OBJECTIVE: Previous studies have shown that women receive fewer invasive procedures for the treatment of coronary artery disease than men, but gender differences in cerebrovascular disease have not been well studied. Our objective was to explore differences in the treatment of stroke between men and women. DESIGN: Secondary database analyses. STUDY PARTICIPANTS: We examined the use of carotid endarterectomy in a nationally representative sample of Medicare enrollees aged 65 to 84, hospitalized for a principal diagnosis of stroke in 1992, the "all strokes group". We also studied a subgroup of patients, the "carotid disease subgroup", admitted with a principal diagnosis of precerebral arterial occlusion and stenosis or transient cerebral ischemia. MEASUREMENTS: We determined rates of carotid endarterectomy for the all strokes group within gender and age groups and calculated corresponding female-to-male relative risks (RR) and 95% confidence intervals (CI). We also performed similar analyses for the carotid disease subgroup. We then used logistic regression to estimate the relative risk of use of carotid endarterectomy for women, controlling for age and comorbidity. RESULTS: The all-strokes group consisted of 3,356 women and 2,927 men, of whom 1,009 women and 990 men were in the carotid disease subgroup. The overall age-adjusted female-to-male relative risk of undergoing carotid endarterectomy for those aged 65 to 84 was 0.69 (95% CI, 0.64-0.74) in the all strokes group and 0.77 (95% CI, 0.72-0.82) in the carotid disease subgroup. In both analyses, the RR became more pronounced with increasing age. In the all strokes group, for example, the RR was 0.80 (0.70-0.92) for those aged 65 to 69 and 0.39 (0.32-0.48) for those aged 80 to 84. The RR for the all strokes group remained similar in magnitude even after controlling for comorbidity (RR, 0.63 and 95% CI, 0.59-0.70). CONCLUSION: We conclude that women hospitalized for strokes undergo fewer carotid endarterectomies than men. Further studies are needed to examine the reasons for and implications of this gender difference.
To date only about 15 abundantly expressed osmoresponsive genes/proteins have been described in rice cultivars. Using in vivo radiolabeling followed by two-dimensional electrophoresis and autoradiography, a record number of at least 35 salinity stress-induced polypeptides (14-90 kDa) and 17 salt stress-repressed polypeptides were detected in a halotolerant Indica rice cultivar Bura Rata. These include > 20 hitherto unreported rice polypeptides that exhibited a low abundance, short-term expression during NaCl stress. Prolonged exposure to NaCl decreased their synthesis. These findings have widened the scope of further investigations into new osmoresponsive genes, especially those with possibly transient regulatory functions in rice halotolerance.
We describe the case of a 56-year-old woman with terminal metastatic breast cancer who had delirium in the form of frightening hallucinations, paranoid delusions, and nightmares resulting in violent agitation. During this period, her bone pains from metastases were well controlled with narcotic analgesics, but her delirium proved refractory to standard doses of drugs such as lorazepam, diazepam, and haloperidol. We report the use of a subcutaneous infusion of midazolam at home and its effectiveness in control of her delirium after other drugs had failed.
A 48-year-old woman with an asymptomatic lung mass on a routine chest roentgenogram was found to have a primary non-Hodgkin's lymphoma involving the lung. Extensive evaluation revealed no evidence of lymphoma at any other site. She required an open lung biopsy for definitive diagnosis and was found to have unresectable tumor at thoracotomy. The patient achieved a complete remission with combination chemotherapy and remains in clinical remission 14 months after completion of therapy.

The patterns of molybdenum (MoO(4) (2-)) absorption and transport were investigated in intact bean (Phaseolus vulgaris L.) and rice (Oryza sativa L. cv. I.R.8) plants. The mobility of MoO(4) (2-) absorbed by roots and by leaves was compared with that of a freely mobile element, Rb(+). Although MoO(4) (2-) absorption by bean roots was nearly as high as that of Rb(+), its transport to the shoot was considerably less. When MoO(4) (2-) was fed to one of the primary leaves, most of it was transported to the stem and root. Evidence obtained here showed that MoO(4) (2-) was mobile. Experiments with intact rice seedlings revealed large differences in the absorption and transport of MoO(4) (2-) between the plants grown in CaSO(4) and those in Hoagland solution. Molybdate uptake by excised rice roots was suggested to be an active process since it was greatly inhibited by a metabolic inhibitor. The presence of Mn(2+), Zn(2+), Cu(2+), CI(-), or SO(4) (2-) in the absorption medium reduced MoO(4) (2-) uptake which was markedly enhanced by the presence of Fe(2+).
A case of Klebsiella pneumoniae osteomyelitis in a patient with sickle cell anemia is reported. The literature is briefly reviewed. The case is presented because of the rarity of Klebsiella pneumonia osteomyelitis in sickle cell disease.
