[Variation in the lateral plate of the pterygoid process and the lateral subzygomatic approach to the mandibular nerve].

OBJECTIVES: The objective of this work is studying wariable anatomy formations at the posterior border of the pterygoid process lateral plate of the sphenoid hobe (lig. et foramen pterygospinale, lig. et foramen crotaphitico-buccinatorium), and its statistical processing considering sex, age and sides of studied sculls. MATERIAL AND METHODS: At 305 sculls of mature persons of both sexes (167 males and 138 females) we studied incompletely ossificated lig. pterygospinale, opening of the same name which has become by its full ossification, incompletely ossificated lig. crotaphitico-buccinatorium and opening made by it's complete ossification, and position of this structures in relation to foramen ovale. RESULTS: Incompletely ossificated lig. pterygospinale was found at 12 sculls on both sides or 3.93% (8 male's and 4 female's sculls). One-sidedly, on the right one, incompletely ossificated lig. pterygospinale was found at 14 sculls (4.59%), and on the left side at 19 sculls (6.22%). Incompletely ossificated lig. pterygospinale, present on both sides of sculls, was in the case of persons older than 50 (9 sculls or 75%). Pterygospinous opening which has become by complete ossification of lig. pterygospinale was found on both sides at 4 sculls (1.31%), two of them were male's and two of them female's sculls. In alll four cases persons were older than 40. Pterygospinous opening found on the left side was present at only 3 sculls (0.98%) and on the right side at 4 sculls (1.31%). Pterygospinous opening was present one-sidedly, every time, in the case of male's sculls. Incompletely ossificated lig. crotaphitico-buccinatorium, meaning incomplete opening, was found at 17 sculls on both sides (5.57%). 10 of them were male persons' sculls, and 7 of them females'. On the right side, incompletely ossificated lig. crotaphitico-buccinatorium was found at 11 sculls (3.60%)--6 male's and 5 female's, and on the left side at 16 sculls (5.24%). Foramen crotaphitico-buccinatorium was found on both sides only at 12 sculls (0.97%)--one male's and one female's. Foramen crotaphitico-buccinatorium was found on the left side at 8 sculls (3.90%) and, on the right side, this opening was found at 3 sculls (0.98%). Incompletely of completely ossificated lig. pterygospinale, located laterally from foramen ovale, was found at 5 sculls (5 male's and 1 female's) or 1.63%, and in the other case, incopletely or completely ossificated lig. crotaphitico-buccinatorium laterally from foramen ovale was found at 7 sculls (5 male's and 2 female's) or 2.29%. CONCLUSION: While applying conductive anaesthesia on mandibular nerve by lateral subzygomatic route, variable ossificated formations at lateral plate's posterior border of pterygoid process should be kept in mind. It is possible that a needle, at a depth of 35 mm, comes across incompletely or completely ossificated lig. pterygospinale et crotaphitico-bucciantorium. Based on ours anatomy research we concluded that these formations are more oftenly present on males aged over 50, and that one-sided presence is more often on the left side. Ossificated formations which were the object of our research are not an obstacle to high-quality applying of conductive anaesthesia on trigeminale ganglion et mandibular nerve each time when they exist, but only when they are located laterally from foramen ovale, what appeared at 12 sculls (3.93%) in our case.

[Types of changes in laboratory technics used in prosthetic technology].

Because of the phenomena which were noticed in clinical and laboratory work of permanent stomatological metallographical and other researches of the consequences of free changed conditions in melting and cooling phase of dental alloy auropal were carried out. Modality of the changes during melting of this alloy in dental laboratory were systematically chosen according to noticed phenomena in praxis. Main objective was to metallographicly determine these phenomena during the process. With application of usual methods for setting of metallographical carved pie, samples were researched with metallographical microscope Epiryp 2 from Zeiss-jena, under different melting conditions and filmed with different enlargement. Even small changes of melting and cooling conditions are causing in microstructure and these are reflecting on the alloy characteristics. According to this melting procedure is a very sensitive and complex problem in termical laboratory processing which demands maximum attention an strict following fabrical instructions in order to avoid unwanted consequences in clinical application.

[Indications for supragingival preparation of teeth for placement of fixed prostheses].

Supragingival preparation offers simple controllable preparational procedure without contact of boring machine with gingive. That is the way to avoid the initial lesion of marginal parodont. The exactivity of leaning of the crown edge is visible. The control of generation of a carious initial lesion is in domain of patient--the carer and doctor. The esthetic aspect of work is not endangered by placing the crown of teeth of molar and premolar regions. The position of the edge of a crown makes possible the complete control and carrying out the measures of oral hygiene. The usage of monocomponent esthetic resins can solve the question of esthetic moment in the area of cement fissure. Supragingival location of the crown edge makes possible the inspection control of preparational border, the edge closing, esthetics in the tooth area and initial carious lesions.

[Oral hygiene in the hadiths of the Holy Prophet, Mohammed S.A.V.S].

The problem of oral hygiene in contemporary dental medicine is the problem of successful maintainance of oral health. Contemporary dental medicine recommends the methods for maintaining of total oral hygiene, as preventive action against dental ills. Hadith of Holly Prophet Muhammad s.a.v.s. is a note about what he has done, recommended or amended. Holly Prophet Muhammad s.a.v.s. recommended entire hygiene of teeth, gums, through tooth brushing and washing mouths by water three times. Actually, this was the first time in the available literature that someone recommended maintaining of oral hygiene. It can be said without reserve that Muhammad s.a.v.s. has established a preventive dental medicine. Muslims are recommended to maintain hygiene through Holly Qor'an: "There are the people that like washing themselves in it (mosque), and Allah loves those, who clean themselves very much."

[The value of horizontal discrepancy on the subgingival position of the tooth crown].

Marginal adaptation of the crown edge has been considered as primary and significant factor of prevention of secondary caries and periodontal disease on carryig tooth. There has been a clear dependence between hunting the periodontal tissue and the quality of edge closing of the crown. If we position subgingivaly the crown which clinically shows a good adoptiveness marginally and at the same time we position marginal crease in the ginguival sucus that should rush the accumulation of the plaque. The bacteries in the plaque would cause the inflammation of ginguive. The end of the crown edge in gingival sucus would cause chronic inflammation at ginguival tissue. The existence of transit zone that includes the crown edge, prepared tooth and dental cement became important if the crown edge has been positioned subgingivaly. If the crown edge ends in the ginguival sucus, the tooth meat is constantly being irritated that results with different degrees of inflammations. In many cases it causes ginguival dislocation. The possibility of clinical control of marginal positioning of subgingivaly positioned crown edge on demarcation line does not exist in terminal phase of cementing. The crown cement can be substratum of bacterial receptiveness and the plaque accumulation in the ginguival sucus. The procedure of cementing is an important cause of incomplete edge closing of the crown on the demarcation line. The form of demarcation line determine the form and the width of crown. They are favorising and degrading the level of marginal adaptations. The existing of horizontal discrepancy at the relation of based tooth--the crown edge is unavoidable. The question is if the amount of this discrepancy fits the assumed biological optimum.

[Canine teeth as an experimental model for fixed stomatologic prosthesis].

Clinical researches, especially pathological-histological experimental researches in mouth of patient reach the level of impossible, from the ethical and law aspect. Trying to define particular state concerning relation of fixed protetical work in the mouth of patient to tooth caries and surrounding belonging tissues, there is a question whether it is necessary to establish milieu-in vivo--which follows scientifically established principles. These principles are necessary for converting results which would become valid in scientific defining of state in the mouth of patient, as well as on the tissues which we want to research clinically and experimentally. Experimental animals are the second choice for researching tissues of human beings. Dogs have many conveniences that classify them into most usually experimental animal.

[Occurrence and etiology of tooth caries in wearers of fixed dentures during the war].

Concerning the destruction, war and caries are complemental. They don't have the same final causes, but on the microplan, both of them have destruction as a consequence. War causes complete destruction of organism, and the crude killing of human being. The people who do not get killed are faced to all negative etiological factors which disturb general health of organism. We have been following the parameter for the need of intervention in the field of dental protetic, frequent erosive changes of dental tissues sediments of caries and etiological factors which facilitate commencement and development lesions of caries on the tooth-carier of fixed protectical work. During this work we have been analyzing the patients who have been treated at The Clinic for Dental Health during the war in Bosnia and Herzegovina. Etiological factors, that caused commencement and development lesions of caries on the tooth-carier of fixed protetical work, are identical to those that are described in accessible literature, but also there have been some specific features which have been seen during the war conditions, as follows: *inadequate nutriment-patients were usually eating adhesive food, noodis, *impossibility of maintaining and providing the extents of oral hygiene, difficult or impossible access to a hygiene knot, *the lack of equipment for maintaining the oral hygiene, toothpaste, toothbrush, water, *Fight for the bare life, war is time for survival and it cites patient to seiftuiage in care for their own health, patient fights for existence and treats the acute states. In this relation brushing the teeth comes to the last place, *war conditions unable patients in approaching to therapeuts, and they can not use stomatological equipment due to the lack of electric power and water. *time as a factor--war circumstances delayed the intervention in the mouth of patient at the right time. Percent of sediments of caries inclinical study on the tooth-carier fixed protetical work is at the high level of 65%. This percentage could be justified by a fact that all patients came with already developed lesions of caries. War is enemy of human health in every segment, so increasing number of lesions of caries is one of the logical consequences. War in BiH has presented that stomatological science and practice during war are just curative. Esthetic stomatology and right-time interventions, preventions don't exist. There is only necessary stomatology.

Oral health in SFR Yugoslavia in 1986.

A pilot oral health epidemiological survey using WHO assessment forms was conducted in Yugoslavia in the year 1986. The study population consisted of 2600 persons aged 6, 12, 15, 18, 35-44, and over 65 yr. The survey included 22 towns (11 developed and 11 underdeveloped) in the six Republics and two Provinces of Yugoslavia. The results showed the prevalence of dental caries in the Yugoslav population to be very high (98.7% in 12-yr-olds). The mean decayed, missing, and filled teeth (DMF) scores were as follows: 6.1 at age 12 yr, 9.6 at age 15, 10.9 at age 18, 18.0 at age 35-44, and 28.0 in persons aged over 65 yr. Assessment of the periodontal status showed calculus to be the predominant disorder in the age groups 18 and 35-44 yr, while loss of sextants prevailed in persons aged over 65 yr.
