Further methodological studies on the intravenous fat tolerance with Intralipid emulsion.

Factors affecting the reproducibility of the intravenous fat tolerance test (IVFTT) with the Intralipid emulsion have been studied in man. Temperature variations of the emulsion at the time of injection did not affect the fractional removal rate k2. Plasma samples could be treated at room temperature without affecting the k2 value. Linear k2 slopes were obtained also in pronounced hypertriglyceridaemia, which suggests that the test can be applied in subjects with wide ranges of serum triglyceride (TG) concentrations. In subjects with a wide range of serum lipids but at steady state with regard to lipid metabolism, the IVFTT was found to be highly reproducible when repeated with intervals of up to 6 months. Furthermore, the turnover of serum TG, determined as the product of the fractional removal rate and the serum TG concentration, was constant. Most Intralipid batches gave identical k2 results. In some comparisons, batches were found which gave k2 curves significantly different from the standard batches. It is recommended that the IVFTT is carried out only with Intralipid emulsions that have been standardized in this respect.

Effect of gemfibrozil on lipoprotein concentrations in different types of hyperlipoproteinaemia.

The lipoprotein lowering effect of gemfibrozil, 400 mg twice daily, was studied for 24 weeks in 26 subjects with primary hyperlipoproteinaemia (type IIa, 5; IIb, 3; III, 3; and IV, 15). Maximum effects were noted after four weeks on very low density lipoproteins (VLDL) and after 12 weeks on low density lipoproteins (LDL), these effects remaining throughout the study. Gemfibrozil decreased VLDL triglycerides (TG) in all types of hyperlipoproteinaemia according to the equation: deltaVLDL TG (mmol/l) = -0.80 X initial VLDL TG concentration + 0.68 The drug affected the low density lipoprotein (LDL) cholesterol= -0.45 X initial LDL cholesterol + 61 This means that LDL cholesterol on average decreased if the initial LDL concentration was above 135 mg/100 ml and increased below that concentration. LDL TG and high density lipoprotein (HDL) TG concentrations also decreased significantly. A slight increase in HDL cholesterol concentration occurred after administration of gemfibrozil. Two subjects had to stop treatment because of nausea and abdominal pain respectively, attributed to the drug. No biochemical side effects were noted.

Removal of exogenous triglycerides in human forearm muscle and subcutaneous tissue.

The removal of exogenous triglyceride (TG) in forearm muscle and subcutaneous tissue of 7 healthy male volunteers has been studied by nephelometric determinations of arterial-deep venous (a-dv) and arterial-superficial venous (a-sv) differences in concentration of fat particles. Exogenous TG was administered as a constant i.v. infusion of Intralipid over a period of 30 min at reat and another 15 min during forearm work. At rest a significant positive a-dv difference in fat particle concentration of 121 plus pr minus 21 mumol TG/l (mean plus or minus S.E.M.) was found, which correspondens to a fractional extraction of 1093 plus or minus 2.0%. Also the a-sv difference was significant, 81 plus or minus 14 mumol TG/l, 5.7 plus or minus 0.8% of the arterial concentration. During exercise no further significant removal was found. Thus both skeletal muscle and subcutaneous tissue seem to be able to remove exogenous TG in substantial amounts under resting conditions. In the exercising muscle, however, the direct removal of exogenous TG does not seem to be of significant importance.

Cholestyramine, clofibrate and nicotinic acid as single or combined treatment of type IIa and IIb hyperlipoproteinaemia.

1. In type IIa and IIb hyperlipoproteinaemia, treatment with 16 g cholestyramine daily reduced the cholesterol concentration 23%. By adding clofibrate this effect was enhanced to a 29% reduction and at the same time clofibrate reduced the triglyceride (TG) concentration 33%. 2. The optimal cholesterol reduction with clofibrate in type II was 17% and was found after treatment with 1.5 g clofibrate/day. The optimal TG reduction appeared to be achieved with a daily dose of 2 g.3. Nicotinic acid in the form of niceritrol had another type of dose-response in type II with doses from 3 to 6 g/day. It appeared as if the optimal dose probably was above 6 g daily. The 6 g dose produced a cholesterol reduction of 22% and in type IIb there was at the same time a 50% reduction of the TG concentration. 4. By combining 3 g niceritrol with 2 g clofibrate almost the same effect on serum lipids was obtained as with 6 g niceritrol. When choosing a drug for treatment of hyperlipoproteinaemia it is necessary to consider not only the lipid lowering effect but also the side effects which are not discussed here. By combining clofibrate with either cholestyramine or niceritrol it was possible to improve the lipid lowering effect. There were no side effects which were not seen when the drugs were used alone. A more frequent use of combinations to improve the treatment of hyperlipoproteinaemia is recommended.
