Cloning and expression of a 5-hydroxytryptamine7 receptor positively coupled to adenylyl cyclase.

A cDNA clone (designated as GP2-7) encoding a novel 5-hydroxytryptamine (5-HT) receptor was isolated from a guinea pig hippocampal library. The receptor shares amino acid homology within the hydrophobic domains with other cloned 5-HT receptor subtypes (34-48%). The sequence of GP2-7 is homologous to that described for a novel receptor previously cloned from a rat brain cDNA library and provisionally designated as 5-HT7. mRNA for GP2-7 was detected in cortical and limbic brain regions. Transiently expressed GP2-7 showed high-affinity binding to [3H]5-HT (pKi = 9.0) with the following rank order of affinities: 5-carboxyamidotryptamine (5-CT) > 5-HT = 5-methoxytryptamine (5-MeOT) > methiothepin > 8-hydroxy-2-(dipropylamino)tetralin (8-OH-DPAT) > spiperone >> sumatriptan. Adenylyl cyclase activity in CHO-K1 cells transiently transfected with GP2-7 was stimulated by several analogues of 5-HT with the following order of potency: 5-CT > 5-HT = 5-MeOT > dipropyl-5-CT > 8-OH-DPAT. Methiothepin and spiperone were potent antagonists. Preliminary analysis suggests that GP2-7 closely resembles a receptor in the guinea pig hippocampus that exhibits a high affinity toward 5-CT.

The measurement of collagenase, tissue inhibitor of metalloproteinases (TIMP), and collagenase-TIMP complex in synovial fluids from patients with osteoarthritis and rheumatoid arthritis.

OBJECTIVE: To compare the levels of collagenase, tissue inhibitor of metalloproteinases (TIMP), and collagenase-TIMP complex in synovial fluid (SF) from patients with rheumatoid arthritis (RA) and patients with osteoarthritis (OA). This study aims to clarify existing data from previously used enzyme or inhibitor activity assays performed following separation by gel filtration, by using a 1-step enzyme-linked immunosorbent assay (ELISA) for each component. METHODS: Total collagenase, free TIMP, and collagenase-TIMP complex were measured using a newly developed, specific double-antibody sandwich ELISA: RESULTS: Levels of both collagenase and TIMP were significantly higher in RA patients (collagenase 1,560 +/- 150 ng/ml [mean +/- SEM], TIMP 1,610 +/- 130 ng/ml; n = 80) than OA patients (collagenase 420 +/- 90 ng/ml, TIMP 1,050 +/- 60 ng/ml; n = 80), with the difference being especially striking for collagenase. Sixteen RA fluids had detectable levels of collagenase-TIMP complex, compared with only 3 OA fluids. CONCLUSION: The level of total collagenase in SF is greater in RA than OA, while levels of free TIMP show more overlap between the 2 diseases; this may simply reflect the increased inflammation seen in RA, or it may reflect a different disease mechanism.

Response of Holstein cows to corn gluten meal used to increase undegradable protein in early or later lactation.

Supplemental corn gluten meal was used to raise CP by 1.1 to 1.5 percentage units and undegradable intake protein from 35 to 39% of CP in the corn-based diet of parity 1 or greater Holstein cows to study effects of undegradability, parity, stage of lactation, and interactions on DMI, milk yield and composition, BW, and related traits during complete lactations. Cows were assigned at calving to treatments (n = 30, 8 primiparous): control, supplement wk 1 to 8 postpartum (early), or supplement wk 9 to 44 postpartum (late). Total lactation means were not affected significantly by treatments. Supplementation with undegradable protein enhanced forage and, thus, total DMI in later lactation by pluriparous cows; it apparently spared BW loss wk 1 to 8 postpartum and enhanced BW recovery thereafter in first lactation cows with no effect in older cows. Effects of supplementation on milk yield were small, and they were negative in early lactation and generally positive in late lactation; effects were positive on fat test in early lactation for both parity categories but distinctly negative for parity 1 cows in late lactation. Supplementation of undegradable protein in late lactation also decreased milk protein content in parity 1 cows and raised it in older cows. Data suggest that Lys may have been first-limiting, followed by Ile in early lactation and Met in late lactation, and that AA adequacy may be more important than undegradability in ration protein balancing. For most traits measured, treatment by parity interactions were significant, indicating that parity 1 cows did not respond in the same way as older ones to protein supplementation.

Hepatic transplants. Liver transplantation: an OR nurse's perspective.

1. Advanced surgical technique, improved immunosuppressive drug therapy, and an increased public awareness of the need for donor organs has resulted in liver transplantation being recognized as an available option for the treatment of end-stage liver disease. 2. The evaluation process is extensive. Medical and surgical assessments are done through a complete history and physical examination; diagnostic testing ranges from blood typing and serologies to radiologic and consultant procedures; and psychological and psychosocial evaluations are done for the patient and family. 3. Ongoing research in organ preservation, immunosuppressive drug therapy, and surgical technique has helped to make liver transplantation programs successful.

Health visiting on a playbus: a community approach.

Inappropriate and overcrowded housing, poverty and lack of social support can affect the health and development of young children and their mothers. Such factors may prevent those families who most need health resources from using them. Rosalind Edwards and Stephanie Ramsey describe a project that attempts to encourage the use of health-related information by families with young children through taking an inter-agency and community-based approach.

Salmonella enteritidis causing joint sepsis.

A 22-year-old male taking dexamethasone following resection of a medulloblastoma developed an acutely painful swollen knee from which salmonella enteritidis was cultured. He had no gastrointestinal symptoms; one stool culture was positive. Active metalloproteinases without inhibitors were detected in the synovial fluid, a characteristic finding in septic joints. S. enteritidis infecting joints is extremely rare despite being the second most frequent salmonella species after S. typhimurium causing infections in man. The recent findings of S. enteritidis in eggs, and poultry points to one more source of infection. Immunosuppression is likely to facilitate infection.

The levels of collagenase, tissue inhibitor of metalloproteinases-1 (TIMP-1), collagenase approximately TIMP-1 complexes and glycosaminoglycan (GAG) in sequential samples of synovial fluid aspirated from patients with osteoarthritis.

OBJECTIVE: Collagen turnover in connective tissues is thought to be controlled by the balance between the levels of interstitial collagenase and tissue inhibitor of metalloproteinases (TIMP-1). The aim of this study was to measure the level of total collagenase (MMP-1), TIMP-1, collagenase approximately TIMP-1 complex and glycosaminoglycan (GAG) in sequential samples of osteoarthritic knee synovial fluid from well documented patients to determine if these parameters changed with time and correlated with clinical indices. METHODS: Twenty-one patients were recruited and randomly allocated to receive tiaprofenic acid, indomethacin or naproxen. Total collagenase, TIMP-1, collagenase approximately TIMP-1 complex and GAG were measured in 80 osteoarthritic synovial fluids taken over a period of six months. RESULTS: The majority of fluids contained a molar excess of TIMP-1 over collagenase, although in seven fluids collagenase was present in excess; six of these samples were from a single patient. GAG levels were relatively unchanged over the six months studied. CONCLUSION: The levels of collagenase and TIMP-1 varied between patients and over time in individual patients. No collagenase approximately TIMP-1 complex was found in any fluid. There was no significant difference in the median levels of collagenase, TIMP-1 or GAG in the different treatment groups. High levels of collagenase were found in one patient with a crystal related disease. These immunoassays give valuable information on the levels of collagenase and TIMP-1 in individual patients with time and may help to determine the mechanisms controlling the turnover of cartilage collagen in different arthritides.
