[Analysis of selected parameters of damage to the liver during the course of tick-borne encephalitis].

In 1993, 80 patients (43 female and 37 male) with tick-borne encephalitis were hospitalized in Clinic of Neuroinfections and Parasitic Diseases of Medical School in Białystok. 6 (7.5%) patients were jaundiced, 16 (20%) had hepatomegaly. We noticed increased enzymatic activity of aspartate aminotransferase (x = 74.5 U/l) in 9 (11.25%) cases, alanine aminotransferase (x = 89.5 U/l) in 11 (13.75%) cases, increased level of bilirubin in blood (from 1.4 to 3.5 mg/dl) in 7 (8.75%) cases. Protein level in blood was average x = 5.95 g/dl, percentage of albumin was average x = 48.61%, globulin--x = 14.62%. Prothrombin index and ammonia blood level were in normal range. Observed pathological changes were transient and referred mainly to patients with severe clinical course of disease.

[LDH activity indication in cerebrospinal fluid in aseptic meningitis].

According to our researches we indicated that LDH activity in CSF of patients with TBE differ from values of control group. Among patients with TBE there was no correlation between LDH activity in CSF and cytosis, protein and glucose concentration. No dependence was noticed between LDH activity in CSF and serum. It seems that indication LDH activity im CSF is not helpful in diagnostics of TBE.

[Changes in ECG examination of patients with trichinosis].

In the years 1963-1992, 560 patients with the diagnosis of trichinosis were treated in the Department of Parasitic Diseases and Neuroinfections, including 310 women (55.3%) and 250 men (44.7%) aged from 6 to 75 years. Out of this number of patients in 59 cases (10.5%) myocardial damage was found in the course of the disease. The most frequently found changes in ECG record were ventricular repolarization disturbances (66.1%) which persisted in 18.6% of cases before discharge from the hospital. Depolarization disturbances accounted for 32.2% of cases and persisted before discharge from the hospital in 10.1% of patients. In 6.7% of patients, persistence of pathological ECG record was found during the 4th month after the hospitalization which may be an evidence of prolongation of the inflammatory process within the myocardium.

[Estimation of LDH activity determination in CSF during bacterial meningitis].

According to performed researchers statistically important highest increase activity of LDH was indicated in the group of patients with the most severe course of illness which remains after the treatment. No correlation was shown between increased LDH activity in CSF and pleocytosis, protein and glucose concentrations.

[Hepatic reactions in the course of viral meningoencephalitis in patients hospitalized in 1985-1989].

In the years 1985-1989 in a department for parasitic and occupational diseases in Białystok, 112 patients were hospitalized with the diagnosis of lymphocytic meningitis. The most common aetiological factor were viruses (30.6%), arboviruses (25.9%), and then herpes viruses (6.25%), and mumps virus (5.3%). In 36 patients (32%) no aetiological factor of the inflammatory process was established. In 22 patients (19.6%) a simultaneous hepatocellular damage was found in the form of jaundice (15 patients, that is 13.4%), and hepatomegaly in seven patients. The observed hepatic involvement was found in the group of patients with severe clinical course of meningitis. Most frequently hepatic reaction was observed in the group of lymphocytic meningitis caused by arboviruses. Among the biochemical indices of hepatocellular damage a significant increase in relation to normal value was found of serum bilirubin level (mean = 1.75 mg%; normally mean = 1.1 mg%), alanine aminotransferase activity was 2.5 times higher than the mean normal value (mean = 77.6 IU; normal value mean = 30 IU). The observed hepatic changes were transient and regressed together with the regression of acute inflammatory state in the central nervous system.

[Suppurative meningitis in 210 cases].

Clinical course, etiology and the results of treatment of the suppurative meningitis in 219 adult patients are discussed. Clinical course was extremely severe in 63.3% of the patients, moderately severe in 33.8% of the patients, and complicated by various coexisting disorders and skull trauma in several patients (35.7%). Mortality rate was 8.6%. Combined therapy with 2 or 3 synergetically acting antibiotics was administered. An emphasis is on diagnostic problems especially in patients treated with various antibiotics prior to hospitalization (37.1%) and low percentage of bacteriological tests (22.9%).

[Epidemiologic and clinical studies of patients with tick-borne encephalitis from northeastern Poland].

The authors discussed the epidemiological and serological evaluation as well as the clinical symptomatology of thick-borne encephalitis in 215 patients from North-East Poland. They found, among other things, that in 94 per cent of cases the infection was percutaneous from the tick bite and caused via the digestive route in 6 per cent of cases by drinking unboiled cow milk. It was demonstrated that during the late phase of convalescence i.e. one to five years after the outbreak of the disease, the injuries to the nervous system were more frequently manifest than in patients during the acute phase. The authors pointed out that 25.3 per cent of those who recovered from tick-borne encephalitis, suffered to a varying degree from a limited physical and psychic efficiency. This condition originated from the existing injuries to the nervous system and led not infrequently to a permanent disability of the persons concerned.

[Neuroborreliosis: a psychiatric problem?].

The authors review the literature concerning clinical characteristics of neuroborreliosis with special focus on mental disorders. The patients develop mental disorders during the acute phase of the disease and even after several years. These disorders are often accompanied with various neurological syndromes. The authors discuss the most common mental disorders connected with neuroborreliosis: encephalopathy, cognitive disorders, dementia, depression, anxiety disorders. The paper describes the problem of neuroborreliosis in psychiatry, especially in the endemic regions.

[Evaluation of preventive vaccination against rabies performed at the Department of Parasitic and Occupational Diseases in Białystok in 1989-1990].

Between 1989-1990, 502 individuals exposed to the animals with rabies or suspected of rabies reported to out-patient clinic of agricultural occupational diseases at the K. Dłuski Hospital and to the Department of Parasitic and Occupational Diseases in the Medical Academy in Białystok. Sixty one (12.2%) of these individuals were vaccinated. Most frequently these people were attacked by dogs (70.4%), cats (11.4%), rats (6.5%), and foxes (3.2%). Rabies was diagnosed only in 9.8% of cases (Negri bodies in the brain and positive immunofluorescence test.). A vaccine inactivated on the human diploid tissue (Institute Merieux) was applied without any postvaccination complications. Transient micro-macular rash on face and chest of 3 patients, and local inflammatory reaction in 2 patients did not disturb vaccination process. Active-passive immunization was used in case of extensive lesions.

[Diagnostic difficulties in tuberculous meningoencephalitis].

Five cases of tuberculous meningoencephalitis with severe course treated in the period 1991-1994 at the Department of Parasitic Diseases and Neuroinfections, Medical Academy in Białystok are reported. Diagnostic difficulties encountered in these cases, the clinical courses and the results of the treatment are discussed. The always lethal outcome of untreated tuberculous meningoencephalitis requires antituberculosis treatment to be tried in all cases of meningitis with severe and protracted course, with equivocal results of cerebrospinal fluid analysis, and lacking effect of non-specific therapy.

[Mental disorders in patients after tick-borne encephalitis (TBE)].

There were 58 patients (aged 17 to 72) analyzed in our study: 34 women and 24 men one year after TBE. Psychic state was estimated with the use of psycho-pathologic scales CPRS, Hamilton Depression Rating Scale and Hamilton Anxiety Rating Scale, Mini Mental State and our own questionnaire. It was assumed that 22 patients required psychiatric treatment because of dementia, personality change, depressive disorder and anxiety disorder. It seems that so many syndromes stated in this group of patients were caused by TBE reaction increase of intellectual-mnestic functions as well as social and professional status.
