[The potentials of intrarectal echography for the preoperative staging and postoperative follow-up of rectal cancer].

The method is reported of intrarectal echography and the ultrasound changes of the rectal wall in cancer. The report is based on experience with 38 patients. The basic criteria for preoperative grading of rectal cancer by the TNM system are determined and the diagnostic exactness of the method is discussed (89.5 per cent). The value of intrarectal echography in determining the extent of the surgical intervention and the postoperative control of the patients is debated.

[The invasive properties of Escherichia coli O18 in relation to cell cultures and infections in inbred mice].

Comparative analyses have been carried out on the dynamics of invasion of cell cultures from epithelioid (MK and HE-p-2) and fibroblast (mice fibroblasts) types with strain Escherichia coli 0 18 isolated and characterized earlier, as well as on the invasion of the viscera of inbred mice (BDF1) after subcutaneous (foot pad lesion test) and intravenous inoculation with the same strain. It was established that the realization of the invasion potential of E. coli 018 depends on the cell culture used: cell cultures of epithelioid type are much more sensitive. The development of the infectious process and the changes in the number of bacteria in the viscera of the inoculated mice for both ways of introduction of the agent were of the same type. The infectious process after subcutaneous inoculation was accompanied with serious orchitis and progressive inflammatory necrotic changes in the inoculated paws leading to demarcation of the whole limb.

[Intraoperative abdominal echography].

Results of the use of abdominal intraoperative echography are reported for the first time in this country. The diagnostic precision, the scanning technique and the problems encountered in examining 56 patients are discussed. The high value of the method in small hepatic tumors, hepatic cysts and abscesses, common bile duct calculosis and pancreas cancer is pointed out. In operative interventions on gastrointestinal neoplasms intraoperative echography may be used for continuing surgical palpation into the butt. To master the method and introduce it as a routine method in clinical practice it is necessary, to use modern equipment, to be skilled in scanning technique and to have thorough knowledge on the characteristics of the intraoperative images of the different pathologic processes.

[Guided punctures in the treatment of hepatic and perihepatic abscesses under ultrasonic control].

The authors discuss the possibilities of ultrasonic tomography and subsequent performance of purposeful punctures for establishing the precise diagnosis of hepatic and perihepatic abscesses. In most cases the method helps in revealing the cavities measuring no more than 1.5-2.0 cm. The indications are determined, as well as the techniques of topical treatment with the use of single and multiple punctures and prolonged percutaneous drainage. Seventy-three patients (32 with hepatic and 41 with perihepatic abscesses) underwent clinical examination. Topical therapy was applied in 21 patients with discovered hepatic abscesses, 17 of them recovered. Operations were avoided in 25 in a group of 34 patients with perihepatic abscesses (in 7 the abscesses were not identified). The success of this treatment is determined by exact diagnosis and precise manipulations.

[Transcutaneous drainage of pancreatic pseudocysts].

The clinical study includes 76 pancreatic pseudocysts localized in the head (24), body (37), and tail (15). The effect of treatment by means of a single or repeated purposeful aspiration in ultrasonic control is discussed. Recovery occurred in 83.3% of cases with cysts measuring up to 5 cm, in 61.3% in those with cysts measuring from 5 to 10 cm, and in 11.1% of cases in which the cysts measured more than 10 cm. As the result of 15 draining manipulations of cysts which were irresponsive to aspiration (9 of them measuring from 5 to 10 cm and 6 larger than 10 cm) complete recovery occurred in 13 cases. The authors discuss the indications for single, two, and many punctures on basis of a complex appraisal of the gland, the size and type of the cyst, and the cytologic results. The indications for percutaneous drainage, the period and techniques of its performance and preliminary and postmanipulation treatment with drugs are determined and the occurring complications are shown.

[The place of echography in the clinical diagnosis of hepatic hemangioma].

On the basis of 104 hemangiomas diagnosed in 81 patients an ultrasound classification of the disease is made which includes four basic variants. Echotomography detects the tumor most successfully in group I (well outlined homogenous hyperechogenisity) while in the other groups it shows a low percentage of correct diagnoses. The average sensitivity of the method is 60.6%. The possibilities of the other basic diagnostic methods are discussed and a scheme for clinical investigation is proposed. Scintigraphy is of no substantial help, computed tomography increases the percentage of the correct diagnoses, angiography and correctly indicated laparoscopy most often end the clinical investigation. The patients with hemangioma are subjected to a prolonged clinical follow up and ultrasound tomography is the best suitable method for this.

[In vitro research on the interaction of natural zeolites with diploid cells from the human embryonic lung].

A study of the biological effect of the natural zeolites, clinoptilolite, mordenite and erionite on monostratum cellular culture of human embryonic lung is performed. The number of pyknotic cells, morphological changes in the cells, and monostratum, mitotic activity and changes in the cellular culture are reported during a continuous experiment. As positive control is used standard quartz dust DQ-12 and for negative dust of TiO2. It is already established that the dust particles of natural zeolites with dimensions under 5 micron display biological aggressiveness. With most strongly expressed effect is the clinoptilolite dust, which related to identical active surface, is comparable to that of quartz DQ-12. The mordenite is with moderate cytotoxic effect, proportional to the time and concentration, while erionite is with slight and very delayed effect. There are differences concerning the effect on the miotic activity of the cells. The biologic aggressiveness of the zeolites is discussed in relation to their strong active surface, their ionometabolic properties and their capacity to generate hydrogen at moistering.

[Method and apparatus setup for preparing test dusts with a specified granulometric composition by air separation].

A method is developed and apparatuses constructed for the preparation of zeolite dusts for biological experiments in vivo and in vitro with dimensions under 5 micron. The method includes autogenic grinding of rocky zeolite material and air separation of the fraction under 63 micron, by means of cyclone and experimentally chosen conditions of work. The received "respirable" dust very well imitates the dust of the air from the working environment. The preparation of different experimental dusts is possible when suitable conditions, for working with the equipment, are chosen and the density of the material for separation is considered.

[Echographic diagnosis of hypernephroid cancer].

A total of 28 hypernephroid tumours have been diagnosed within a period of 32 months. The data from echographic, intravenous urography, computer tomography and thin-needle aspiration biopsy and renovasography were compared in the patients studied. Two basic variants of echographic picture have been described. It has been concluded that the combination of abdominal ultrasound tomography with thin-needle aspiration biopsy is a valuable method for the diagnostics of hypernephroid cancer. Computer tomography proved to be appropriate in solving the problem of operability, renovasography--pre-operatively, and the rest of the methods--in suspected and undistinguished echographic evaluations.

[Biological activity of Bulgarian zeolite rocks].

An evaluation of the biological activity of Bulgarian zeolite rocks was made by the methods in vivo and in vitro. The results pointed out to specific biological effect--cytotoxic and fibrogenic--most strongly expressed for clinoptilolite followed by mordenite and erionite. The clinoptilolite is characterized by strong cytotoxic effect in comparison with dust of calibrated quartz DQ-12 which confirms the lack of correlation between cytotoxicity and fibrogenicity for some types of dust. On the basis of the results a temporary hygienic norm for clinoptilolite dust--2 mg/m3 is proposed.
