[Clarification of the ultrasound features of benign kidney tumors. Apropos of 10 cases].

Benign tumours of the kidney are rare conditions; their pre-operative diagnosis is of great importance, as in many cases it avoids the need for a total or even extended nephrectomy. The authors report 10 cases of benign renal tumours, which were explored by ultrasound and confirmed: an adenoma, a haemangiopericytoma, 4 angiomyolipomas, 2 oncocytomas and 2 multilocular cysts. The ultrasound appearance of these tumours does not seem to be specific. The angiomyolipoma only had a typical appearance in two cases, while the other two cases had an appearance no different from an adenocarcinoma or from another benign tumour.

[Peroperative ultrasound echotomography of the kidney].

The authors report their experience of seeking calyceal lithiasis and venous neoplastic spread in carcinomas of the kidney by peroperative echotomography. The reliability of the method was excellent since it led to exact localisation of residual stones in 10 cases out of 11 with a definition of the order of a millimetre. A simple and effective method, echotomography should be of great value in lithiasis surgery. Ultrasound is also of value in surgery for carcinoma of the kidney, both for verification of the state of venous flow as well as investigation of the quality of the remaining parenchyma following conservative surgery.

[Real-time ultrasonography. Application to the intra-operative location of renal calculi].

Real-time ultrasonography was used intra-operatively in 12 cases of renal lithiasis, in association with radiography. The 10 MHz instrument with sectorial probe is small and easy to sterilise. Calculi larger than 2 mm (including radiotransparent ones) were detected by this method in 10 patients. One of the two failures was due to the surgical incision hindering the positioning of the probe, and the other to the presence of air after pyelotomy of a dilated renal pelvis in front of a horseshoe kidney. Ultrasonography appears to be a sensitive method allowing tridimensional location of small calculi, even when they are transparent to X-rays. The site and size of nephrolithotomy can be selected according to the thickness of renal tissue and to the presence of arteries, also visible on ultrasounds. The operating time is shortened and the risk of residual lithiasis is reduced. Owing to the possibility of artefacts, such as air bubbles, suture material and clots, contact radiography should be combined with ultrasonography, at least for roughly locating the stones.

[Diagnostic contribution of ultrasonography in choledochal cysts (author's transl)].

With regard to observations of 6 children with choledochal cysts, the importance of the anatomic data obtained by ultrasonography is emphasized. In the most common types of cystic dilatation of the common bile duct, the precise details concerning the size and place of the extra-hepatic biliary abnormality make ultrasonography the best pre-operative investigation.

Unusual fat-containing tumors of the kidney: a diagnostic dilemma.

Unusual fat-containing renal tumors in a series of 27 cases comprised five categories: atypical and complicated angiomyolipomas (AMLs) (n = 15), including AMLs with extrarenal growth (n = 5), AMLs with undetectable fat (n = 4), and hemorrhagic AMLs (n = 6); fat-containing renal cell carcinomas (RCCs) (n = 9); lipoma (n = 1); liposarcoma (n = 1); and fat-containing renal oncocytoma (n = 1). Fat was present within RCCs by the following mechanisms; lipid-producing necrosis within a large RCC (n = 2), intratumoral bone metaplasia with fatty marrow elements and calcification within a small RCC (n = 2), and entrapment of perirenal (n = 4) or sinus (n = 1) fat by large irregular RCCs. Fat-containing RCC must be considered in cases of fat-containing renal tumors, even though the presence of intratumoral fat is characteristic of AML. A dedicated computed tomography scanning protocol and strict diagnostic criteria are mandatory for accurate diagnosis. Malignancy should be suspected on the basis of the following criteria: presence of intratumoral calcifications; large, irregular tumor invading the perirenal or sinus fat; large necrotic tumor with small foci of fat; and association with nonfatty lymph nodes or venous invasion.
