Adult T-cell leukemia in spouses.

A married couple who developed adult T-cell leukemia (ATL) is described. The husband presented with acute ATL and died soon after admission in spite of aggressive chemotherapy, and his wife, who is diagnosed as smoldering ATL, has been followed in the out-patient clinic. The couple had serum antibodies against human T-cell leukemia virus type I (HTLV-I) and the monoclonal integration of HTLV-I proviral DNA in their lymphocytes. The patients described represent the first reported example of ATL in a married couple.

[Bronchial arterial infusion of lymphokine-activated killer cells stimulated by autologous tumor cells].

Bronchial arterial infusion (BAI) of Lymphokine activated killer (LAK) cells, stimulated in vitro by autologous tumor cells, was performed for a primary lung cancer patient, a 47-year-old male patient with primary squamous cell carcinoma of the lung who underwent probe thoracotomy and had part of the tumor tissue removed. Peripheral blood lymphocytes were cultured in 500 U/ml of recombinant IL-2(Shionogi Pharm. S6820) for 9 days after in vitro stimulation with mitomycin C-treated primary tumor cells for 3 days. These cells (designated St-LAK cells) were inoculated from the bronchial artery of patients who received 60 Gy irradiation of the primary site and mediastinal lymph nodes. The tumor regressed significantly from 8 X 7 to 3 X 2.5 cm in diameter. The advantages of LAK-BAI using St-LAK cells with irradiation were discussed.

Gastroduodenal complications in patients with adult T-cell leukemia.

Gastroduodenal endoscopic examinations were performed on 15 patients with adult T-cell leukemia (ATL). Twelve had the disease in acute form, two in chronic form and one patient was in crisis. Eight patients had gastroduodenal lesions, four esophageal candidiasis, three gastric infiltration and two duodenal ATL-cell infiltration. Four out of the five patients who had the gastroduodenal ATL-cell infiltration complained of gastroduodenal symptoms such as anorexia, upper abdominal pain, diarrhea and melena. Our observations suggested that these gastroduodenal symptoms were related to the gastroduodenal ATL-cell infiltration. Esophageal candidiasis in ATL could be related to immunodeficiency.

Favorable results of thymectomy combined with prednisolone alternate-day administration in myasthenia gravis.

Administration of anti-cholinesterase before operation and high doses of prednisolone after the operation are commonly prescribed in cases of thymectomy for the treatment of myasthenia gravis. Our policy is to carry out an extended thymectomy combined with pre- and post-operative administration of prednisolone on alternate days. Our method was used in the treatment of 50 patients with generalized myasthenia gravis, from January 1979 to December 1984. The results were examined in conjunction with the necessity for preoperative administration of an anti-cholinesterase preparation and also with factors influencing the follow-up result and postoperative process. This method made it feasible to perform surgery under conditions of stabilization of clinical symptoms and satisfactory postoperative management. A postoperative period of unstable symptoms could thus be avoided. The palliation rate at one year after the operation was 96 per cent, while the remission rate at 5 years after the operation was 70 per cent. The preoperative supplement of anti-cholinesterase and preoperative per cent forced vital capacity were considered to be predictive factors concerning the necessity of artificial respiration.
