[Intentional normovolemic hemodilution (preliminary results)].

The authors, after illustrating the advantages of hemodilution from the rheological, hemodynamic and district microcirculatory standpoint, illustrate their experience about 11 patients treated with such method. They point out the advantages this method can bring in both reducing the iatrogenous pathology involved in the hemodilution and reducing the difficulties faced by blood-banks and the costs burdening on the medical services.

Blood Transfusion, Autologous↗

[Indications and technic for autotransfusion in surgery].

The authors, after considering the advantages of autotransfusion, report their experience about 14 election patients operated on, and emphasize the usefulness of the usefulness of the method, designed to remove the iatrogenous pathology involved in the transfusions of homologous blood.

Reduced quality of life in patients with chronic hepatitis C: effects of interferon treatment.

BACKGROUND: Quality of life is an area of increasing interest in hepatology. Studies, so far, have assessed quality of life in patients with chronic virus C-related hepatitis in relation to antiviral therapy by means of generic questionnaires. AIM: To measure quality of life in chronic hepatitis patients without cirrhosis by means of the Nottingham Health Profile questionnaire, a measure of "distress" in comparison with the Medical Outcome Survey SF-36, an index of well-being. PATIENTS: A series of 126 outpatients with chronic hepatitis; 37 on and 89 not on active interferon treatment. METHODS: The two questionnaires were used in random order. Clinical and laboratory data were also collected. The final score of any domain of the two questionnaires, for any individual patient, was compared to age-adjusted normal values obtained in 2 random samples of Italian population. RESULTS: Patients showed a significant modification of 3 domains of Nottingham Health Profile (Energy, Social Isolation and Physical Mobility) and 6 domains of SF-36. In relation to interferon treatment, the Nottingham Health Profile questionnaire was able to detect differences in Energy, Physical Mobility and Pain, which were modified only in treated patients. SF-36 did not show any differences in relation to treatment. In addition, the Nottingham Health Profile demonstrated that treated patients had a lower prevalence of concern for family life, possibly due to expectations of treatment itself. CONCLUSIONS: Active interferon treatment causes considerable distress in chronic hepatitis C patients, adding to the perceived change in health status caused by liver disease.

EEG findings in West syndrome a follow-up of 20 patients.

Clinical and EEG findings of 36 patients with West syndrome (WS) were reported. Twenty patients (7 males, 13 females; mean age 7.4 months at the first EEG examination) were followed for a mean period of 19.3 months. Fifty per cent of the 8 cases with cryptogenic SW showed a full recovery, 2 cases showed persistence of psychomotor retardation or seizures and 2 of both these features. At the end of follow-up period, 92% of the 12 patients with symptomatic WS showed a psychomotor retardation and 75% a persistence of seizures. EEG pattern was a typical hypsarrhythmic one in 11 patients and atypical in the remaining cases. The typical pattern correlated with a better outcome with full recovery in 27.3% compared with 11.1% in the group with atypical EEG. Moreover EEG monitoring was useful for the evaluation of therapeutic response and in the follow-up of the disease, showing a correlation with clinical course, incidence of spasms and outcome.

The burden of obesity on everyday life: a role for osteoarticular and respiratory diseases.

The burden of obesity on patients' everyday life is high; obese subjects perceive a poor Health-Related Quality of Life (HRQL) in both physical and mental dimensions. We aimed to identify the areas of everyday life limited by health status and factors mainly responsible for perceived problems. The Nottingham Health Profile questionnaire (NHP) was used in 274 obese subjects seeking treatment at a university-based obesity center. Values were compared with normative Italian data, corrected for age and sex. Anthropometric and clinical data were also recorded, and correlated with health status. All domains of NHP were significantly impaired in obesity, the effect size ranging from 0.14 (Emotional Reactions; p = 0.02) to 0.99 (Physical Mobility; p < 0.0001), and varying in relation to gender, age and obesity class. Female subjects reported a higher-than-expected prevalence of problems in most areas of daily life [from 20% (Paid Employment) to 44% (Jobs around the home), compared with 14-26% in controls]. Males reported a high prevalence of problems in Sex life (31%), Holidays (37%) and Hobbies (49%), compared with 14, 14, and 16% in controls, respectively. Logistic regression analysis identified osteoarticular pain (knee and hip pain) and respiratory diseases as major factors predicting a poor HRQL in its physical dimensions, or perceived problems in everyday life. The relative importance of knee pain was higher than that of hip pain. Osteoarticular and respiratory diseases are major determinants of poor HRQL in obesity. Prevention strategies and treatment of somatic diseases are mandatory for a comprehensive approach to obesity.
