis were seen in the homosexual patients.

Serum immunoglobulin G subclass dysbalances in the lymphadenopathy syndrome and acquired immune deficiency syndrome.

Serum IgG subclass levels were measured by an indirect competitive immunoenzymatic assay with monoclonal antibodies in 61 adult patients of different geographic origins affected either with the lymphoadenopathy syndrome (LAS, 46 cases) or with the acquired immune deficiency syndrome (AIDS, 15 cases). In spite of considerable variations from patient to patient, IgG-1 and IgG-3 levels were higher than in normal Caucasians, with IgG-1 levels much more elevated in LAS than in AIDS patients. In Caucasians with AIDS or LAS, IgG-4 levels were low. IgG-2 levels tended to be low and correlated negatively with IgG-1 levels. IgG subclass imbalances were especially striking in patients with lymphoid interstitial pneumonitis. The results suggest that the IgG increase predominantly affects the less T-dependent subclasses. The low levels of the more T-dependent isotypes do not appear to play a clear role in the occurrence of pyogenic infections in certain patients.

[Treatment with halofantrine of Plasmodium falciparum malaria imported into France].

Halofantrine is a 9-phenanthrene-methanol effective against multiresistant strains of Plasmodium falciparum. It is extremely effective and well-tolerated in treating cases of malaria imported into France. 1,500 mg in 3 doses at 8 hour intervals produced 100% cure rate in semi-immune patients. This dosage could be repeated after 14 days in order to obtain the same cure rate in non-immune patients. Side-effects are minor and include epigastric pain, nausea and one case of skin rash.

[Digestive manifestations of the acquired immunodeficiency syndrome (AIDS): study in 26 patients].

We studied the gastrointestinal manifestations in 26 cases of AIDS. The patients belonged to two different epidemiological groups: the first group included thirteen french homosexual men, the second group included 6 Haitians, 6 Africans and a Pakistanian, none of them admit homosexual activity. The clinical manifestations were: chronic watery diarrhea in 17 cases, bloody diarrhea in 2 cases; loss of weight in the 26 cases; dysphagia in five cases; jaundice in one patient (due to Kaposi sarcoma of the ampulla of Vater). The digestive lesions found, alone or associated, were necrotizing enteritis (2), ulcerative colitis (1), pseudomembranous colitis (1), Candida esophagitis (10), erythematous duodenitis (6), proctitis (4), Kaposi sarcoma (3), diffuse (2) or localized (1). Thirteen patients out of the 26 presented opportunistic digestive infections due to one or several germs. These were 10 cases of esophageal infection (due to Candida albicans) and 8 cases of enterocolonic infection due to Cytomegalovirus (3 cases), Cryptosporidium (3 cases), Mycobacterium avium intracellulare (1 case), Cryptococcus neoformans (1 case). The other digestive infections cases were due to non-opportunistic pathogens: Entamoeba histolytica (3 cases); Giardia lamblia (3 cases); Strongyloides stercoralis (2 cases); Salmonella typhi (2 cases); Shigella (1 case); Herpes simplex virus (1 case). No difference was noticed between the homosexual and the heterosexual groups with respect to the nature and the frequency of the digestive infections.

[Fertility in Gabon (Haut Obooué). Retrospective study in Okondja and Franceville. Obstetrical activity at the maternity hospital in Franceville during the first trimester of 1981].

This is a report of 2 studies led in Gabon (Haut Ogooué) in 1981: investigation of retrospective fecondity in 2 groups of women (in Franceville and Okondja). The mean number of pregnancies per women (3.8 and 4.3) and the mean descent are close to official reports. This investigation reveals that there is a high neonatal mortality rate, which is likely to have an effect on hypofecondity among women of this area, even more for the older ones; activity of the "Maternité Provinciale de Franceville" during the first semester 1981.

[Cost of transfusion in AIDS. Experience at the Claude-Bernard Hospital apropos of 28 cases].

The study of 28 confirmed cases of AIDS shows that transfused patients use a large amount of hospital's blood products, especially platelets. For these patients, the average cost of transfusion is higher than that, for intensive care unit patient's. The use of irradiated products increases the cost (+ 35%). As there is a shortage of donors, if the present precautions are respected, there is no reason why the hospital should not continue to ask the patient's family and circle of friends, to donate blood.

[Cryptosporidiosis. II. Biological diagnosis].

The biological diagnosis of human cryptosporidiosis is made primarily by identifying Cryptosporidium oocysts in stool specimens under the microscope. It is advisable to fix and stain the fecal smears by the technique according to Henriksen. It is possible to examine fresh specimens and to carry out concentration techniques but this requires excellent knowledge of the cytology of the parasite. Cryptosporidium can also be identified from intestinal biopsies.

[Cryptosporidiosis: apropos of 11 cases].

Fecal samples of 61 patients were investigated for Cryptosporidium: it was identified in four cases of acquired immunodeficiency syndrome, five cases of lymphadenopathic syndrome, one case of immunosuppressive therapy. One person was immunocompetent but in close contact with one of the patient. The authors report clinical and epidemiological characteristics of the 11 patients.

[Induced expectoration: its role in the diagnosis of pneumocystis carinii pneumonia].

OBJECTIVES: The purpose of this retrospective study was to describe the changes and the effect of Pneumocystis carinii pneumonia (PCP) prophylaxis on induced sputum sensitivity during these five last years METHODS: An induced sputum examination was performed in 80 cases of PCP over a 5-year period. RESULTS: The induced sputum were positive in 30 cases (sensitivity = 37.5%). This sensitivity changed very little during these five years. Thirty of these 80 patients (37.5%) received PCP prophylaxis at least for four weeks before PCP diagnosis. Induced sputum sensitivity was 50% in the group with prophylaxis versus 30% in the group without prophylaxis (p = 0.073). CONCLUSION: In our institution, the induced sputum examination remains an interesting diagnostic procedure for PCP. PCP prophylaxis does not seem to have effects on induced sputum sensitivity.
