Serum, saliva, and sputum levels of metronidazole in acute exacerbations of chronic bronchitis.

We have evaluated the absorption and the penetration of metronidazole into the bronchial secretions and saliva in acute infective exacerbations of chronic bronchitis. Seventeen patients were given 400 mg orally three times daily for seven days and "steady state" levels were measured in serum, saliva, and sputum on the last day of treatment. Mean levels in the three biological fluids were not significantly different. Higher metronidazole levels in sputum tended to occur in patients with higher serum levels. In all but one patient, levels in serum and saliva were well within the therapeutic range. We conclude that this oral regimen results in therapeutic tissue levels in acute exacerbations of chronic bronchitis.

The carbon dioxide requirements of anaerobic bacteria.

Some clinically significant anaerobic bacteria were incubated in pure culture in anaerobic jars containing a range of atmospheric concentrations of CO2. Growth of Clostridium perfringens, C. sporogenes and C. septicum was independent of the amount of CO2. Small supplements of CO2 (0.25%) allowed good growth of the majority of anaerobes studied--an observation contrary to established teaching. The exceptions were Fusobacterium necrophorum which showed an absolute requirement for CO2 of at least 1% and B. melaninogenicus which needed an atmospheric content of 10--40% CO2 for optimal growth. The inclusion of CO2 in the anaerobic jar at a final concentration of 10% is to be recommended for all routine isolation procedures.

Enhanced DRL and impaired forced-choice alternation performance following hippocampal lesions in the pigeon.

In four experiments, pigeons with bilateral lesions of the hippocampus were compared to unoperated control birds and operated control subjects that received bilateral lesions of the neostriatum on tasks sensitive to hippocampal damage in mammals. Consistent with the mammalian literature, hippocampal birds were impaired on forced-choice alternation in the T-maze, although they were unimpaired on spatial alternation and reversal learning in an operant chamber. Furthermore, in contrast to hippocampal mammals, hippocampal pigeons showed enhanced performance on a differential reinforcement of low-rates of responding schedule of reinforcement. The results are discussed with respect to a behavioural homology between the avian and mammalian hippocampal formations.

Growth and nutrition in Rett syndrome.

PURPOSE/METHOD: In this paper we review the existing literature on factors that contribute to growth failure in Rett syndrome (RS) with particular emphasis on the extent and nature of the feeding difficulties that arise. Data on growth and feeding related problems, collected over four years in a specialized clinic for females with Rett syndrome are presented and management protocols developed in the clinic discussed. RESULTS/CONCLUSION: Feeding related problems and growth failure occur commonly in Rett syndrome yet our understanding of the mechanisms causing growth failure are poorly understood. Both nutritional and non-nutritional factors are thought to contribute and it has not been possible to develop efficacious intervention strategies. Consequently, clinical management of growth failure in Rett syndrome is not evidence based.

Behavioural and emotional features in Rett syndrome.

PURPOSE/METHOD: There is increasing agreement that many genetic disorders have characteristic behavioural phenotypes; that is genetic anomalies have specific effects on behaviour. In this paper the existing literature is reviewed with an aim to identify behavioural and emotional features that are candidates for Rett syndrome (RS) specific behaviours. RESULT/CONCLUSION: A number of behavioural and emotional features have been reported to be common in individuals with RS. These behaviours may constitute an RS-specific profile of behaviour or behavioural phenotype. Alternatively, these behaviours may simply reflect the multiple disabilities found in individuals with severe or profound cognitive impairment. The diagnostic criteria for Rett syndrome include a number of the behavioural features, such as hand stereotypies and breathing difficulties, although other behavioural features are not included.

Emotional responses of hospitalized patients with burns to debridement during the acute phase.

The objectives of this descriptive study were to investigate anxiety and depressed mood of patients with burns associated with debridement and their preferences for and perceptions of control during the debridement procedure. Measurements began within 48 hours of hospital admission, continuing every 3 days over 2 weeks for subjects with small burn injuries (less than 30% of total body surface area). Data suggested the possibility of a mildly depressed mood for some subjects during the first week in the hospital and high levels of association between depressed mood and anxiety based on the large number of significant correlations (p < .05). Over time, the amount of control that patients preferred and the amount of choice they perceived during debridement increased. These findings about responses of patients with burns to debridement suggest that their emotional responses were well managed for the most part and support their involvement in the debridement procedure.

Tooth eruption in failure-to-thrive infants.

The purpose of this study was to determine whether differences existed between the numbers of erupted teeth of infants who were failing to thrive and their controls. The results suggest a relationship does exist.
