In this article, two clinical nurse researchers describe experiences with integrating a program of nursing research, including the use of the findings and methods of research and the conducting and disseminating of research, into two acute care hospitals. Strategic and contextual factors are specified in relation to targets within and outside of the hospitals.
The literature on family caregiving has focused primarily on caregiver burden and demographic characteristics of caregiver groups. Little has been written about the existential aspects of caring for an aging relative. The article focuses on the meaning of the experience as expressed by family caregivers to elderly relatives and the role this meaning plays in enabling the family member to remain in the caregiving role. The caregiving passage includes families facing loss, identifying turning points, rediscovering a sense of self, and experiencing feelings of satisfaction and enhanced self-worth.
Primary care of patients infected with human immunodeficiency virus (HIV) presents major challenges for the nurse practitioner. HIV disease is a chronic illness requiring frequent visits to the primary care provider and a treatment regimen that is complex and often poses difficulty in patient adherence. A patient satisfaction survey designed to include aspects of care specific to HIV was given to HIV-positive individuals presenting for care at an urban medical teaching clinic. Fifty-two patients with HIV/acquired immunodeficiency syndrome completed the patient satisfaction survey between February and May 1994. Overall satisfaction with patient care was high. When nurse practitioners were compared with physician providers, however, nurse practitioners fared more favorably in the areas relating to clinic waiting time, provider knowledge about the disease, continuity of care, and patient education.
