DREZ surgery on conus medullaris (after failed implantation of vascular omental graft) for treating chronic pain due to spine (gunshot) injuries.

The results of DREZlesioning procedure used for the treatment of chronic intractable pain due to deafferentation caused by gunshot injuries at the thoracolumbar (T10-L1) spine level are reported in six patients. The specificity of these cases arises from the fact that all the patients underwent, after decompressive laminectomy, an implantation of vascularized omental graft on the injured cord segments, 4-17 months after injury. Because of the failure of this method, which did not improve spinal function nor hinder the development of pain, surgery in the DREZ was performed 2-5 years after implantation. The results of the microsurgical DREZotomy procedure in those patients, 7-12 months after the surgery were: 4 patients with complete pain relief and 2 patients with pain relieved of 80%. All the patients with well-confined segmental pain were completely cured.

New technique for artificial lung maturation. Direct intramuscular fetal corticosteroid therapy.

The aim of this study was to present a new technique of administration of antenatal corticosteroid therapy in order to cause fetal lung maturation. A single dexamethasone dose of 4 mg was applied directly to the fetal gluteal musculature by ultrasound-guided intramuscular injection 48 h before delivery. This technique of fetal corticosteroid therapy was applied in six cases. Our patients had high risk pregnancies (preeclampsia diabetes mellitus, intracranial hemorrhage, epilepsy, hyperthyreosis). The pregnancies were terminated in the mother's vital interest. The lecithin/sphyngomyelin (US) ratio was < 1.5:1. There were no procedure-related complications. The fetuses were delivered by cesarean, 48 hours later except for the vaginal delivery in the patient in which fetal death occurred in utero. In five cases an uneventful outcome of fetuses indicated that direct fetal corticosteroid treatment improved postnatal lung function in preterm fetuses. A new technique of corticosteroid application successfully prevents respiratory distress in preterm infants decreasing the risk of maternal complications. To our knowledge, this is the first report of fetal intramuscular corticosteroid therapy in the human population.

The changing pattern of renal tuberculosis.

The authors present a series of 2,173 patients with renal tuberculosis seen between 1950 and 1979. This is group 1, serving only to show the incidence of renal tuberculosis during the antibiotic era. The fact is that the incidence of renal tuberculosis is going down slightly but constantly in the last years because the resistance of patients is increasing and the virulence of the TB bacillus is decreasing. The second group consists of 128 patients completely examined and the conclusion is that certain forms of very active TB have disappeared; high fever, very active forms of renal TB with deep and limited ulcerations in the kidney do not occur anymore. The third group is formed of 208 patients reviewed in order to demonstrate the residues after long antituberculotic treatment. It has been found that certain residual changes in the parenchyma are minimal but present as the remnants of antibiotic treatment indicating that probably the bilateral forms of renal TB are more common than we supposed before, in full agreement with Cibert's claim.

Family history of cancer among cases of upper urothelial tumours in a Balkan nephropathy area.

The highest relative risk of developing tumours of the renal pelvis and ureters (TPU) in a general population is confined to areas where Balkan nephropathy (BN) is endemic. Whether there is a higher frequency of cancer of any site in families with TPU cases from this BN endemic part of the world was tested by a case-referent study. A group of 65 rural patients with histologically proven TPU was individually matched with the same number of their nearest neighbours, who served as controls. Comparison of age-standardized ratios revealed that over a period of 30 years family members of the cases experienced a significantly higher risk of (non-urinary) cancer deaths than family members of their pairmates.

Steroid receptors in human uterine carcinoma. Their biological importance and therapeutic implications.

Surgical material of primary uterine carcinoma from postmenopausal patients was analyzed with regard to cytosol content of estradiol (ER) and progesterone (PR) receptors, as well as their binding sites in nuclear compartment. A dextrancoated charcoal technique was used and binding data were calculated from Scatchard plots. In all examined specimens intracellular high-affinity, low-capacity estrogen - and progesterone-binding proteins (which have the characteristics of steroid receptors), with equilibrium dissociation constants (Kd's) of 10(-10)M and 10(-9)M respectively, have been observed in detectable levels. DNA binding ability of steroid-receptor complexes was examined and clearly demonstrated even at 4 degrees C by using DNA-cellulose slurry. Preheating of complexes at 25 degrees cause increased DNA binding ability in some specimens, but significant loss of this ability in other tumors. The relationship between the nuclear retention of steroid-receptor complexes and their interaction with nuclear components (giving a response) is discussed on the basis of steroid-responsive and unresponsive systems. Present investigation indicate that described steroid analysis may become an important tool in predicting the value of endocrine therapy in human uterine carcinoma.

The value of tumor tissue penetration into the renal veins and lymph nodes as anatomical classification and kidney tumor prognostic parameters.

Renal vein invastion (great renal veins and vena cava) does not give such a bad prognosis in renal neoplasia. We could even say that this fact is relatively irrelevant for the prognosis but nevertheless signifies an extracapsular stage of renal tumor with the destiny under question. Lymph node invasion cannot be satisfactorily classified only as N+ and NO as proposed by UICC. The author proposed the classification from N1 to N4 as we must distinguish some stages in lymph node invasion of a renal neoplasm. The very distinct difference is if we have a case with invasion of only some nodes (1--5 in number) or we have a very large invasion making a pack around the great vessels (cava and aorta). In the first group we can have a fairly good prognosis (37% surviving 5 years or more), but in the second group the survival is very low (2%).

[Conservative surgery of renal pelvic tumors].

In a large series of renal pelvic and ureteral tumors in Belgrade (Jugoslavia), 26 cases of renal pelvic tumors were operated with conservation of the kidney. The indications must be strictly observed, the possibility of a relapse being maximally excluded. The results, under strict indications, are quite well: from 26 operated cases the 12 from 16 followed longer that 5 years are alive and with good kidney function plus 1 with lost kidney. So the 74% success for 5 or more years completely justifies the conservation of the kidney. In other cases the nephroureterectomy must be practised. The author did in 188 cases.
