Unstimulated salivary flow rates of young children.

OBJECTIVE: Studies on salivary flow rates in human beings have mainly been carried out with adults. The purpose of this study was to determine the unstimulated salivary flow rates of children 4 to 7 years old. In addition, the relative contributions of the variables age, gender, race, height, body weight, dentition status, use of prescription medication, and health status (information obtained from parents) to the unstimulated salivary flow rates of children were also studied. STUDY DESIGN: Data were obtained from children (n = 447) at 2 sites in the United States (site 1, southeast Michigan; site 2, northern Michigan) and at 5 sites in Brazil (site 3, Porto Alegre; site 4, São Paulo; site 5, Belém; and sites 6 and 7, sites in Rio de Janeiro). In northern Michigan (site 2) the participants were cognitively or developmentally disabled, or both. In Rio de Janeiro (site 7), a group of 8- to 12-year-olds served as a control group. Saliva samples were collected for 3 minutes between 9 AM and noon in the spring or summer, and the saliva rate was determined gravimetrically. Data were analyzed by analysis of variance, bivariate analysis, and regression analysis. RESULTS: The secretion rates at the 7 sites were (in milliliters per minute) 0.19 +/- 0.15, 0.23 +/- 0.28, 0.34 +/- 0.23, 0.48 +/- 0.37, 0.25 +/- 0.27, 0.37 +/- 0.28, and 0.61 +/- 0.34, respectively. There were significant differences among sites (P <.0001). The older group (site 7) had flow rates that were significantly higher than the flow rates of any other group. In addition, children from Michigan (sites 1 and 2) had significantly lower rates than most groups of children in Brazil. Girls had lower unstimulated salivary flow rates than boys did at all the sites, but the differences were not statistically significant. Race was shown not to affect the flow rates. The use of any prescription medication by children in the previous 3 months was associated with lower salivary flow rates than were found in children not using prescription medication. Children who were in good health and who had no previous medical conditions had higher flow rates--but not significantly so. Higher flow rates occurred in children with mixed dentition than in children with primary dentition, although again the differences were not statistically significant. Regression analysis revealed weight to be of significance in explaining the variability of the unstimulated salivary flow rates at 2 sites, height at 1 site, the use of prescription medication at 2 sites, and age at 1 site. CONCLUSIONS: The unstimulated salivary flow rates in children in the northern United States are comparable with those reported for Japanese children, whereas the flow rates of children in Brazil are comparable with those reported for North American and European adults. In addition, none of the demographic variables/parameters tested contributed consistently to the variability of the unstimulated salivary flow rates in children at the 7 sites assessed in this study.

Bone induction by implants coated with cultured osteogenic bone marrow cells.

The availability of osteoinductive coatings on dental and orthopedic implants will result in an improved fixation of these devices. Those cases where implants are placed in poor-quality bone or where high failure rates are obtained are especially expected to gain from such coatings. This paper presents a novel, biological approach to obtain bioactive and osteoinductive coatings on bone-replacement implant materials. This so-called tissue engineering approach utilizes osteogenic bone marrow cells that are cultured on an implant material to form a bone-like tissue. The implant materials used herein included porous calcium phosphate scaffolds and metallic plates, the latter of which were coated with a biomimetic calcium phosphate coating to facilitate cellular attachment. Bone marrow cells were obtained from a variety of species, including humans, and were grown to facilitate cellular proliferation. The cells were subsequently seeded onto the implants and cultured for an additional week to facilitate osteogenic differentiation and extracellular matrix production. The resulting hybrid implants, encompassing the biomaterial carrier and cultured bone-like tissue, were subsequently implanted subcutaneously in nude mice for 4 weeks, followed by histological examination for de novo bone formation. The results revealed that newly formed bone was seen both in porous implants and on flat metallic surfaces. This bone tissue engineering approach, therefore, offers great potential to enhance bony healing around implants in a compromised bone bed.

["Off pump" coronary surgery and simultaneous carotid endarterectomy].

The occurrence of significant carotid artery disease in patients requiring coronary artery bypass grafting (CABG), results in a dilemma regarding the best surgical management. Our philosophy has been to perform simultaneous carotid endarterectomy and CABG off-pump. We reviewed the efficacy of this therapy in six patients treated in our hospital between January 2001 and December 2002. The result was satisfactory and there was no hospital mortality.

[Video-assisted thoracic surgery in the diagnosis of interstitial diffuse pulmonary diseases].

Interstitial lung diseases are best diagnosed by surgical lung biopsy. Videoassisted thoracoscopy (VATS) is less aggressive than open lung thoracotomy and its development is associated with an increased number of lung biopsies. Between July 1994 and July 2004 we performed 70 VATS biopsies for interstitial lung disease patients. Only one biopsy was considered inconclusive (1.4%). The remainder 69 (98.6%) established a final diagnosis. The results in our series show that VATS lung biopsy is a safe method with low mortality and morbidity and an excellent rentability.

[Pulmonary carcinoid tumors--ten years experience].

Pulmonary carcinoid tumors are rare, accounting for as many as 2% of all pulmonary neoplasms and for 10% of carcinoid tumors overall. Previously classified as bronchial adenomas, actually are classified as neuroendocrine tumors. They have a subclassification into typical classed as low-grade malignant neoplasm and atypical more aggressive, with more potential to cause local invasion. In this paper, the authors report a retrospective study of 25 patients, who had the diagnosis of pulmonary carcinoid tumors and had been operated between January of 1994 and August of 2004. We conclude that this tumors must be considered malignant in the surgical approach.

[Surgery of mediastinal tumors: 11-years experience].

Between 1993 and 2004, 70 operations on patients with the diagnosis of mediastinal tumors were performed in the Cardiothoracic Surgery Department of Santa Maria Hospital. In this study we did not include diagnostic surgery of mediastinal tumors. In 70 patients treated, 63 p (90%) had the tumor located in the anterior mediastinum, 21 p with thymoma, 6 p with lymphoma, 4 p with carcinoid tumors, 1 p with sarcoma and 1 p with adenocarcinoma. 3 patients had tumors located in the medium and 4 patients in the posterior mediastinum. We analysed the results of the mediastinal tumors surgery during the 11-years period. Also, we evaluated the clinical effects of the thymectomy in myasthenia gravis.

[Surgical diagnosis of mediastinal pathology: eleven-years experience].

Between January 1993 and December 2004, 68 patients with mediastinal pathology were submitted to diagnosis surgery. All the pathological process were located in the anterior mediastinum, and most of them were malignant (64.1%). The most common pathology was limphoma (26.5%), sarcoidosis (23.5%) and thymomas (23.5%). Seven cases consisted in ganglionar tuberculosis (10.3%), six cases were ganglionar metastases of lung adenocarcinoma (8.8%), and three cases were thymic carcinoma (4.4%). One patient had no definite diagnosis (1.5%). In this experience high diagnosis accuracy (98.5%), and low morbidity were demonstrated. Although the present video assisted thoracic surgery progress and development, mediastinoscopy still remains as a reliable, safe and efficient surgical approach for the diagnosis of mediastinal pathology.

[Breast cancer in the pregnancy-puerperal cycle].

From 1978 to 1988, 611 cases of breast cancer were seen by the Mastology Department of the Beneficência Portuguesa Hospital in São Paulo, Brazil. Thirteen (2.12%) cases were associated to pregnancy, and nine (2.4%) became pregnant after treatment. The patients were submitted to surgery and the authors waited till the end of pregnancy to perform the Rt and/or Qt. Two cases could not be submitted to any kind of treatment. Whenever tests indicated fetal maturity, the option was to perform a caesarean section, and oophorectomy was performed in the most advanced cases. Among patients who became pregnant after treatment, initial cases were predominant and their pregnancies went to term with no problem. The authors compare their results to data found in literature and draw attention to anticonception in mastectomized patients.

Lesions resulting from attempted Shwartzman reaction in turkey poults inoculated with Pasteurella multocida lipopolysaccharide.

Five-week-old turkey poults were given two consecutive intravenous injections (24 hours apart) of highly purified Pasteurella multocida lipopolysaccharide (LPS) in an effort to induce a generalized Shwartzman reaction. There were no gross lesions, and microscopic lesions were limited to focal hepatic necrosis with heterophil infiltration. Hepatic lesions did not differ qualitatively from lesions in turkeys given a single dose of lipopolysaccharide. Margination of heterophils in the pulmonary vasculature was observed in turkeys 4 hours after a single injection of LPS, but it was not present in turkeys given the consecutive injections of LPS. To induce a dermal Shwartzman reaction, turkeys were given intradermal injections of LPS followed by an intravenous injection of LPS 24 hours later. Although no grossly visible hemorrhagic dermal necrosis occurred, microscopic lesions, including heterophil infiltration, vasculitis, thrombosis, and necrosis, were present. Thrombosis and vasculitis were observed only in turkeys given the intravenous and intradermal LPS, whereas the other inflammatory changes were observed in turkeys given the intradermal injection of LPS and intravenous water. Prominent lymphocytic perivascular cuffing at the site of dermal injection was present in all turkeys given intradermal LPS.
