Magnetic resonance imaging of a malignant transformation of an intracranial cellular blue nevus. A case report.

As a follow-up to a case previously reported, a rare case of malignant transformation of cellular blue nevus (CBN) in the central nervous system preoperatively diagnosed by magnetic resonance imaging (MRI) is reported. On MRI, the malignant portion of the nevus was slightly hyperintense on both T1- and T2-weighted images. In contrast, the benign portion with a great deal of melanin was hyperintense on T1-weighted image and hypointense on T2-weighted image. MRI was useful and indispensable for detecting the malignant transformation of CBN.

Glucocorticoids promote localized activity of rat hippocampal CA1 pyramidal neurons in brain slices.

The effects of glucocorticoids on rat hippocampal CA1 pyramidal neurons were studied using brain slice preparations. At 10 days after bilateral adrenalectomy, a localized region of CA1 showed a drastic reduction of excitability induced by CA3 stimulation as compared to control. The region of CA1 most effected was 1.4-2.0 mm from the most rostral side of the hippocampus. Upon perfusion of corticosterone, the response to synaptic activation was reduced in this region in slices from adrenalatomized animals increased rapidly toward control values, volatile responses in other regions were unaffected. These results suggest that glucocorticoid receptors are concentrated in restricted regions of hippocampus and that these receptors have important roles in regulation of synaptic excitability.

Appearance of NMDA receptors triggered by anoxia independent of voltage in vivo and in vitro.

Using rat hippocampus we have studied the pattern of neuronal death, abnormal discharge and loss of electrical excitability in slices prepared from animals subjected to bilateral, four-vessel cerebral anoxia and in slices prepared from normal animals that are subjected to anoxia in the recording chamber. As others have reported, pyramidal neurons in area CA1 are lost first after anoxia, while CA3 neurons have an intermediate sensitivity, and those in dentate are relatively anoxia-resistant. After anoxic damage to the intact animal, neurons in both CA1 and CA3 show abnormal bursting discharges in response to synaptic activation for several days, and then the response in CA1 decreases in amplitude and finally the area become unexcitable. While antagonists for N-methyl-D-aspartate (NMDA) receptors have essentially no effect on synaptic responses in control animals, they reduce the bursting responses and greatly depress the small responses in CA1 as neurons are becoming unexcitable after anoxia. With intracellular recording CA1 neurons from animals made transiently anoxic, in contrast to controls, show prolonged synaptic responses, the later components of which are blocked by NMDA antagonists. When slices from normal animals are subjected to anoxia such that excitability is totally lost over a period of about 10 min, there is no significant membrane depolarization during the anoxic episode and recovery of excitability occurs with reoxygenation. However, a period of hyperexcitability and bursting follows and electrical excitability is lost in CA1 but not CA3 neurons after about 90 min.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical and experimental studies on pancreatic functions and gastrointestinal hormones after major pancreatectomy].

The present study was undertaken to evaluate the changes of gastrointestinal hormones and pancreatic functions after major pancreatectomy in dogs and human. After more than 92% pancreatectomy in dogs, all dogs developed diabetes mellitus (DM). In these dogs, the remnant pancreas showed poor regeneration rate of 24.0% at 6 weeks, decreased sigma IRI in IV-GTT had not been recovered, pancreatic glucagon response was diminished, pancreatic exocrine function had been declined with increased plasma levels of secretin and CCK, and gastric secretion increased in spite of diminished gastrin response. After 74 to 92% pancreatectomy, 17.6% of dogs developed DM. The dogs with DM had poor pancreatic regeneration rate of 22.7% at 12 weeks, hypersecretion of glucagon and decreased gastric secretion with low plasma concentration of gastrin. On the other hand, in dogs without DM, pancreatic regeneration rate showed 42.7% at 12 weeks, insulin release and pancreatic exocrine function had been recovered well, and plasma CCK levels increased without changes of gastric secretion. In the clinical study, gastric secretion, CCK response and pancreatic endocrine and exocrine functions had been maintained better after pylorus preserving pancreaticoduodenectomy than after conventional pancreaticoduodenectomy.

[Malignant mesenchymoma of the retroperitoneum: a case report].

This is a case report of a malignant mesenchymoma of the retroperitoneum. On December 1988, a 60-year-old man was hospitalized with complaints of painless and increasing swelling in the left side of the abdomen. The tumor was resected on March 1, 1989; tumor weight was 4,780 g. Histologically, the lesion contained liposarcomatous, chondrosarcomatous, fibrohistiocytic and fibrosarcomatous components. After the tumor resection, he received chemotherapy according to the CYVADIC regimen, and he has been doing well without any evidence of recurrence or metastasis.

Brain metastasis from transitional cell carcinoma of the bladder--case report.

A rare case of brain metastasis from a transitional cell carcinoma (TCC) of the bladder is presented. A 66-year-old female underwent total removal of a bladder carcinoma and postoperative radiation therapy and chemotherapy. Right hemiparesis and seizures developed about 16 months later, and a solitary brain metastasis was discovered in the left Rolandic area. The tumor was totally removed and microscopic examination showed it to be TCC.

Renal function and growth after antireflux surgery in children.

The outcome of antireflux surgery for vesico-ureteral reflux (VUR) was studied in 39 children (65 renal units). The T max of the renogram improved significantly after operations in 51 (78.5%) of the 65 renal units due to a reduction of the back pressure. Improvements in scarring were observed in only 3 (7.3%) of 41 scarred kidneys. The growth of 38 kidneys was examined using the renal ratio, more than 2 years (mean, 3 years and 1 month) after the operation. The normal renal ratio was restored in 1 of 12 small kidneys with a preoperative size of less than the mean -2SD. The other 11 kidneys remained small, but 6 of these grew with a normal growth curve. The growth of the remaining 5 kidneys was unremarkable. Of the 26 kidneys with a normal renal ratio, 4 became small after the operation, but all the others had growth within the normal range.

Acute-phase kidney transplants show early peak appearance in Tc-99m DTPA renogram.

Kidney transplants have been evaluated using a radionuclide technique to determine perfusion index (PI), mean transit time (MTT), and the parameters in renogram curve (Tmax, T1/2, and T2/3). The values for Tmax, T1/2, and T2/3 were also determined in normal volunteers. The results indicated that the Tmax value was less than 2 minutes, one week after transplantation. The other data were within normal limits. The appearance of Tmax was very early. The early time activity curve (TAC) was quite similar to the blood disappearance curve, TAC did not have the functional-phase, so-called, 2nd-phase. These data suggested that there could be an abnormality of the renal circulation in the transplanted kidney. The suggestion offers the new finding in a field of renal transplantation using radionuclide technique.

[Moyamoya disease in fraternal twins].

The authors have reported here fraternal twins of moyamoya disease. The one has the onset at the age of two years and six months. Then he had suffered from multiple cerebral infarction and resulting in severe neurological deficits. Now he has right hemiparesis, left homonymous hemianopsia, aphasia and mental retardation. The encephalomyo synangiosis was done to the boy bilaterally at the age of five years. The other one has the onset at the age of five years and five months. He had good physical and neurological development. The Superficial temporal artery-Middle cerebral artery anastomosis and Encephalomyo synangiosis were done bilaterally. Now his development has no problems. The twins and their younger sister all have the same HLA type. The hereditary and environmental factors may be completely related to the pathogenesis of this disease.

[Brain metastases from carcinomas of the digestive organs--a comparative study with those from pulmonary carcinomas].

During the past 11 years, we have experienced 7 cases of a metastatic brain tumor with the primary lesion in the digestive organs. Their clinical courses were retrospectively compared with those of 19 cases with a brain metastases from pulmonary carcinomas. Leptomeningeal carcinomatosis was found in 2 patients of the former group but none in the latter. Six out of 7 brain metastases in the former group were found to have averaged 13 months before their appearance after the diagnosis of the primary lesion. In contrast, 14 out of 19 patients in the latter group had an onset with symptoms of a brain metastasis. The average survival after intracranial surgery on the metastatic tumor was 174 days for the former group and 268 days for the latter. The prognosis of a brain metastasis from a carcinoma of the digestive organ appears to be extremely probable.

Effect of aminophylline on ventilatory response to hypercapnea in patients with asthma.

The nature of ventilatory response to CO2 in patients with airway obstruction is controversial. Although the bronchodilating effect of aminophylline has been well documented, its effect on direct stimulation of breathing has not been systematically evaluated. Therefore, the effect of intravenous aminophylline on ventilatory response to CO2 of patients with asthma was studied by comparing with that of the healthy subjects. Ventilation (VE) at rest was not significantly different between in asthmatics and healthy subjects, and mouth occlusion pressures (P0.1) at rest was significantly higher in asthmatics than in healthy group (p less than 0.05). P0.1 after aminophylline increased significantly (p less than 0.05) compared to control value in asthmatics. Changes in VE and P0.1 relatively correlated well in both groups before and after aminophylline (r = 0.58). Ventilatory response to CO2 (VE/PA CO2) and P0.1 response to CO2 (P0.1/PACO2) in asthmatics were slightly increased, but not significantly different from those in the healthy group before and after aminophylline. These findings suggest that aminophylline might be effective in enhancing ventilatory response to CO2 in asthmatics than in healthy subjects, because of its bronchodilating effect and stimulation to the respiratory neurons.

Synthesis of new heparinoids with high anticoagulant activity.

New heparinoids were synthesized by the chemical method starting from ring-opening polymerization of anhydro sugar derivatives. Sulfation of synthetic (1----6)-alpha-linked 3-amino-3-deoxy-D-glucopyranan and its copolymers gave dextran-type heparinoids having a sulfamide group on the C-3 carbon of the sugar unit. Heparinoids with different sulfamide contents indicated that the anticoagulant activity (35.3-41.3 units/mg) is independent of the sulfamide content, while an increase in sulfamide content lowered the toxicity. Sulfation of (1----5)-alpha-D-xylofuranan and -ribofuranan provided furanan-type heparinoids the anticoagulant activities of which were higher than those of the corresponding sulfated pyranan-type polysaccharides (1----4)-beta-D-xylopyranan and -ribopyranan. The highest activity (69.1 units/mg) was shown by sulfated (1----5)-alpha-D-xylofuranan. The dextran-type heparinoid having a sulfamide group showed a high anticoagulant activity also in vivo and high lipemia-clearing activity.

[A case of malignant peritoneal mesothelioma with a good response to treatment].

A rare case of malignant peritoneal mesothelioma in a 49-year-old Japanese woman is presented. The tumor was limited to the peritoneum, and was cytologically and histologically identified as being composed of two components, epithelial and non-epithelial forms. The transition in her CA-125 level suggested that this tumor had good prospects of developing into a malignant mesothelioma. The good prognosis of the patient is considered to be attributed to detecting this tumor in its early stage and to aggressive chemotherapy.

[Clinical studies on flomoxef in respiratory infection].

Clinical efficacy, bacteriological effect and safety of a new antibiotic flomoxef (FMOX, 6315-S) in respiratory infections were studied. Efficacy of FMOX in 6 patients with infectious diseases including 2 cases with pneumonia, 3 cases with acute exacerbation by respiratory infection, 1 case with obstructive pneumonia were clinically evaluated. Two strains of Haemophilus influenzae, 1 strain of Streptococcus pneumoniae and 1 strain of Staphylococcus aureus which were detected as causative organisms in 2 cases disappeared or decreased after treatment with FMOX. Assessing both clinical and bacteriological findings, effects of FMOX were good in 5 cases and fair in 1 case. No adverse effects were observed in clinical or laboratory findings. Consequently, FMOX is considered to be a very useful antibiotic in the treatment for respiratory infectious diseases.
