[Percutaneous ethanol injection therapy for small hepatocellular carcinoma].

Hepatocellular carcinoma is different from other solid tumors. Because of concomitant cirrhosis or multiple lesions, most hepatocellular carcinoma is unresectable. Still worse, hepatocellular carcinoma frequently recurs after surgical resection; the 5-year cumulative recurrence rate is 70-90% even after curative hepatectomy. The situation is similar in small hepatocellular carcinoma 2 cm or less in diameter. Thus, non-surgical treatment plays an important role. At present, we think that percutaneous ethanol injection therapy (PEIT) is best for the treatment of hepatocellular carcinoma because of its local curativity, minimal adverse effect on liver function, and the easy feasibility of repeated treatment for recurrence. We have recently treated about 85% of hepatocellular carcinoma cases by PEIT and have achieved satisfactory long-term results. Here we describe our results in PEIT for small hepatocellular carcinoma. By the end of December 1995, we performed PEIT on 410 patients with hepatocellular carcinoma. Among them, 140 patients were diagnosed as having small hepatocellular carcinoma 2 cm or less in diameter. The 1-, 3-, 5-, 7-, and 10-year survival rates of the 140 patients were 93%, 73%, 55%, 51%, and 32%, respectively. Furthermore, in 83 patients who had a single, small hepatocellular carcinoma 2 cm or less in diameter, the 1-, 3-, 5-, 7-, and 10-year survival rates were 92%, 82%, 72%, 66%, and 66%, respectively. Thus PEIT achieved satisfactory long-term survival rates in the treatment of small hepatocellular carcinoma.

Purification and Characterization of the (alpha)-Glucuronidase from Thermoanaerobacterium sp. Strain JW/SL-YS485, an Important Enzyme for the Utilization of Substituted Xylans.

A cell-associated (alpha)-glucuronidase was purified to gel electrophoretic homogeneity from the thermophilic anaerobic bacterium Thermoanaerobacterium sp. strain JW/SL-YS485. This enzyme had a pI of 4.65, a molecular weight of 130,000, and two subunits; the molecular weight of each subunit was 74,000. The enzyme exhibited the highest level of activity at pH 5.4 and 60(deg)C, as determined by a 5-min assay. The K(infm) and k(infcat) values of the enzyme for 4-methylglucuronosyl xylobiose were 0.76 mM and 1,083 IU/(mu)mol, respectively. The Arrhenius energy was 26.4 kJ/mol. The specific activities of the enzyme with 4-O-methylglucuronosyl xylobiose, 4-O-methylglucuronosyl xylotriose, and 4-O-methylglucuronosyl xylotetraose were 8.4, 4.8, and 3.9 IU/mg, respectively. The purified (alpha)-glucuronidase and a (beta)-xylosidase purified from the same organism interacted synergistically to hydrolyze 4-methylglucuronosyl xylotetraose.

[Allelic losses of tumor suppressor gene on chromosome 17 in ovarian cancer].

Twenty-one human ovarian epithelial carcinomas (13 serous, 3 mucinous, 2 endometrioid and 3 clear cell type) were examined for allelic losses of p53 tumor suppressor gene, pYNZ22 or pTHH59 on chromosome 17. High-molecular-weight DNA was extracted from ovarian tumor tissues and normal ones obtained from the same patients, and was subjected to Southern blot analysis. The filters were hybridized with p53 cDNA probe or two VNTR probes on chromosome 17(pYNZ22, pTHH59). By comparing the hybridized band pattern of the normal tissue with that of the tumor, loss of heterozygosity (LOH) was detected. Frequent LOHs were detected in 5 of 14 informative cases (36%) on p53 and in 9 of 20(45%) on pYNZ22, but LOH on pTHH59 was observed less frequently than those of p53 and pYNZ22 (one of 7 cases: 14%). These results suggest that allelic loss of p53 may play an important role in the development and/or progression of ovarian carcinomas.

[Chemotherapy sensitivity of gynecologic malignancies determined by the human tumor stem cell assay (HTSCA)].

HTSCA was used to study the chemotherapy sensitivity of gynecological malignant tumors. 34 specimens were obtained from 32 patients. 11 of the 34 specimens (32%) yielded a sufficient number of colonies for testing. On these, 51 separate drug assays were done, each at the 1/10 peak plasma level. 20 drugs were identified as active among 51 drugs tested. Clinical correlations of sensitivity (S) and resistance (R) were studied in 6 patients treated with combination therapies, including drugs selected on the basis of in vitro sensitivity. (formula; see text) We conclude that HTSCA can aid in identifying active drugs for use in designing treatment protocols. However, because of difficulty in preparing a single cell suspension, and the small proportion of tumors suitable for testing, HTSCA will not be used in routine drug sensitivity testing until these drawbacks are eliminated.

[Report on 15 cases of multiple primary cancers in our department].

Of 512 gynecological malignant cases treated in our department, 22 were suspected of multiple primary cancer; 15 were confirmed by clinical and pathological findings, representing an incidence of 2.9% (15/512) of all malignant cases. The most frequent sites of other cancers in patients with cervical cancer were intestinal (5/15), mammary (3/15) and urinary (2/15). Nine (60%) of 15 multiple primary cancers were noted within 12 months. Two rare combinations are also reported. There was a patient with Grawitz's tumor (kidney) and epidermoid cancer (uterine cervix) and one with leiomyosarcoma (uterine corpus) and epidermoid cancer (uterine cervix).

Secretion of gastrin and calcitonin after ingestion of meat extract in patients with peptic ulcer.

Serum gastrin and calcitonin were determined after administration of meat extract to patients with peptic ulcer in order to investigate what role gastrin and calcitionin are playing in peptic ulcer. Serum gastrin levels were elevated after ingestion of meat extract in 9 out of 11 patients with peptic ulcer. Serum calcitonin (CT) levels were elevated rather gradually in 7 subjects. It was suggested that increased endogenous gastrin levels may have stimulated the release of CT. When salmon calcitonin (SCT) was admininstered intravenously at a dose of 100 MRCu before administration of meat extract, the rise of gastrin and CT was suppressed in 67% of cases as to gastrin and in 43% as to CT.
