Studies on the mode of action of clomiphene citrate.

In order to further investigate the mode of action of clomiphene citrate, 5 oophorectomized women in the age range 28 to 41 years were followed with weekly determinations of serum FSH and LH for 20 weeks after the operation. They received no treatment during the first 4 weeks but during the next 16 weeks they were given estradiol 2 mg X 2 daily orally, and from the 12th to the 16th week in addition clomiphene citrate 50 mgX2 daily. The high level of FSH after the oophorectomy decreased gradually during the treatment with estradiol alone, but this effect was partly neutralized by clomiphene citrate, as we observed a significant increase in FSH within 1 week of initiating treatment with clomiphene citrate. On the other hand, we did not find any significant changes in FSH within 4 weeks after treatment with clomiphene citrate was discontinued. The high level of LH after oophorectomy did not show any significant changes during the whole treatment period. It is concluded that clomiphene citrate primarily stimulated FSH secretion, most probably by a competition with estradiol for the receptor sites in the hypothalamus or the pituitary. In addition, it is concluded that clomiphene citrate has a prolonged effect, probably because of a tight binding to the receptor sites.

LH and FSH in serum after intramuscular administration of LH/FSH-releasing hormone in normal and hypogonadal men.

The increase in serum concentrations of LH and FSH after intramuscular administration of LH/FSH-RH was investigated in normal and hypogonadotrophic hypogonadal males. LH and FSH were measured by specific radio-immunoassays. To evaluate the appropriate dose of LH/FSH-RH for intramuscular administration different doses were administered and with 200 mu g an increase in LH and FSH was found comparable to the results obtained by other investigators using intravenous administration. After the initial studies a simple test including two determinations before LH/FSH-RH administration and one 60 min and 120 min after administration was developed and used in all cases. In 12 normal males a significant increase in the serum concentration of both LH and FSH was found averaging 381% and 148% respectively 60 min after 200 mu g LH/FSH-RH with a slight decrease in LH but not in FSH after 120 min. A positive response similar to that in normal men was observed in 11 hypogonadotrophic hypogonadal patients presenting different clinical pictures. This indicates that in all these patients the hypogonadotrophic state was due to a hypothalamic defect and not to a hypophyseal disorder.

Histological and meiotic evaluation of infertile men.

A meiotic analysis in a group of 90 infertile men was carried out and also histological examination. In one third of patients there were no cells in divisions and Sertoli cells were only seen in most of them. In 30% of patients the process of spermatogenesis stopped at pachytene or between the first and second meiotic division. In 38% of patients all stages of spermatogenesis and of meiosis were present. Chromosomal translocations were detected in three patients in diakinesis. The testicular biopsy is an important tool in diagnosis and management of male infertility. The histological and meiotic investigations of the testicular tissue specimen give us the information obtainable in no other way. These both investigations are essential for prognosis and treatment of infertile men.

Pentoxifyllin treatment of oligoasthenospermic men.

A randomised group of 90 men with idiopathic oligoasthenozoospermia was subjected to control study with pentoxifyllin. Pentoxifyllin was given to 51 men 1200 mg daily for 3 months. The other group of 39 infertile men was followed during the same period of time. Significant improvement was observed, in treated group, in percentage of motile sperms and normal shaped sperms after 3 months treatment. This study emphasizes that pentoxifyllin is a relatively good drug for restoring sperm motility and morphology in oligoasthenozoospermic men.

The effect of the gametogenesis on serum FSH, LH and prolactin levels in infertile men.

In a group of 270 infertile men serum FSH, LH and prolactin levels were estimated and testicular biopsy was done on all subjects. This investigation showed highly significant influence of histologic appearance on serum FSH and LH but not on serum prolactin levels. Significant elevation of serum FSH level was first detected in men with spermatogenic arrest at the level of the primary spermatocyte, but the greatest number was in men without any germ cell in the seminiferous tubules. Serum LH level depended very much on the presence or absence of germ cells in the seminiferous tubules and were significantly elevated only in cases with seminiferous tubules deprived of the germ cells. According to this study serum FSH and LH levels were found to be good discriminator of testicular histologies.
