[Cesarean section for the second twin].

Twin pregnancies delivered after 26 weeks gestation were reviewed. Of 167 twin pregnancies 101 (60.5%) were delivered by cesarean section. There were 8 combined vaginal-abdominal deliveries, including 4.8% of all twins delivered and 7.3% of all twins delivered by cesarean section. The commonest indication for the cesarean birth of the second twin was transverse lie. The data suggest that cesarean section of the second twin is not possible to predict by obstetrics variables including antepartum ultrasound. In order to decrease the rate of combined vaginal-abdominal deliveries the need to reassess the currently protocol of management for twin delivery is indicated.

[Hematologic parameters in the cord blood labor complicated by meconium-stained amniotic fluid].

The aim of our study was to compare hematological values of the cord blood in the presence and absence of meconium-stained amniotic fluid (MSAF). MSAF was associated with decreased total platelet count, increased values of platelet distribution width and red blood cell distribution width. The mean nucleated red blood cells was significantly higher in meconium group. The values of red blood cells, white blood cells, reticulocytes counts and their fractions (HFR,MFR,LFR,IFR) were no different in cord blood of neonate exposed to meconium and in those who were not. These findings can suggest that MSAF contribute to fetal infection rather than hypoxia.

[Reinsemination of oocytes by intracytoplasmic sperm injection after the failure of conventional fertilization. Early embryonal development].

OBJECTIVE: To analyze the influence of intracytoplasmic sperm injection (ICSI) on early embryo development used for reinsemination of unfertilized 1-day-old oocytes. DESIGN: Prospective observational study. MATERIALS AND METHODS: A total of 126 embryos resulted from reinsemination by ICSI were analyzed with regard for the time course of cleavage and the quality of embryos at 48 hours after injection. Results were compared with those obtained in 698 embryos obtained after routine ICSI program. RESULTS: It has been shown that embryos derived from reinsemination developed slower in comparison with those obtained after regular ICSI program. At 48 hours after microinjection 50% of embryos achieved 4 blastomeres stage and 37% remained on 2 cells stage. 71% of regular ICSI embryos at this same time showed 4 blastomere or more, only 16% remained on 2 blastomeres stage. The quality of embryos was similar in two compared groups, however those obtained following reinsemination at 4 blastomere stage were significantly poorer quality. CONCLUSION: Embryos derived from reinsemination of 1-day old oocytes unfertilized after conventional IVF developed slower and the quality of embryos at 4 blastomere stage was poorer comparing to regular ICSI embryos.

[Prolonged desensitization pretreatment for in vitro fertilization in women with polycystic ovary].

OBJECTIVE: To analyse the outcome of intracytoplasmic sperm injection (ICSI) in women with severe polycystic ovary disease (PCD), stimulated after prolonged pituitary desensitization in comparison with regular long protocol therapy. MATERIALS AND METHODS: The results of controlled ovarian hyperstimulation (COH), fertilization rate, early embryo development parameters and clinical pregnancy rates were compared depending on the duration of pituitary desensitization before gonadotropin administration. A total of 60 patients with severe PCOD were desensitized with GnRH agonist up to 30 days before COH. The results were compared with the group of 213 patients with mild PCOD treated with regular long protocol (control). RESULTS: We observed a slightly higher rate of mature oocytes MII as well as the number of embryos obtained in the control group, however the pregnancy rate in the group with prolonged desensitization was significantly higher (50%) in comparison to the control group (33.8%). CONCLUSION: The prolonged pituitary desensitization before controlled ovarian hyperstimulation gives better chance for obtaining pregnancy in patients with severe PCOD. It could be recommended facing previous treatment failure.

[Acute fatty liver of pregnancy: underestimated complication of the third trimester?].

Two cases of acute fatty liver of pregnancy (AFLP) are described. The liver function abnormalities have been ignored in the both cases. The review literature on this rare clinical entity unique to pregnancy is presented. The importance of clinical and biochemical findings in the diagnosis of disease is emphasized. Although etiology is unknown, deficiency of fatty acid beta-oxidation has been suggested.

[Acute fatty liver in pregnancy: treatment, prognosis, rules of management].

Acute fatty liver of pregnancy (AFLP), rare and potentially fatal disease, is considered as obstetric emergency. The perinatologist must be familiar with this complication of pregnancy because early diagnosis, prompt delivery and supportive care is the mainstay of therapy. When pregnant woman has nausea or vomiting, abdominal pain and jaundice, increased transaminase activity, coagulopathy and hypoglycemia during third trimester AFLP should be suspected. Mothers who have experienced AFLP must be informed of the possibility recurrence and undergo close surveillance in the next pregnancy.

[Antepartum amniotic fluid index and pregnancy outcome].

Our purpose was to evaluate prognostic value of antepartum amniotic fluid index (AFI) on fetal and newborn outcome, because oligohydramnion is associated with increased risk of fetal and newborn distress. Perinatal outcome was defined by 1' and 5' Apgar score, intrapartum fetal heart rate (FHR), blood gas, acid-base status in umbilical artery, mode of delivery and meconium-stained amniotic fluid. Data was analysed with CSS Statistica for Windows package by test t for independent samples and chi 2. Prospective observations was conducted with 84 singleton pregnancies complicated by oligohydramnion (AFI < or = 5 cm). A 336 pregnant women, with normal amniotic fluid index and no complicated pregnancy, was considered as controls. No increased operative delivery and meconium-stained amniotic fluid rate was noted. There was no significant effects AFI on blood gases in umbilical artery and Apgar score. Oligohydramnion was associated with increased risk of fetal acidosis (pH < 7.16) (p = 0.009) and abnormalities of FHR (variable decelerations) (p = 0.004). Reduced AFI had poor predictive value of fetal acidosis and newborn depression positive predictive value (8-13%). Summarizing the results, our investigation showed that amniotic fluid index is only one point in the prognosis of perinatal outcome.

[Seasonal changes in donor's sperm motility: CASA parameters].

Computer assisted semen analysis was employed to find out whether there are some changes in movement parameters between autumn-winter and spring-summer time. 473 sperm specimens obtained from 12 sperm donors were analyzed. Statistically significant percentage of motile spermatozoa was higher during spring-summer time.
