Efficacy of low-dose pravastatin in patients with mild hyperlipidemia associated with type II diabetes mellitus.

A 16 week, randomized, double-blind, parallel, placebo-controlled study was designed to determine the effects of low-dose pravastatin on cholesterol concentrations in patients with mild hypercholesterolemia and non-insulin-dependent diabetes mellitus (NIDDM). Following a 6-to 8-week dietary run-in period, a mean serum total cholesterol (TC) level > 5.2 mmol/L (200 mg/dL), but < 7.8 mmol/L (300 mg/dL) was required for entry. Metabolic control of diabetes was determined by a hemoglobin Alc (HbAlc) level less than twice the upper limit of normal on two occasions. Eighty six (86) patients recruited in 5 French diabetic clinics, were randomized in a ratio of 1:1 (pravastatin 10 mg or placebo), and 74 completed the study. There were 12 discontinuations: 5 (11.6%) in the pravastatin group and 7 (16.3%) in the placebo group. Drop-out was due to an adverse event in 1 patient (2.3%) in the pravastatin group and in 5 patients (11.6%) in the placebo group. Thirty five (35) placebo patients and 14 pravastatin patients had their dose of treatment doubled at week 8: the dose of treatment was to be doubled at week 8 in the event of non-response to treatment (TC at week 7 > 5.2 mmol/L and TC decrease < 15% from baseline). At week 16, pravastatin lowered TC from 6.4 to 5.6 mmol/L (-13.8%, p < 0.001 versus placebo), low-density lipoprotein cholesterol (LDL-C) from 4.3 to 3.4 mmol/L (-20.4%, p < 0.001 versus placebo) and slightly increased high-density lipoprotein cholesterol (HDL-C) from 1.18 to 1.25 mmol/L (+6.7%). Side effects were similar in both groups. Blood glucose control was not altered as assessed by serial HbAlc measurements which were unchanged during treatment. This study demonstrated that low-dose pravastatin is effective in lowering cholesterol levels in patients with hypercholesterolemia and NIDDM.

[Twin pregnancy with acute hepatic steatosis followed by antehypophyseal insufficiency and diabetes insipidus].

A 22-year old woman in the 37th week of her third pregnancy (twins) developed acute fatty liver complicated with a haemorrhagic syndrome from disseminated intravascular coagulation. Two normal girls were delivered by caesarean section. Persistent surgical bleeding required hysterectomy and a short stay in an intensive care unit. The disseminated intravascular coagulation subsided within 8 days. Three weeks after delivery a pituitary insufficiency (Sheehan's syndrome) was diagnosed. A second liver biopsy showed that the lesions had regressed. One week after delivery, the patient developed polyuria and polydipsia. The diagnosis of diabetes insipidus was confirmed by the lack of increase of plasma antidiuretic hormone level during an 8-hour water deprivation test. The pathophysiology of these different syndromes is discussed. Disseminated intravascular coagulation might be the link between hypopituitarism and diabetes insipidus.

[Calcium and phosphorus metabolism in insulin dependent and non-insulin dependent diabetics, well-controlled and poorly-controlled].

We studied phosphorus and calcium metabolism in 50 adult insulin dependent and non insulin dependent diabetics arranged in 4 groups according to therapy and control of diabetes. We observed: a low level of blood magnesium in all diabetics a lower level of P T H, more pronounced with poorly controlled diabetes. a decrease of 1-25 (OH) 2 D levels without modification of the 25 (OH) D levels in badly controlled diabetics. This decrease may be related to the low level of PTH with a 1 alpha hydroxylation defect. These results are in favor of the hypothesis of a primary bone problem leading to the pre-senile and subclinical osteoporosis observed in diabetics. Hyperglycaemia rather than insulinopenia may be involved. Rigorous diabetes control significantly decreases all the observed differences, except the low magnesium level.

[Association of autoimmune dementia, Basedow's disease, and Gougerot-Sjögren syndrome].

The authors observed one case of myoclonic dementia reversible with steroid therapy, associated with Grave's disease and Sjögren's syndrome. A circulating IgG fraction was demonstrated reacting specifically with cerebral micro-vessels and which evolved in parallel with the neuro-psychiatric disturbances. These results support the hypothesis of an autoimmune vasculitis causing the dementia. The persistence of abnormalities one year after clinical cure showing an alteration of the blood-brain barrier associated with the appearance of circulating antibodies against the hypophyseal cells, and in view of the predictive value of these antibodies, suggests a possibility of the eventual apparition of an Alzheimer type dementia.

[Thyroid hormones during a hypocaloric diet in obese patients].

The levels of T3, RT3, T3/RT3, T4, FT4 index or T4/TBG, TSH, at days 1, 4, 8, 15 of a 800-1 000 diet with 60-80 g of carbohydrates, were studied in 45 subjects. Statistical analysis was performed using parametric and non parametric tests for comparison of the mean levels between days 1 and 4, 1 and 8, 1 and 15, 4 and 8, 4 and 15, 8 and 15. We observed no difference between men and women. We noted no modification in the evolution of T4, FT4 or T4/TBG and TSH levels. A significant T3 decrease and a RT3 increase with a clear cut decrease of the T3/RT3 ratio from day 1 to 15 principally marked from day 4 to 15 were found. These hormonal modifications correlate well with the weight fall.

[Beta 2 microglobulin in diabetic patients. Apropos of 190 subjects].

Beta-2 microglobulin (beta 2 M) is a low molecular weight protein filtered by the renal glomerulus, then reabsorbed and metabolized at the proximal tubule. Its blood concentration is a good renal index, as it is independent from muscle mass and diet contrary to creatinine. We assayed serum beta 2 M in 190 cases of diabetes mellitus divided into 71 non-insulin-dependent and 119 insulin-dependent forms. We found no significant difference between both groups. Serum beta 2 M was not correlated with Hb A 1 C. Conversely, a highly significant positive correlation between beta 2 M and serum creatinine and a negative correlation between beta 2 M and creatinine clearance were demonstrated. Furthermore, patients with borderline serum creatinine and those with normal renal function show very significant differences in mean serum beta 2 M concentrations, thus making combined assay of both parameters advisable. On the other hand, we were unable to evaluate modifications in beta 2 M according to other complications of diabetes mellitus as vascular and neurologic involvement are very often associated with renal dysfunction.

[Study of the course of development in 84 non-insulin-dependent diabetics over a 5-to-10-year period. Their therapeutic outcome].

The clinical, biological and therapeutic course of non-insulin-dependent (type II) diabetes mellitus was studied in 84 patients followed up for five to ten years. Three-quarters of these patients, who had had diabetes mellitus for approximately ten years in most cases, were well controlled although they remained overweight, presumably as a result of poor compliance with nutritional prescriptions. Disturbances in lipids were only incompletely corrected in 60% of patients. Arterial hypertension persisted in 15%. Oral therapy remained effective in 70% of patients. Among the 30% of patients given insulin, 13% were so treated because of renal failure. Notwithstanding the satisfactory control of diabetes mellitus, probably as a result of the persistence of other risk factors, micro and macroangiopathic complications occurred or worsened during the follow up period.

[Insulin reserve of insulin-dependent diabetics. Role of C-peptide in the hormonal balance in diabetes].

Insulin secretion was investigated in 65 insulin-dependent diabetic patients by determining C peptide levels before and after stimulation with either an 800 calories test meal or an injection of glucagon or arginine i.v. Most patients had very low basal C peptide levels and response to all stimulation tests was poor as compared to controls. In normal subjects, response to glucagon or arginine stimulation appeared to be slightly stronger than response to test meal stimulation. There was apparently no correlation between diabetes control, as assessed by haemoglobin A1c values, and C peptide levels before and after stimulation. On the other hand, C peptide levels were inversely proportional to the duration of diabetes. Finally, C peptide levels after test meal stimulation were found to correlate with the doses of insulin administered.

[Pituitary-gonadal function in male diabetic patients (author's transl)].

Pituitary-gonadal function was studied in 50 male diabetic patients under 53 years of age. Forty-three had normal sexual activity and 7 were impotent. Plasma testosterone levels and urinary 17 ketosteroids, androsterone and dehydroepiandosterone levels were measured. LH and FSH levels before and after LHRH, and prolactin levels before and after TRH were also measured in plasma. No significant changes in pituitary-gonadal function were detected, irrespective of the patient's sexual activity. Neither the type and degree of control of diabetes nor the presence of absence of microangiopathy had any influence on the results. Basal LH and FSH levels were slightly higher in older patients. Prolactin levels after TRH were significantly higher in the later stages of the test in patients with microangiopathy.

[Partial progressive lipoatrophy. A new case-report (author's transl)].

A new case of lipoatrophic diabetes with some peculiar features is reported. The disease was acquired, with onset at approximately thirty years of age, and partial, involving only the face and the upper limbs. The authors were able to observe the successive occurrence of the various symptoms: progressive lipoatrophy, hyperinsulinism, insulin-resistant diabetes, major lipid disturbances, and finally renal failure with a nephrotic syndrome and high blood pressure. Nosologic borders and physiopathology are discussed with a review of the literature.

[Diabetic proteinuria. Statistical study (author's transl)].

One hundred and six records belonging to diabetics carrying proteinury were studied by uni-dimensional analysis, then multi-dimensional of thirty six variables characterizing diabetes, its complications micro and macroangiopathics and th associated biological perturbations with regards to proteinury. The results permit us to lessen the criterium of risks of the appearance of a diabetic nephropathy and also the linked anomalies.
