Intra-articular calcifying synovial sarcoma mimicking synovial chondromatosis.

A case of intra-articular calcifying synovial sarcoma of the right knee joint in a 27-year-old man is reported. Prior to admission the patient had been suffering pain in the right knee for 5 years. Plain radiographs showed multiple calcified densities in the anterior aspect of the right knee closely mimicking synovial chondromatosis. Histological examination revealed that the lesion was a synovial sarcoma with extensive calcification subjacent to the synovial surface. Although intra-articular calcifying synovial sarcoma is very rare, it is important to be aware of the fact that synovial sarcoma, as well as synovial chondromatosis, can be present as a calcified lesion in the joint cavity.

[Gastric mucosal atrophy and prevalence of Helicobacter pylori in reflux esophagitis of the elderly].

This study is aimed at a role of Helicobacter pylori (HP) infection in reflux esophagitis of the elderly. 46 patients with reflux esophagitis aged at older than 60 years are selected for this study with informed consent. 43 patients without reflux esophagitis, peptic ulcer, and gastric cancer are used as a control group. In reflux esophagitis, gastric mucosal atrophy is judged as closed type of endoscopic findings in all cases. In control, 27 of 43 patients were judged as open type. Serum pepsinogen I, II ratio is 4.73 +/- 1.28 which is higher significantly than 3.39 +/- 1.69 in control. Serological positive rate of HP antibody is 39.1% in reflux esophagitis. This rate is significantly lower than 62.7% in control. In conclusion, low frequency of chronic HP infection protects gastric mucosa from atrophy, and keeps secretion of gastric acid, resulting in reflux esophagitis of the elderly accompanied with various abnormal esophago-gastric functions.

[A case of thymoma in an elderly patient associated with myasthenia gravis].

A case of thymoma in an elderly patient associated with myasthenia gravis (MG) was reported. A 81-year-old female with growing dim of eye was diagnosed as MG, and treated with Mestinon. Two months later, she was detected to have an anterior mediastinal tumor on chest rentogenography and CT. She was diagnosed as MG (Osserman type IIA) with thymoma by Tensilon test and high anti-acetylcholine-receptor antibody level. Extended thymothymectomy was performed and the tumor was encapsulated and not invasive (stage I, lymphocyte predominant type). Postoperative course was good and subjective symptoms were improved without medication.

[Methodological study of 13C-urea breath test for detection of Helicobacter pylori infection].

In this study, we investigate simple breath test for detection of Helicobacter pylori (HP) infection using 13C-urea. Thirty-nine patients (30 were HP positive, 9 were HP negative) were given three different doses (50, 100 and 150 mg) of 13C-urea at fasting, and keep sitting after mouth washing with water. Breath samples were taken before and 10, 20, 30, 45, and 60 minutes after urea administration. More than 100mg of 13C-urea was necessary for correct diagnosis of HP infection, because 2 HP positive cases were not detected by 50mg 13C-urea administration. In cases with patchy distribution of HP in the stomach, it may be necessary to change the posture to distribute urea within the whole stomach. In most of HP positive cases, peak delta 13CO2 were obtained within 30 minutes, but one HP negative case showed high delta 13CO2 at 10 minutes, which was probably caused by urease activity in the mouth. So it is appropriate to take breath sample at 20 minutes after urea administration. In this study, cut-off value for a positive test can be setted between 4 to 7 delta/1000, it is necessary to investigate much more cases to set exact cut-off value.

Isolation and characterization of insulin-like growth factor-I from rainbow trout, Oncorhynchus mykiss.

Insulin-like growth factor-I (IGF-I) has been purified from plasma of adult rainbow trout, Oncorhynchus mykiss. Plasma samples were collected 48 hr following injection of recombinant tuna growth hormone at a dose of 0.5 microgram/g body weight. Acid-ethanol extract of plasma was fractionated by gel filtration on a Sephadex G-50 superfine column. Two-step ion-exchange chromatography on DEAE-52 and then Mono-S columns followed. Rainbow trout IGF-I was further purified by immunoaffinity chromatography with anti-recombinant coho salmon IGF-I (rsIGF-I) serum and HPLC on a reverse-phase C18 column. During purification, trout IGF-I was monitored by SDS-PAGE, immunoblotting with anti-IGF-I-serum, homologous radioimmunoassay (RIA) for salmon IGF-I, and sulfation bioassay. The trout IGF-I appeared on SDS-PAGE as a single band with a molecular weight of 7 kDa, the same size as rsIGF-I. The partial N-terminal amino acid sequence (residues 1-20) was identical to the predicted mature trout IGF-I cDNA sequence. Trout IGF-I cross-reacted with anti-rsIGF-I serum in immunoblotting and its dilution curve was parallel to the rsIGF-I standard curve in salmon RIA. In concentrations of 50 and 500 ng/ml, trout IGF-I significantly stimulated sulfation uptake by the cultured branchial cartilage of rainbow trout. This stimulatory effect of trout IGF-I was dose-dependent and similar in its biological potency to rsIGF-I.

Immunocytochemical localization of IGF-I in the ovary of the red seabream, Pagrus major.

There are many biochemical and molecular biological data indicating that IGF-I is present in teleost fish. The present study examines red seabream for the presence and cellular distribution of IGF-I in the ovarian follicles during oocyte growth and final maturation of oocytes by immunocytochemistry. Immunoreactivity appeared in the granulosa cells at the lipid stage. Immunoreactivity became strong at the primary yolk globule stage and diminished in intensity during development of maturational competence (responsiveness to maturation-inducing steroid) and final maturation of oocytes. Intense immunoreactivity was also localized in the outer layer of the zona radiata and the peripheral region of the ooplasm at the primary yolk globule stage. Postovulatory follicles had low immunoreactivity in the early phase of degeneration. These immunocytochemical observations show that the granulosa cell layer is the main site of IGF-I production. IGF-I may be involved in granulosa cell proliferation and differentiation.

Switching dynamics and the transient memory storage in a model enzyme network involving Ca2+/calmodulin-dependent protein kinase II in synapses.

The signal processing through a chain of phosphorylation-dephosphorylations mediated by a pair of enzymes, Ca2+/calmodulin-dependent protein kinase II and the associated phosphatase, is formulated as a nonautonomous dynamical system in the framework of nonautocatalytic, intraholoenzyme reaction dynamics. A classification of switching characteristics of the system is made in the parameter space comprising the three controllable system parameters: an input-pulse intensity and initial concentrations of the two associated enzymes. It is found that a region of parameter space exists termed the transition zone, that exhibits a quasi-switching behaviour characterized by a signal storage time being prolonged by more than several orders of magnitude (10(4) times in certain cases) for the increase of two orders of magnitude in the input signal intensity. The effect of alterations of certain rate constants on the quasi-switching property is explored. It is numerically demonstrated that the Ca2+/calmodulin-dependent kinase II-related phosphatase is the most important key enzyme for regulating the signal storage time.

Multiplex ARMS-PCR-RFLP method for high-resolution typing of HLA-DRB1.

A reliable method for high-resolution HLA-DRB1 typing using the combination of group-specific amplification and RFLP analysis is described. Group-specific PCR amplification (multiplex ARMS-PCR) was carried out under the same conditions for all groups using seven different primer pairs divided into four groups: (1) DR1 and DR10; (2) DR2, DR7 and DR9; (3) DR3 DR5, DR6 and DR8, and (4) DR4. The subsequent polyacrylamide gel electrophoresis was used to determine the group(s) contained in each sample. DR1, DR2/7, DR3/5/6/8, DR4, DRB1*0901 and DRB1*1001 could be distinguished easily using this system. Computer analysis of the various restriction enzyme cleavage sites was carried out on 105 DRB1 allele sequences. It was shown that all DRB1 alleles, except for five allele pairs and some alleles possessing silent mutations, could be distinguished with commonly available restriction endonucleases. Computer analyses on the discrimination of the heterozygous and homozygous combinations were also carried out. Although some heterozygous combinations could no be distinguished with single digestion, double digestion using two restriction enzymes could distinguish most of such heterozygotes. The results of the typing of 100 Japanese individuals using this method showed good agreement with those obtained by other methods.

Increased plasma insulin-like growth factor-I (IGF-I) following oral and intraperitoneal administration of growth hormone to rainbow trout, Oncorhynchus mykiss.

Changes in plasma insulin-like growth factor-I (IGF-I) and growth hormone (GH) levels in rainbow trout, Oncorhynchus mykiss, were investigated after either intraperitoneal or oral (intragastric) administration of salmon GH. Plasma IGF-I and GH levels were measured by homologous radioimmunoassays for salmon IGF-I and GH. After intraperitoneal injection of salmon GH at doses of 0.1 or 1.0 micrograms/g body weight to rainbow trout, plasma GH levels increased to a maximum after 6 h and declined rapidly thereafter. Plasma IGF-I levels were elevated after 12 h, reaching maximum at 24 h and (after the higher dose of GH) remaining high at 78 h after GH injection. In contrast, following intragastric administration of salmon GH at the same doses, plasma GH levels were significantly elevated 12 h after administration, reaching a maximum at 15 h and declining to basal levels at 24 h. Plasma IGF-I levels increased significantly after 48 h and remained elevated (after the higher dose of GH) for more than 96 h. Effects on plasma GH and IGF-I were dose-dependent notwithstanding the route of administration of GH. These results indicate that plasma IGF-I in salmonid fish is under GH control and that oral administration of the hormone is followed by longer-lasting effects than those achieved by intraperitoneal injection.

[Treatment of 11 cases of pulmonary arteriovenous fistula].

We have experienced a total of 11 patients operated for pulmonary arteriovernous fistula (PAVF) from 1976 to 1993 consisting of three males and eight females with the average age of 35 years. To reserve the pulmonary function, limited resections were applied for all cases (partial resection in 8, fistula excision in 2, and segmentectomy in one case). The most recent case, a 36-year-old female who had two abnormal shadows on her routine chest X-ray, was diagnosed as PAVF by pulmonary arteriogram. She was treated by fistula excision in the right S8 lesion and embolization in the left S1 + 2 lesion. Because of its unsatisfactory natural course, PAVF should be treated aggressively by surgery and/or by embolization according to the size, shape and localization.

[A case of near total tracheal obstruction from recurrent esophageal carcinoma].

We have treated a 58-year-old man with almost complete tracheal occlusion arising as a result of recurrence of esophageal cancer. Although laser photoresection using partial cardiopulmonary bypass was desirable, neither laser nor cardiopulmonary bypass was available in our hospital. It was then decided to resect the tumor by forceps. In order to prevent bleeding from tumor during such emergency procedure, intratumoral injection of ethanol was preceded and tracheal lumen could be opened successfully. He was managed by insertion of a Dumon silicone stent seven days after the emergency. The result was satisfactory for maintaining the patency of the airway where the stent was placed. Unfortunately the patient died 113 days after the insertion of Dumon silicone stent from massive hemoptysis, which might resulted from the rupture of vessel feeding the tumor or the perforation of aorta involved by the tumor.

[Clinical analysis of lung cancer in patients with chondromatous hamartoma].

Between 1978 and 1993, 776 patients with lung cancer and 18 patients with chondromatous hamartoma were operated on in the department of Surgery II in Okayama University Hospital. 3 patients had both lung cancers and chondromatous hamartomas. They were all men. Two patients were 65 years old and the other was 82 years old. They all pointed out them by a medical examination of chest X-ray. 2 patients had lung cancers and chondromatous hamartomas in the same lobe, the other had them in other lobe of the same side. Lung cancer and chondromatous hamartoma localized in near area. It was difficult to discriminate chondromatous hamartoma between pulmonary metastasis of lung cancer before an operation.

Development of a homologous radioimmunoassay for coho salmon insulin-like growth factor-I.

A specific homologous radioimmunoassay (RIA) for measurement of insulin-like growth factor-I (IGF-I) in plasma of salmonid and a few non-salmonid fish species was developed using recombinant coho salmon IGF-I (rsIGF-I) as tracer and standard, and antiserum against this peptide raised in rabbits. The minimum detection level of IGF-I was 1.5 ng/ml and linearity was obtained in a range from 1.5 to 23 ng/ml. No cross-reaction was detected in the salmon IGF-I RIA with mammalian growth factors, salmon pituitary hormones, salmon or mammalian insulin, or any peptide in rat plasma. Although salmon IGF-I has high sequence similarity to mammalian IGF-I, it did not cross-react with anti-human IGF-I serum in human RIA and serial dilutions of plasma from salmon were not parallel to the human IGF-I standards in this assay system. In contrast, dilution curves for plasma of salmonids, such as coho (Oncorhynchus kisutch), Atlantic (Salmo salar), and sockeye (O. nerka) salmon, rainbow trout (O. mykiss), some other teleost fish, such as tilapia (Oreochromis mossabmica), carp (Cyprus carpio), eel (Anguilla rostrata), Atlantic halibut (Hippoglossus hippoglossus), and agnathan, the sea lamprey (Petromyzon marinus), assessed in salmon IGF-I RIA were parallel to the rsIGF-I standards. Acid-ethanol extraction of plasma samples altered the molecular weight, but not the quantity, of immunoreactive IGF-I, implying that IGF-I binding proteins in salmon plasma do not affect the performance of the salmon IGF-I RIA. Gel filtration of nonacidified plasma on a Sephadex G-75 superfine column produced two immunoreactive IGF-I peaks of molecular weights of approximately > 70 k and 7 kDa, whereas acidification of plasma increased the relative amount of the 7-kDa peak (IGF-I) and the > 70-kDa peak disappeared. The recoveries of rsIGF-I added to extracted or nonextracted plasma were 97.4 and 94.9%, respectively. Inter- and intraassay coefficients of variation were 3.6 and 3.3%, respectively. Plasma IGF-I levels in coho salmon smolts were 117.4 +/- 19.1 ng/ml as compared to IGF-I levels in parr (45.3 +/- 2.5 ng/ml) or in adult fish (45.2 +/- 5.4 ng/ml) measured in the same assay. Injection of salmon growth hormone, but not prolactin or somatolactin, caused a significant and dose-dependent elevation of plasma IGF-I levels, while either fasting or injection of streptozotocin led to a significant decline in systemic IGF-I.(ABSTRACT TRUNCATED AT 400 WORDS)
