Scleroplasty in progressive myopia--selection of materials.

Scleroplastic surgery is known to encounter the pathogenetical agent when the progressive myopia develops. In the first part, the implemented surgical approaches as well as up-to-date surgical techniques are presented. The experimental part is reserved for the selection of material which is necessary to perform scleroplasties. Both the allogenic and xenogenic grafts have been studied for histocompatibility on 20 rabbits. Following materials were used: 2 types of collagenic sponge, native and deproteinated human sclera, modified dura mater as well as both the native and devitalized sclera from rabbits. The best results were obtained with allogenic sclera, deproteinated xenogenic sclera and collagenic sponge. The application of other materials is discussed.

[Spectrum and dynamics of free amino acids in bovine ocular tissue and their interaction with pilocarpine. IV. The sclera].

Two-dimensional ascending chromatographic analysis revealed that a 3% aqueous pilocarpine solution dropped into the conjunctival sac of cattle 2.5 hours after the last instillation shifts in a specific qualitative and quantitative way the levels of different free amino acids in the anterior, equatorial and posterior portion of the sclera. This alkaloid in the anterior and posterior part reduces and in the equatorial part of the sclera increases the total levels of free amino acids. The greatest drop in the anterior portion occurred in leucine + isoleucine methionine + valine and glutamine, in the posterior part threonine and leucine + isoleucine. The greatest rise in the equatorial part was observed in glutamine, leucine + isoleucine and taurine.

Fibrinolytic activity of human brain.

Authors studied the fibrinolytic human brain activity using the fibrin film in order to determine the location of tissue plasminogen activators. An amount of 20 human brains has been evaluated issued from cadavers with no signs of cerebral disease when alive. Most high concentration of plasminogen activators was evidenced in samples from plexus chorioideus and pia mater, while the fibrinolytic activity of grey and white matter showed uniformly low values. Large amount of plasminogen activators may be released into cerebro-spinal fluid, what can lead to a rapid fibrin clot dissolution around the aneurysm, so the rebleed may be consequential to it.

[Long-term results after surgical treatment of head injuries].

It outflows from a survey of 174 cases operated during five years for skull injuries that the resulting state depends of preoperative level of consciousness. In injuries with the loss of consciousness, there is a close correlation between the type of motor-topain response and the resulting state. It was stated from the delay for up to 5 years after surgery that the independence from the day life has been achieved in 65% of injured patients which experienced preoperatively a mild loss of consciousness as they localized the pain stimulus or produced unintelligible sounds in response to pain. The similar condition has been achieved in only 14% cases of preoperatively deep loss of consciousness, which responded to pain or not with decerebrative extension, or unconsciously flexion of extremities. The resulting state is less dependent of operative diagnosis. Epidural hematomas show better results, where the independence of the day life has been achieved in 62% of cases. The majority of them was able to perform their previous working activities later on. In contrast, the worst results were achieved in association with brain contusions, where only 18% of injured patients sustained the independence of environs with no return to the previous work. Prognostically, the factor of age is the most important. In the age group over than 60 years, the mortality represented 58%, whereas in 20-40 years of age it was only 24%.

[Prostanoids and leukotrienes in ophthalmology. II. Role of prostanoids and leukotrienes in the physiology and pathophysiology of the eye].

The authors give an account on the development and contemporary knowledge regarding the participation of prostanoids and leukotrienes in physiological and pathophysiological processes of the eye. According to many data in the literature of all intraocular tissues of the human eye the greatest prostaglandin production was found in the anterior segment of the choroid. The authors describe the role played by these substances in the pathogenesis of inflammations, their importance in the regulation of intraocular pressure and in the development of cystoid macular oedema.

[Prostanoids and leukotrienes in ophthalmology. III. Results of a clinical study of local administration of indomethacin in preoperative and postoperative care].

The authors submit results of the clinical investigation involving indomethacin administration during preoperative and postoperative care of patients with primary glaucoma who were submitted to trabeculectomy. The investigation was made by the method of a double blind trial. Indomethacin 0.5% eye drops or placebo were administered on the day prior to operation to the 10th day after operation, three times a day. During the final comparison of the two groups after indomethacin a reduction of the number of postoperative complications was observed, as well as a shorter hospitalization period, and the result of the filtering operation was not affected.

[Microsurgery knives with crystalline blades].

In collaboration with the national enterprise Monokrystaly Turnov and the workshops of the Medical Faculty, Charles University, Hradec Králové the authors designed and produced microsurgical knives with an exchangeable inserted blade made from leucosapphire. They describe their experience with the manufacture and clinical use of these new local instruments.

[The spectrum and dynamics of free amino acids in the tissues of the bovine eye and their interaction with pilocarpine. III. The lens].

The authors revealed by chromatographic analysis that in the investigated tissues of the bovine eye a 3% aqueous pilocarpine solution 2.5 hours after the last instillations alters specifically the levels of different free amino acids. In the capsule and anterior coat an increase was observed and in the nucleus and posterior coat a decline of the total amount of these components. In the control and pilocarpine-treated capsule and in the lenticular nucleus glutamic acid, is present in the highest concentration and in the anterior and posterior coat of the lens aspartic acid and lysine.

[Permanent manual magnets in microsurgery of the eye].

Intraocular magnetic foreign bodies represent still an important problem. Giant electromagnets make possible to extract the foreign magnetic bodies in nearly all cases. The hand-manipulated electromagnets are advantageous for their easier use. The miniature permanent magnets equipped with shaped extensions are constructed for purposes of the ocular microsurgery. Authors have designed, in cooperation with the domestic technical institutes and manufactures, the original type of the permanent magnet equipped with needed extensions and experimentally verified its usage during microsurgical extractions of the magnetic intraocular bodies.

[The spectrum and dynamics of free amino acids in bovine ocular tissue and their interactions with pilocarpine. II. The aqueous humor, choroid epithelium and the choroid].

The authors revealed by two-dimensional ascendent chromatography on paper and thin-layer chromatography that a 3% aqueous solution of pilocarpine acts specifically on the free amino acids in the choroid, in the choroid epithelium and the aqueous humour of the bovine eye and that it reduces the total free amino acid level more in the choroid epithelium than in the aqueous humour. Specific changes of free amino acid levels caused by pilocarpine indicate the routes of transport, distribution and metabolic changes of pilocarpine and free amino acids. Based on these changes it is possible to evaluate also the physiological and morphological condition of ocular tissues and the organ of vision resp.
