[Diagnostic and therapeutic aspects of endometrial polyps].

The diagnostic and therapeutic utility of hysteroscopy in intracavity uterine pathologies is now widely recognised. The Authors have evaluated the efficacy of hysteroscopy in 640 patients in a study of endometrial polyps for which the endoscopic technique represents the elective form of therapy, avoiding excessive trauma to the patient and preserving the endometrial mucosa intact. The results obtained are fully discussed. Hysteroscopic resection of polyps was carried out in 49 patients in a day-hospital setting and without further complications.

[LH-RH analogues in the preparation of hysteroscopic resection of uterine fibromyoma].

Ten patients with submucous and/or intracavitary myomas, aged between 40 and 50 years old, underwent preliminary and preoperative (endoscopic resection) treatment with LH-RH. A using a depot formula. The Authors report the results of treatment efficacy (disappearance of symptoms and reduced myoma volume) in relation to an optimal surgical approach using an endoscopic route. This form of treatment allowed both operating times and intraoperative bleeding to be reduced.

[Colpo-cyto-histological correlations in intraepithelial lesions of the uterine cervix].

The aim of this study was to assess the clinical efficiency of colposcopic findings as diagnostic tests for intraepithelial lesion using the analysis of colpo-cyto-histological correlations. The results of 3340 colposcopic tests performed in the Colposcopy Unit of the Second Clinic of Obstetrics and Gynecology between March 1990 and May 1991 were analysed, taking into account 326 (9.7%) cases of Abnormal Transformation Zone (ANTZ). The colpo-histological correlation in ANTZ 1 cases (52 cases of white epithelium, 92 standard mosaic, 42 standard dotted and 61 mixed cases) was 65.2% (52.3%) for HPV; 12.9% for CIN). The colpo-histological correlation in ANTZ 2 cases (30 cases of thickened white epithelium, 5 irregular mosaic, 4 irregular dotted, 2 atypical vessels, 4 thickened gland outlets and 6 mixed cases) was 70.5% (43.3% for HPV; 27.2% for CIN an and Ca). In this retrospective study colposcopy showed a diagnostic accuracy of 64.6% in the cases examined and played a decisive role in the diagnosis of intraepithelial lesions of the uterine cervix.

[Laser CO2: results of treatment of HPV lesions in male the partner].

Hundred-fifty-seven male partners of patients affected by Human Papillomavirus (HPV) lesions, underwent CO2 Laser vaporization between January 1989 and January 1990. The age ranged between 18 and 45 (median 28.2). The vaporization was performed with a Coherent System 451 CO2 Laser equipment, at a power of 16 watt in continuous mode, depth of 2 mm and a free margin of 2 mm. First Laser treatment proved successful in 129 patients (82.1%); 17 out of 28 patients with persistent or relapsed pathology underwent a second CO2 Laser vaporization with a total cure percentage equal to 92.9%. Laser-surgery is the method of choice in the treatment of HPV genital lesions.

[Effects of prolonged administration of dapiprazole in rats].

In this paper we have studied the rat under chronic treatment with dapiprazole ip. (a drug with alpha-adrenergic blocking properties). For a 4 week period we have recorded, once a week, weight, rectal temperature, tail-flick, general activity and explorative behaviour on rotarod and one-arm radial maze. Surface (SEEG) and deep (DEEG) EEGrams were also recorded at the end of the experiments (4th week). SEEG and DEEG underwent the Fourier analysis (Fast Fourier Transform--see text). The findings were: 1) No change was observed in weight, rectal temperature and tail-flick time, glycemia and haematocrit. 2) Motor coordination on rotaroad dropped in 1st and 3rd week of treatment. 3) The treated rats, when in a one arm radial maze, showed signs of sedation (no grooming) from 2nd week of treatment. 4) SEEG and DEEG recordings showed a slowing activity; Fourier analysis confirmed these results. The slowing of activity (4) well agrees with the signs of sedation and behaviour (2, 3). It seems thus confirmed that alpha-adrenergic blockade is a feature of the drug together with the psychotropic effects previously described.

Synthesis of perfluoroalkylated xylitol ethers and esters: new surfactants for biomedical uses.

New, well-defined surfactants and cosurfactants were synthesized with the objective of enhancing the stability of fluorocarbon emulsions destined to serve as oxygen carriers for biomedical applications. Monoperfluoroalkylated ethers of xylitol were achieved by addition of perfluoroalkyl iodide on the double bond of a protected xylitol allyl ether in a one-step addition-elimination reaction. Monoesters were obtained specifically on position 5 by treating 1,2:3,4-di-O-isopropylidenexylitol with perfluoroalkylated acid chlorides of various chain lengths in pyridine at room temperature. The products display strong surface activity and produce a remarkable synergistic stabilization of a fluorocarbon/Pluronic F-68 type emulsion. Biocompatibility data are reported, which include in vitro toxicity tests on Namalva cell cultures and hemolysis tests on human blood cells; the latter was found to decrease as the length of the F-alkyl chain increased. IV injection in mice (n = 10) showed that these products were innocuous at 400-1000 mg/kg of body weight. Preliminary exchange-perfusion experiments on rats with an emulsion containing the F-octyl xylitol ether were encouraging.

The viral lesion (HPV) of the lower genital tract in teen agers: clinical and therapeutic aspects.

From January 1985 to July 1987, 116 adolescents affected by the lesions of the lower genital tract (Human Papilloma Virus +/- Intraepithelial Neoplasia) came to the laser surgery service of the 2nd Department of the Obstetric and Gynecologic Clinic of the University of Rome "La Sapienza" for the treatment of the lesions. The patients were examined also from the epidemiologic point of view and sides effects of the treatment were evaluated. A cure rate of 92% is certainly a significant value, especially considering the higher frequency of recurrence at the vulvovaginal level.

The rationale for treatment of cervical lesions (HPV +/- CIN).

In the laser surgery service of the 2nd Department of the Obstetric and Gynecologic Clinic of the University of Rome "La Sapienza", from October 1984 to March 1987, we have treated with CO2 laser surgery (vaporization and conization) 228 patients affected with cervical lesions (HPV +/- CIN). The choice of the treatment must be based on two parameters: site and extension of the lesion. The results we have obtained with CO2 laser vaporization (201 cases) are most satisfactory (92%); patients have been followed-up from two to twenty-six months. As far as the CO2 laser conization is concerned (27 cases), patients were followed-up from two to twenty-two months and the percentage of success was 96.1%. The Authors evaluated also the side effects of the CO2 laser surgery.

Design, synthesis and evaluation of fluorocarbons and surfactants for in vivo applications. New perfluoroalkylated polyhydroxylated surfactants.

The progress achieved since the advent of Fluosol-DA is summarized in this paper, with special focus on the synthesis and evaluation of more adequate, reliable, industrially feasible fluorocarbons, structure/property relationships--molecular weight being recognized as the pre-eminent determining factor of both the fluorocarbon's excretion rate and the emulsion's stability-, the preparation of significantly more concentrated, more efficacious emulsions, etc. The key to further progress and better mastery of the emulsions' characteristics, especially in relation to increased shelf-life, prolonged i.v. persistence, and versatility, now lies in the development of new surfactants, better adapted to this objective. New families of well defined, monodisperse perfluoroalkylated polyhydroxylated surfactants derived from sugars and related compounds have been synthesized and fully characterized. Preliminary evaluation of their surface-active properties, emulsion stabilizing capacity and biocompatibility are reported.

Cell receptors to sulphated polysaccharides in the acute and chronically inflamed synovial joint.

Receptors to sulphated polysaccharides have recently been discovered on "free" joint fluid cells and synovial membrane cells in the normal joint. A search for these receptors on cells was made in rabbits with acute and chronic adjuvant inflammatory arthritis in an attempt to further elucidate their role in joint homeostasis. These experiments demonstrated a significant increase in cell numbers within the joint. Receptor activity was most marked on macrophages found free within the synovial fluid. It is postulated that exogenous cells may be important in the process of joint destruction and are outside the control of the normal joint regulatory mechanisms. The endogenous cell population, which exhibits receptor activity, may be responding to the process of joint destruction by proliferation as a secondary phenomenon.

Todays' stage in differential diagnosis between cervical condyloma and CIN.

Seven colposcopic signs (acetic acid staining--borders--thickness--surface contour--colour--vascular atypias--iodine staining) are examined in differential diagnosis between condyloma and CIN. For a better prognostic and therapeutic approach, we think it is useful to follow a diagnostic path whose main steps are: colposcopy, cytology, microcervicoscopy, histology. With the introduction of sophisticated diagnostic techniques (molecular hybridization--morphology cells study--chromosomes map study) it is possible to determine morphologically the observed lesion and predict, as much as possible, the following evolution.

Role of diet in endometrial cancer patients.

The hypothesis of a correlation between diet and cancer is widely supported by several epidemiological and experimental studies. Certainly the exact mechanism of this intervention is still unknown; nevertheless in the tests of a carcinogenesis the direct action of some food is recognised, whereas the indirect action of others should occur through modifications of immune and endocrine systems. Many studies conclude that a high intake of fats and animal proteins appeared to be associated with the etiology of some endocrine related neoplasms, such as the cancer of prostate breast endometrium and ovary. We studied the dietary practices and some constitutional parameters of women affected by endometrial cancer and control women. We did not find a significant difference between the two groups related to height. Conversely the daily intake of lipids and particularly of proteins, glucides and calories turned out much higher in the group of patients. Moreover we found a significant difference related to weight; obese women were more numerous in the group of patient. The above results confirm the importance of a right diet in the prevention of this neoplasm.

Skin necrosis due to antiblastics (procedures of prevention and therapy).

Among the toxic effects of antitumor drugs the injury for extravasation occurs too. The kinds of damage which result can achieve dramatic features with serious consequences on the psychophysic activity of patients who once tried other kinds of toxicity following chemotherapy. As the extravasation is caused by way of use and by drug-giving methods, we present this accident: it is necessary to observe rigorous rules of procedure during infusion and, if extravasation occurs, to make use of efficient drugs and physical methods. A recent case of extravasation occurred, at our Gynecological Oncology Service, in a patient who carried out chemotherapy without hospitalization. This case is here proposed and discussed; we think that the rarity of this accident is in our experience, due to the several precautions we observe during the infusion of drug.
