HPLC determination of dexamethasone in human plasma and its application to an in vitro release study from endovascular stents.

A simple, accurate and precise HPLC assay was developed and validated for determination of dexamethasone in human plasma. Triamcinolone acetonide was used as internal standard (I.S.) and plasma samples were extracted using liquid-liquid extraction with ethyl acetate. Chromatography was performed on a C-18 column with acetonitrile-triple distilled water (28:72% v/v, pH adjusted to 2.3 with phosphoric acid) at a flow rate of 1.2 mL/min and detection wavelength 254 nm. The assay was linear at a concentration range of 0.25-6 microg/mL with recoveries >77%. Precision and accuracy were within the acceptable limits. The method was used to determine dexamethasone release from different material coated endoluminal vascular stents in in vitro human plasma experiments. The results were useful in identifying a good coating material for the stents.

The Combitube in elective surgery: a report of 200 cases.

BACKGROUND: The Combitube has proved to be a valuable device for securing the airway in cases of difficult intubation. This study investigated the effectiveness of the Combitube in elective surgery during both mechanical and spontaneous ventilation. METHODS: Two hundred patients classified as American Society of Anesthesiologists physical status I and II, with normal airways, scheduled for elective surgery were randomly allocated into two groups: nonparalyzed, spontaneously breathing (n = 100); or paralyzed, mechanically ventilated (n = 100). After induction of general anesthesia and insertion of the Combitube, oxygen saturation, end-tidal carbon dioxide and isoflurane concentration, systolic and diastolic blood pressure and heart rate, as well as breath-by-breath spirometry data were obtained every 5 min. RESULTS: In 97% of patients, it was possible to maintain oxygenation, ventilation, and respiratory mechanics, as well as hemodynamic stability during either mechanical or spontaneous ventilation for the entire duration of surgery. The duration of surgery was between 15 and 155 min. CONCLUSIONS: The results of this study suggest that the Combitube is an effective and safe airway device for continued management of the airway in 97% of elective surgery cases.

The nutritionally at risk child. Part 1 : study of some socio-economic and cultural factors.

Various socio-economic and cultural factors prevalent in the Al Hussein Locality were studied, in an attempt to decide the groups of children that are nutritionally at risk. The study was carried out on 110 children suffering from energy protein malnutrition and 98 normal children. Their ages ranged between 4 and 36 months. The per capita income was found to be inadequate to meet the least requirements and it was considered to be the most important high risk factor that malnutrition needs to be tackled in this respect. The results and contribution of the other factors to malnutrition were discussed.

The nutritionally at risk child. Part 2 : study of factors pertinent to the child, siblings or mother.

Several high risk factors were evaluated in an attempt to determine the Egyptian children who are more prone to become malnourished. The study included 110 children with energy protein malnutrition and 98 normal children. Their ages were between 4 months and 3 years. Among the factors investigated, the 2nd 6 months of life was found to be the most critical for nutritional deficiency. Presence of previous siblings with malnutrition, presence of more than 2 sibling deaths and presence of history of severe diarrhoea with dehydration were significantly related to the nutritional status of children. Twinning, birth order and birth interval showed some relation to malnutrition but were not statistically significant. The sex of the child and maternal weight did not influence the nutritional state of children. However, these low-socialclass children were found to be mostly (92.8%) breast fed.
