[Corneal complications of climatic keratopathy].

The "keratopathie de Bietti" or climatic droplet keratopathy or keratopathie climatique (KC) is a degenerative disease of the cornea which is probably due to the exposure to ultraviolet rays. Usually the evolution is a progressive invasion of cornea by amber nodules which lift up the corneal epithelium. We observed that numerous patients suffered from severe corneal ulcerations which appeared to be spontaneous and which evolution was frequently disastrous. The aim of this work was to pick up epidemiologic data on KC in order to check the following hypothesis: the corneal opaquenesses, due to non traumatic perforations, would be more frequent among patients suffering from KC than among the rest of population. The study, carried out in Republic of Djibouti, dealed with two groups of people: an hospitable group of 217 patients suffering from a KC and a random sample of 2,446 persons among Djibouti people, from which a study ill-healthy was carried out among people from 40 years old. The mistakes were controled by adjustment on age and climatic conditions. The result is that corneal blindnesses which seemed to be spontaneous were more frequent in a significative way among people suffering from KC than among healthy people as old as ill people and living in the same conditions (12% against 0.7%; p = 0.003; Odds ratio adjusted on age: 31.8 IC: 8.8-115.20). On the etiopathogenic point of view the bacterial, fungal and deficiency causes could scarcely be accepted. The unfavourable evolution, resisting to all the treatments, drove to several hypothesis among them is responsibility of free amoebas as acanthamoeba.

[Epidemiological course of trachoma in Djibouti].

A survey on a representative sample of population showed that inflammatory trachoma is rare in Republic of Djibouti. However, it is significatively more frequent in rural surroundings (0.4%, standard rates) than in urban surroundings (0.4%, standard rates) where it affects particularly people recently coming from border countries. In rural surroundings the frequency changes according to the distances from capital (p = 0.01). On the contrary, cicatricial trachoma is more frequent in the city (16.4%) than in rural surroundings (8.2%--Standard rates). The blindnesses caused by trachoma are relatively rare (0.1%) when entropiontrichiasis appear especially in urban surroundings (0.7%) where people from Arab Peninsula are the most affected (p = 0.008). When comparing with the 1968 situation, we notice a perceptible decrease of the prevalence of trachoma and also the disappearing of ethnic differences which existed at that time concerning the illness active forms. This good evolution is probably due to the effects of several mass campaigns added to an improvement of accessibility to medical care.

[Epidemiology of Bietti's keratopathy. Study of risk factors in Central Africa (Chad)].

Climatic Droplet keratopathy (synonyms: spheroidal degeneration, Bietti's keratopathy) is a corneal degeneration characterized by an opacification at the level of the Bowman's membrane in the area of the palpebral fissure. It is particularly common in areas of bright sunlight and in Labrador; ultraviolet light thus being thought at its genesis. Two objectives were included in this study: studying the Climatic Droplet Keratopathy (CDK) prevalence by looking for the influence of the climatic factor; looking for possible association with three other degenerative ocular diseases: cataract, open-angle, glaucoma and exfoliation syndrome. A random cluster sampling survey was carried out in different climatic areas in Chad (N = 3,241 people). The resulting analysis showed a significant relation between the CDK prevalence and the kind of climate: the more the climate is dry; the more the higher the frequency: subdesert area = 1.57%, Sahelian area = 0.73%, tropical area = 0.18% (p less than 0.0001, rate standardized to age). The CDK occurs earlier in the subdesert area (mean age = 60.0) than in the Sahelian one (mean age = 66.7) (p less than 0.05). The CDK only exists among people age 40 or older, and is more frequent among males than females (p less than 0.005). The cataract is more frequent among individuals having a CDK than among those without it: relative risk = 4.3 (C.I.: 2.3-7.9 p less than 0.00001) (Mantel-Haenszel's method with adjustment to climate, age and sex). As for open-angle glaucoma, it is more frequent among individuals suffering from CDK: relative risk = 4.2 (p = 0.05). Finally, we noticed that the exfoliation syndrome was 5 times more frequent among chadians suffering from a CDK (p less than 0.02).(ABSTRACT TRUNCATED AT 250 WORDS)

Familial intracranial aneurysms, with two occurring at the distal anterior cerebral artery.

Familial aneurysms of the distal anterior cerebral artery are a neurosurgical rarity. We report a family with four of its members (a mother and three sons) suffering from subarachnoid hemorrhage secondary to ruptured aneurysms, two of them being of the distal anterior cerebral artery. There was no evidence of endocarditis, septic emboli, coarctation of the aorta, or trauma. There was no family history of Marfan's syndrome, polycystic kidney, or hereditary connective tissue disorder. We believe that there has been no report in the neurosurgical literature of familial cases of distal anterior cerebral artery aneurysms.

A new method for chronic intraventricular measurement of intracranial pressure.

A new simple method, which allows rapid access to the ventricular system to monitor intracranial pressure, is described. One of the lateral ventricles is punctured percutaneously, with a hand drill operated by a flash-light battery, and a lumbar puncture needle (20 gauge) attached to a water manometer or to a strain gauge connected to a venous pressure monitor.

Interhemispheric subdural empyema: angiographic diagnosis and surgical treatment.

Interhemispheric subdural empyema, secondary to frontal sinusitis in two girls is described. Headache, hemiparesis more marked in the lower extremity, fever, focal seizures, stupor and stiff neck were the principal features of the clinical course. The angiographic appearance of the lesion was the key to the preoperative diagnosis. Surgical evacuation of the purulent collection resulted in complete cure in both cases.

A new method for ventriculocentesis.

A new, safe, simple technique for percutaneous ventriculocentesis has been devised and used in 15 consecutive patients with excellent results and no complications. This method allows rapid access to the ventricular system for diagnostic and therapeutic purposes.
