[Proximal gastric vagotomy].

The incidence of uncomplicated peptic ulcer disease has markedly decreased all around the world over the last two decades. The medical and surgical management of patients with peptic ulcer disease, has also changed with the use of more effective drugs. Currently, the objectives of ulcer surgery are to eradicate the disease, to leave the gastrointestinal digestive apparatus as normal as possible, and to achieve these goals with the lowest possible mortality and recurrence rates. Proximal gastric vagotomy is currently the procedure of choice for a large number of gastric surgeons in the treatment of patients with chronic duodenal ulcer, who have failed medical therapy. Its application in treating the complications of peptic ulcer disease, which represent the greater percentage of all operations done for peptic ulcer disease, is less well-tested. However, initial series report good results in selected patients. Proximal gastric vagotomy without a drainage procedure, is the latest contribution to ulcer surgery. The operative mortality, is nearly zero, the recurrence rate of 4% to 11%, and there is virtual absence of significant side effects.

[Transduodenal sphincteroplasty].

Transduodenal sphincteroplasty is a surgical procedure indicated for treatment and prophylaxis of biliary tract stones and Oddi's sphincter fibrosis. This report analyzes the experience in Mexico City at Instituto Nacional de la Nutrición "Salvador Zubirán", in the last 35 years. We report the indications, morbidity and mortality rates, risk factors which have influence in the final outcome, as well as the long term effectivity. We used the chi-square analysis for measuring this correlation. Sphincteroplasty was effective for long term biliary permeability in 93%. Mortality rate was 4.3% and we had complications in 25.9%. In 150 biopsies, we found histological abnormalities in 90%. Presence of cholangitis was a risk factor for mortality, and serum albumin less than 3 g/dl for developing biliar or duodenal fistulas (p less than 0.02). Patients with chronic pancreatic disease in which sphincteroplasty was performed had poor outcome, compared with patients who had only biliary tract stones or Oddi's sphincter fibrosis. We concluded that transduodenal sphincteroplasty is an effective surgical procedure which supplies long term biliary tract permeability with a real present therapeutic value.

[Sporadic trichinosis].

A retrospective analysis was carried out to disclose the clinical and laboratory characteristics of 13 cases with sporadic trichinosis, proved by muscle biopsy at the "Salvador Zubirán" National Nutrition Institute and studied between 1977 and 1986. The number of cases per year was one or two with an increase to five in 1986. None of this patients showed a direct correlation between the type of diet or a specific area of the city. The clinical symptoms and physical findings were similar to those described previously for trichinosis, regardless of the clinical setting. In 69.1 percent of the cases the diagnosis was suspected based on clinical and physical data, and 84.6 percent had blood eosinophilia. Muscle biopsy was taken due to predominant muscular symptoms (92.3%) and this method was the most useful test to establish a specific diagnosis; no correlation was found between the number of larvae or cyst and the clinical symptoms. Thiabendazole and/or prednisone was administered and resulted in apparent improvement of the myopathy and blood eosinophilia during the follow-up (x: 7 months). A discussion is presented in relation to the usefulness of the muscle biopsy when a lack of specific enzyme tests exists; we emphasize the clinical manifestations and laboratory findings to suspect this zoonosis when a multisystemic inflammatory disease with striking myopathy is detected.

[A case of Brunner's gland hamartoma as a cause of digestive tract hemorrhage].

A twenty six year old woman was admitted suffering an anemia syndrome, postprandial vomiting and intermittent melena of six months evolution. The hemoglobin was 3.5 g/dL: an endoscopy detected a large tumor in the duodenum with a 90% obstruction of the lumen. A biopsy reported an erosive, acute and chronic duodenitis. Subsequently a surgical exploration with duodenotomy showed a large nodular polyp attached to a stalk that was removed: a hamartomatous polyp of Brunner's glands was reported. It coursed with gastrointestinal bleeding and symptoms of duodenal obstruction, which are two of the most common symptoms of this rare tumor.
